Em um sentido mais amplo, incorpora dimensões que passam por aspectos econômicos, técnicos, políticos e simbólicos. Para doenças negligenciadas, como a hanseníase, por exemplo, reverte-se em maiores desafios. Este estudo objetivou caracterizar o acesso aos serviços de saúde por pessoas atingidas pela hanseníase em pós-alta de poliquimioterapia (PQT) nos municípios de Fortaleza e Sobral. Estudo transversal descritivo que incluiu 10% da população que recebeu alta entre 2003 e 2005 (304 pessoas), com caracterização sociodemográfica e clínica, bem como exploração de modalidades e contextos relativos ao acesso no pós-alta. A população era em sua maioria masculina (58%), com alta proporção de incapacidades físicas (34%) e estados reacionais (37%). A abordagem biomédica dos estados reacionais contemplou, em grande parte, a busca e a oferta de atenção nesse período. O acesso à atenção pós-alta foi frágil principalmente em intervenções como cirurgias reabilitadoras e atenção psicológica. Foram identificadas importantes lacunas em termos da operacionalização da atenção às pessoas atingidas no momento do pós-alta em ambos os municípios, o que traduz a relativa negligência em relação à hanseníase como condição crônica e o consequente distanciamento para a abordagem integral a essas pessoas no Sistema Único de Saúde (SUS).Palavras-chave: hanseníase; acesso aos serviços de saúde; atenção à saúde. AbstractUniversal access to health services represents a complex process of inter-relationships between users and health services. In a broader sense it includes economic, technical, political and symbolic aspects. For neglected diseases such as leprosy, this is a major challenge. This cross-sectional study aims to characterize access to health care services for people affected by leprosy after release from treatment in the cities of Sobral and Fortaleza. The study included a representative sample 10% of those concluding treatment from 2003 to 2005 (304 cases). Participants were characterized regarding socio demographic and clinical variables and access to health services after release from treatment. The study population was mostly male (58%), physical disabilities (34%) and with leprosy reactions (37%). The biomedical approach of leprosy reactions contemplated in large part to search for and supply of care in this period. The access to care after discharge was particularly weak in rehabilitative interventions such as surgery and psychological attention. We identified important gaps in the operationalization of attention to people affected after release from treatment in both cities. The date also shows the negligence of leprosy as a chronic condition and the resulting difficulties regarding on integrated distance to the comprehensive approach to people with leprosy after release from treatment in the Unified Health System (SUS).
Objetivo: Descrever a experiência vivenciada por enfermeiras em conexão com Agentes comunitários de saúde para enfrentamento local da pandemia COVID-19. Método: Relato de experiência desenvolvido a partir de ações de enfermeiras que atuam na Estratégia Saúde da Família do município de Icó, Estado do Ceará, destacando o importante papel que os Agentes comunitários de saúde desenvolvem junto à população nesse cenário. Resultados: A partir da experiência vivenciada, pode-se observar o importante papel que os Agentes Comunitários de Saúde têm apresentando frente à pandemia da COVD-19, mesmo apresentando medo e insegurança. Evidenciou-se ainda durante esse período que seu papel de educador em saúde é imprescindível nas ações desenvolvidas dentro dos territórios. Além disso, esses profissionais têm sido protagonistas importantes nas atividades realizadas dentro das Estratégias de Saúde da Família, fortalecendo assim o trabalho da equipe bem como o controle e combate da infecção pelo novo Coronavírus. Conclusões: Contar com o trabalho desses profissionais nos territórios tem sido estratégico e representa um diferencial para o cuidado, por auxiliarem no controle da disseminação e no monitoramento de grupos de risco, bem como das pessoas infectadas pela doença.Descritores: Enfermagem; Atenção Primária à Saúde; Agente Comunitário de Saúde; Infecção pelo Coronavírus. COMMUNITY HEALTH AGENTS IN FRONT OF COVID-19: EXPERIENCES WITH NURSING PROFESSIONALSObjective: To describe the experience lived by nurses in connection with community health workers to face the pandemic COVID-19 locally. Method: Experience report developed from the actions of nurses working in the Family Health Strategy in the municipality of Icó, State of Ceará, highlighting the important role that community health agents play with the population in this scenario. Results: Based on the experience, it is possible to observe the important role that Community Health Agents have playing in the face of the COVID-19 pandemic, even with fear and insecurity. It was also evident during this period that his role as a health educator is essential in the actions developed within the territories. In addition, these professionals have been important protagonists in the activities carried out within the Family Health Strategies, thus strengthening the team’s work as well as controlling and fighting infection by the new Coronavirus. Conclusions: Counting on the work of these professionals in the territories has been strategic and represents a differential for care, as they