Abstract. On the occasion of the twentieth anniversary of the 'Belgian Association of Burn Injuries' an inventory was made of all surgical techniques used in the five largest Belgian burn centres in order to draw up a consensus document. A questionnaire covering the whole treatment of severely burned patients was sent to the surgeons of each burn unit, ranging from emergency treatment, through diagnostic techniques, burn surgery and post-healing treatment (scars, contractures). During the past decade, several (often expensive) new techniques have been commercialised, resulting in a wide variety of treatment modalities in the different centres. For the majority of burns, however, a similar protocol is followed : the superficial burns are treated conservatively and the deep burns surgically. Nevertheless, burns of indeterminate depth are susceptible to variations in their treatment. There are also different therapeutic options for the deepest burns, from the use of donor skin to dermal substitutes as well as cultured keratinocytes. The writing of this consensus required intense communication on the different therapeutic modalities, as well as an increased exchange of experiences, which could lead to an improvement in the quality of burn treatment in Belgium.
Key Clinical MessageThe splenic epidermoid cysts are rare benign tumors, and a definitive treatment remains unclear. Although some spleen‐preserving approaches have been reportedly used, splenic cyst recurrence usually occurs in true cyst cases, wherein the cyst is incompletely removed. In our case, partial splenectomy was performed and the giant cyst was completely removed.
Resumen Introducción: La endosalpingiosis en apéndice cecal es una patología benigna muy rara. Caso clínico: Mujer de 39 años con dolor abdominal de 5 días de evolución acompañado de náuseas. Analíticamente destaca elevación de reactantes de fase aguda y en ecografía hallazgos compatibles con apendicitis aguda. Se realiza apendicectomía y en informe histológico se describe como hallazgo endosalpingiosis apendicular. Discusión: En la literatura médica sólo existen publicados 8 casos de esta entidad infrecuente. Es raro que la endosalpingiosis afecte al apéndice cecal , siendo confundida con apendicitis aguda o con un tumor apendicular. Su diagnóstico preoperatorio es difícil. Por tanto, se presenta una revisión sistemática de la literatura descrita para esta patología tan específica e infrecuente, con posibilidad de transformación maligna.
Resumen Introducción: El linfangioma quístico es una malformación benigna y normalmente congénita de los vasos linfáticos. Presentamos el caso de linfangioma quístico raro tanto por su localización inguinal (menos del 2% de todos los linfangiomas) como por su debut tardío en el adulto (el 50% son congénitos y hasta un 90% se diagnostican antes de los dos años). Caso clínico: Se trata de una mujer de 50 años que tiene una masa de consistencia blanda y móvil en región inguinal derecha no reducible y que tampoco protruye con maniobras de Valsalva. Se solicita ecografía abdominal en la que se observa una formación ovalada de naturaleza quística. Se realiza exéresis de la misma confirmando el diagnóstico anatomopatológico de linfangioma quístico. Discusión: El linfangioma suele producir síntomas por compresión de estructuras vecinas o por complicación del mismo. El gold estándar para el diagnóstico es la ecografía, aunque el diagnóstico definitivo es anatomopatológico. Se aconseja la escisión completa del mismo por el riesgo de recidiva.
Presentamos el caso de un paciente con síndrome de Marfan y varias cirugías de aorta por dicha patología, que sufre de una fístula aortoesofágica, por lo que se traslada a nuestro hospital para tratamiento urgente conjunto por parte de cirugía general y cirugía cardíaca. Sin embargo, fallece antes de poder llevarlo a quirófano.
