Resumen Introducción: La endosalpingiosis en apéndice cecal es una patología benigna muy rara. Caso clínico: Mujer de 39 años con dolor abdominal de 5 días de evolución acompañado de náuseas. Analíticamente destaca elevación de reactantes de fase aguda y en ecografía hallazgos compatibles con apendicitis aguda. Se realiza apendicectomía y en informe histológico se describe como hallazgo endosalpingiosis apendicular. Discusión: En la literatura médica sólo existen publicados 8 casos de esta entidad infrecuente. Es raro que la endosalpingiosis afecte al apéndice cecal , siendo confundida con apendicitis aguda o con un tumor apendicular. Su diagnóstico preoperatorio es difícil. Por tanto, se presenta una revisión sistemática de la literatura descrita para esta patología tan específica e infrecuente, con posibilidad de transformación maligna.
Resumen Introducción: El cáncer de mama metastásico se considera una enfermedad incurable. En los países desarrollados se calcula que entre el 3,5 y el 7% de los cánceres de mama son metastásicos al diagnóstico. En los países en vías de desarrollo pueden representar hasta el 25% de los diagnósticos. En los últimos años se ha descubierto un grupo de pacientes con metástasis limitadas que se podrían beneficiar de un abordaje quirúrgico en combinación con la terapia sistémica. Gracias a la mejora del diagnóstico por imagen este grupo de pacientes oligometastásicas está en aumento. Cirugía del tumor primario: Eliminar el tumor primario puede suponer una disminución de la carga tumoral y una sensibilización al tratamiento sistémico. Diversos estudios han puesto de manifiesto un aumento de la supervivencia en estos casos, siempre que se asocie terapia sistémica y en pacientes seleccionados. Cirugía de las metástasis: La resección de las metástasis en pacientes con respuesta completa o intervalo libre de enfermedad prolongado aumenta la supervivencia global. Se debe proponer metastasectomía en toda mujer con enfermedad M1 limitada, con edad menor de 55 años, ECOG <2, tumores RE/RP o HER2 positivos y grado favorable. Efecto abscopal e inmunoterapia: El efecto abscopal permite el control local y sistémico de la enfermedad tumoral. La radiación local es capaz de mejorar la respuesta sistémica alterando los mecanismos de defensa tumorales a nivel local. Conclusiones: En pacientes M1 seleccionados, la intervención sobre el tumor primario y/o sobre enfermedad metastásica limitada, puede cambiar el pronóstico global de la paciente.