help in controlling the dissemination and monitoring at-risk groups, as well as people infected by the disease.Descriptors: Nursing; Primary Health Care; Community Health Agent; Corona virus infection. AGENTES DE SALUD DE LA COMUNIDAD FRENTE AL COVID-19: EXPERIENCIAS CON PROFESIONALES DE ENFERMERÍAObjetivo: Describir la experiencia vivida por las enfermeras en relación con los trabajadores de salud de la comunidad para enfrentar la pandemia COVID-19. Método: Informe de experiencia desarrollado a partir de las acciones de enfermeras que trabajan en la Estrategia de Salud de la Familia en el municipio de Icó, Estado de Ceará, destacando el importante papel que los trabajadores de salud comunitarios juegan con la población en este escenario. Resultados: En base a la experiencia, es posible observar el importante papel que los agentes de salud comunitarios tienen ante la pandemia da COVID-19, incluso con miedo e inseguridad. También fue evidente durante este período que su papel como educador de salud es esencial en las acciones desarrolladas dentro de los territorios. Además, estos profesionales han sido protagonistas importantes en las actividades llevadas a cabo dentro de las Estrategias de salud familiar, fortaleciendo así el trabajo del equipo y controlando y luchando contra la infección por el nuevo Coronavirus. Objective: to analyze the temporal trend and epidemiological patterns of leprosy indicators in Sobral, a municipality countryside of the state of Ceará, from 2001 to 2016. Method: a time series study based on data from the Department of Informatics of the Unified Health System. The time trend analysis was performed using the join point regression model. Results: There were 2,220 new cases of leprosy in Sobral from 2001 to 2016. Of these, 158 (7.2%) in children younger than 15 years of age, the proportion of new male cases was 52.8% (1,162), cases with grade 2 were 7.0% (156), and proportion of cases diagnosed by contact examination 5.7% (126). Final considerations: leprosy remains hyperendemic in adults and children, demonstrating the character of neglected disease. Analysis of the temporal trend allowed to verify that the instability in the detection coefficients, reflect operational problems in the organization of the services
The objective of this study was to report the experience in the light of Paulo Freire about an experience carried out in a village of residents of the municipality of Maracanaú-Ceará. It is a qualitative study of the experience report type. The experience was carried out from the discipline of popular education and school from the perspective of health promotion of the Postgraduate Program in Collective Health at the State University of Ceará. The visit took place in the second half of 2018, and residents were interviewed in order to understand the vision and perspective of popular education and health carried out in the environment in which they lived. Dialogic conversations were carried out using the conversation wheel strategy to record information. It can be seen that the lifestyle of the residents is shared in a way that enables the interaction (action) of everyone who lives in the environment in daily and financial activities. It is concluded that the non-formal environment as a teaching practice enables knowledge and knowledge in a shared way and maturation of the human being as a social being.
Objetivo: Nuestro objetivo es identificar en la literatura el conocimiento y las prácticas de los Agentes de Salud de la Comunidad (ACS) sobre promoción de la salud.Método: Esta es una revisión integradora realizada en las bases de datos MEDLINE / PUBMED, LILACS y BDENF, de artículos completos en inglés, portugués o español. Utilizamos el método de análisis temático con el uso de tablas para una mejor sistematización.Resultados / Discusión: Fue posible analizar que los procesos de trabajo de ACS están fuertemente asociados con la capacitación técnica. Se observan las brechas en la producción de conocimiento sobre promoción de la salud, como práctica diaria, acciones preventivas y distribución de insumos / medicamentos, recolección de información sociodemográfica y la situación de salud de la población, perdiendo así el espacio para la producción de atención basada en promoción de la salud.Conclusión: La creación de lazos y la bienvenida como una herramienta inseparable para el trabajo de la ACS se vuelve pulsante en las discusiones. Objective: We aim to identify in the literature the knowledge and practices of Community Health Workers (CHWs) on health promotion.Method: This is an integrative literature review carried out in the MEDLINE/PUBMED, LILACS and BDENF databases, with full text articles in English, Portuguese or Spanish. We used the Thematic Analysis method, using tables for better systematization.Results / Discussion: It was possible to analyze that the CHW work processes are strongly associated with technical training. There are gaps in the production of knowledge about health promotion as a daily practice, with the presence of preventive actions and distribution of commodities/medications and collection of socio-demographic information and of the health situation of the population, leaving the production of care based on health promotion on the background.Conclusion: The discussions highlight the creation of bonds and empathy, as inseparable tools in the work of the CHW.
