Key Clinical MessageThe splenic epidermoid cysts are rare benign tumors, and a definitive treatment remains unclear. Although some spleen‐preserving approaches have been reportedly used, splenic cyst recurrence usually occurs in true cyst cases, wherein the cyst is incompletely removed. In our case, partial splenectomy was performed and the giant cyst was completely removed.
Resumen
Introducción: la obesidad mórbida es hoy en día unos de los principales problemas de salud a nivel mundial y la gastrectomía vertical es la técnica quirúrgica bariátrica más utilizada para su tratamiento. En el presente artículo analizamos sus indicaciones, detalles técnicos, resultados y principales complicaciones, para intentar comprender el por qué se ha impuesto como la técnica quirúrgica más utilizada.
Material y método: nuestros pacientes son optimizados preoperatoriamente. El paciente es colocado con las piernas abiertas y en posición semisentada en la mesa de quirófano convencional. Habitualmente utilizamos cinco trócares y realizamos una gastrectomía vertical (GV) respetando el antro, tutorizada con sonda de Fouchet de 34 Fr, comenzando la sección gástrica a 6 cm del píloro, con endograpadora Flex de 60 mm mecanizada protegidas con Seamguard®. Se coloca un drenaje de Blake a lo largo de la sección gástrica y nunca dejamos sonda nasogástrica.
Discusión: se discuten las múltiples ventajas de la técnica, como son su estandarización, su corto tiempo operatorio, sus múltiples y variadas indicaciones (especialmente en pacientes de alto riesgo y en las edades extremas), el mantenimiento de la continuidad digestiva, haciendo accesible el tracto digestivo superior a exploraciones endoscópicas, su eficacia en cuanto a pérdida de peso y resolución de comorbilidades, su tasa aceptable de complicaciones, sin complicaciones nutricionales importantes a largo plazo y sus diferentes posibilidades de conversión en caso de cirugía de revisión. También se discuten los puntos débiles de la técnica, como son la fuga a nivel del ángulo de His, la falta de resultados a largo plazo y su relación con el reflujo gastroesofágico. Las ventajas superan los inconvenientes, por lo que se ha convertido en la técnica bariátrica más realizada en la actualidad.
Conclusiones: la GV es una técnica quirúrgica segura y eficaz, tanto en la pérdida de peso como en la resolución de comorbilidades, que mantiene la continuidad del tracto digestivo, con múltiples indicaciones, un número aceptable de complicaciones y con muchas posibilidades de conversión en caso de necesitar cirugía de revisión, motivos por lo que se ha hecho muy popular entre pacientes y cirujanos, hasta el punto de convertirse en el procedimiento bariátrico más realizado en la actualidad.
Resumen
Introducción: La endosalpingiosis en apéndice cecal es una patología benigna muy rara.
Caso clínico: Mujer de 39 años con dolor abdominal de 5 días de evolución acompañado de náuseas. Analíticamente destaca elevación de reactantes de fase aguda y en ecografía hallazgos compatibles con apendicitis aguda. Se realiza apendicectomía y en informe histológico se describe como hallazgo endosalpingiosis apendicular.
Discusión: En la literatura médica sólo existen publicados 8 casos de esta entidad infrecuente. Es raro que la endosalpingiosis afecte al apéndice cecal , siendo confundida con apendicitis aguda o con un tumor apendicular. Su diagnóstico preoperatorio es difícil. Por tanto, se presenta una revisión sistemática de la literatura descrita para esta patología tan específica e infrecuente, con posibilidad de transformación maligna.
