As internações por condições sensíveis à atenção primária representam condições de saúde que podem ter o risco de hospitalização desnecessária diminuído, por meio de ações efetivas da atenção primária. Essas internações vêm sendo usadas como indicador do acesso e qualidade da atenção básica, mas não existe consenso quanto às doenças que devem fazer parte desse indicador. Apresenta-se uma descrição das etapas seguidas para a construção da lista brasileira de internações por condições sensíveis à atenção primária. A lista final é composta por vinte grupos de diagnósticos, que representavam 28,3% das hospitalizações realizadas pelo Sistema Único de Saúde em 2006, em um total de cerca de 2,8 milhões de internações. Gastroenterites e complicações, insuficiência cardíaca e asma corresponderam a 44,1%. De 2000 a 2006, as internações por condições sensíveis à atenção primária caíram 15,8% no país, e o declínio dessas hospitalizações foi maior do que as internações não-internações por condições sensíveis à atenção primária. São discutidas as aplicações e limites da lista nacional de internações por condições sensíveis à atenção primária.
In 1994Brazil launched what has since become the world's largest community-based primary health care program. Under the Family Health Program, teams consisting of at least one physician, one nurse, a medical assistant, and four to six trained community health agents deliver most of their services at community-based clinics. They also make regular home visits and conduct neighborhood health promotion activities. This study finds that during 1999-2007, hospitalizations in Brazil for ambulatory care-sensitive chronic diseases, including cardiovascular disease, stroke, and asthma, fell at a rate that was statistically significant and almost twice the rate of decline in hospitalizations for all other causes. In municipalities with high Family Health Program enrollment, chronic disease hospitalization rates were 13 percent lower than in municipalities with low enrollment, when other factors were held constant. These results suggest that the Family Health Program has improved health system performance in Brazil by reducing the number of potentially avoidable hospitalizations.
The current analysis has pointed out the importance of clinical and health service characteristics as potential indicators of non-adherence after initiating therapy. Early assessment and intervention strategies should be priorities in these AIDS public referral centres. Feasible and reliable indicators for the routine monitoring of adherence should be incorporated in clinical practice.
Há uma tendência mundial de mudanças qualitativas na abordagem clínico-laboratorial do paciente em tratamento para o HIV/aids. A terapia anti-retroviral (TARV) no Brasil, como aconteceu em países industrializados do Hemisfério Norte que adotaram o acesso universal a esse tratamento, trouxe benefícios na sobrevida e redução da morbidade e mortalidade relacionada à doença. O presente artigo teve como objetivo apresentar uma revisão de literatura sobre o tema da adesão à TARV. Encontraram-se 56 artigos com as seguintes variáveis de classificação: fatores sociodemográficos; vulnerabilidade ao HIV; uso dos serviços de saúde; e TARV. A proposição principal extraída dos estudos é a necessidade de monitoramento contínuo dos pacientes. A fase inicial da terapia é crítica e, para criar vínculo com o paciente, é necessário acompanhamento individual, suporte social e informação sobre aids. Os serviços de referências para HIV/aids devem monitorar o início do tratamento no sentido de superar prováveis obstáculos, otimizar as boas experiências e incrementar a adesão aos anti-retrovirais.
To assess the understanding of information related to antiretroviral therapy among HIV-infected patients enrolled in public AIDS services (Belo Horizonte, Minas Gerais State, Brazil), a cross-sectional analysis was carried out, based on interviews with patients after initial provision of antiretroviral drugs. The study evaluated the information on antiretroviral therapy provided by healthcare professionals and the patients' level of understanding in relation to prescription information. This level was classified as insufficient if there was disagreement of more than 30.0% between the information reported by the patient and the written prescription. Divergence between prescriptions and information reported by 358 interviewed patients was observed. The level of understanding regarding the prescribed antiretroviral was obtained, and 26.3% of patients displayed insufficient understanding. The results show an important proportion of patients with misunderstanding of information regarding antiretroviral therapy, mainly those with limited schooling and low income. It is necessary to bolster strategies to increase quality of recommendations provided to these patients. Improving the multidisciplinary team approach to patient care should help reverse the observed situation.
The results of this study have important implications for the healthcare model in countries that base their national health systems on primary healthcare. This is the first study to evaluate the association between FHP coverage and PHCSC admissions rates in all the Brazilian states over a long and relevant period for the consolidation of FHP in the country.
OBJECTIVE To evaluate the effectiveness of antiretroviral therapy and the associated factors according to the type of regimen used: Single Tablet Regimen or Multiple Tablet Regimen.METHODS Prospective cohort of 440 patients (male, 74.3%, median age, 36 years old) who initiated antiretroviral therapy between Jan/14 and Dec/15 at a referral service in Belo Horizonte. Efficacy was defined as viral suppression (viral load, VL < 50 copies/ml) and evaluated after six and twelve months of treatment. Sociodemographic, clinical and behavioral data were collected from clinical charts and from Information Systems. Multivariate analysis of overall effectiveness was performed by logistic regression.RESULTS Most patients initiated Multiple Tablet Regimen antiretroviral therapy (n = 255, 58%). At six months, overall viral suppression was 74.6%, being higher among patients who used Single Tablet Regimen (80.6%, p = 0.04). At twelve months, 83.2% of patients reached viral suppression, with no difference between groups (p = 0.93). Factors independently associated with viral suppression at six and twelve months varied, being negatively associated with effectiveness: VL ≥ 100,000 copies/ml, symptoms of AIDS, longer interval time between diagnosis and initiation of antiretroviral therapy, antiretroviral switching, smoking or current illicit drugs usage (p < 0.05). Factors positively associated with viral suppression included adherence to antiretroviral therapy and category of risk/exposure of men who have sex with men (p < 0.05). Reaching viral suppression at six months was the main predictor of effectiveness at one year (OR = 8.96 and p < 0.01).CONCLUSIONS Viral suppression was high and better results were achieved for patients who used Single Tablet Regimen regimens at six months. Clinical, behavioral, and antiretroviral therapy -related factors influence viral suppression and highlight the need for interventions to increase early diagnosis and initiation of antiretroviral therapy, patient’s adherence, and to reduce illicit drugs and cigarette smoking in this population.