Objectives: to understand how the therapeutic itineraries of people affected by leprosy are processed. Methods: this is a descriptive, qualitative study, conducted in April 2018 in Barão de Grajaú in Maranhão, with interviews in the form of narratives of seven patients who had a late leprosy diagnosis. Results: the search for diagnosis is a major difficulty in accessing health services, resulting in a late diagnosis and, consequently, with the presence of visible deformities. It was noticed that the health units do not have a flow, nor protocols for comprehensive treatment, and these people are referred to a referral unit in another state to perform sputum smear microscopy. Final Considerations: leprosy control actions need reformulations that seek the relationship between operational activities, epidemiological indicators and risk factors, in accordance with the real needs of each region, thus highlighting the gaps evidenced in the therapeutic itineraries.
Objetivo: Analisar as matérias publicadas pelo Conselho Federal de Enfermagem no transcurso da pandemia COVID-19 à luz da teoria da ação comunicativa habermasiana. Método: Estudo documental qualitativo, realizado durante o período de maio a julho de 2020, utilizando-se como fonte primária as notícias relacionadas à pandemia publicadas pela autarquia. O material foi organizado através da análise de conteúdo temática e discutido com base na teoria e literatura relacionada à temática. Resultados: O conselho produziu materiais notas técnicas como instrumentos norteadores aos profissionais e aprovou resoluções e normativas. A atuação do sistema esteve focada na aquisição e distribuição de equipamentos de proteção por intermédio dos regionais, ações judiciais, criação do observatório e de um canal de apoio emocional aos profissionais. Conclusão: Priorizou-se uma comunicação com sensibilização e conhecimento aos profissionais de enfermagem, empoderando-os nos processos de trabalho no contexto pandêmico.
Objetivo: analisar a gestão do processo de trabalho do enfermeiro nos serviços de vacinação. Método: trata-se de uma pesquisa de método misto. Foram aplicadas 18 entrevistas, e 203 questionários aos enfermeiros responsáveis pelas salas de vacinas, nos 14 municípios selecionados do estado do Ceará. Os dados foram coletados de julho a setembro de 2018, e analisadas as temáticas que envolvem o processo de trabalho do enfermeiro: acesso, acessibilidade e funcionamento das salas de vacinas; o cotidiano das salas de vacinação: rotinas e fluxos; e gestão e supervisão do processo de trabalho. Resultados: segundo os respondentes, o turno que possibilita maior acessibilidade à sala de vacinas é o da manhã, com 45,3%; 50,2% dos enfermeiros afirmaram que ofertam todas as vacinas do calendário básico de vacinação. Alguns demonstraram pouco conhecimento da rotina diária da sala de vacinação; 47,8% afirmaram ter limpeza semanal nas salas de vacinação; 25,1%, 9,4% e 17,7% relataram limpeza quinzenal, mensal e outras periodicidades, respectivamente; verificou-se que 74,4% dos profissionais realizam o monitoramento da temperatura das caixas térmicas ou refrigerador; em relação a organização dos materiais e impressos, 89,7% afirmaram realizar a organização destes. Quanto à supervisão e gerenciamento, 76,8% afirmaram não receber a supervisão municipal nas salas de vacinas, enquanto que apenas 23,2% recebem a supervisão da coordenação. Conclusão: verifica-se a não implicação do enfermeiro com as ações de imunização e seu pouco empoderamento. Sente-se como se as atividades de imunizações não fizessem parte de suas atividades.
