Resumen Introducción: la obesidad mórbida es hoy en día unos de los principales problemas de salud a nivel mundial y la gastrectomía vertical es la técnica quirúrgica bariátrica más utilizada para su tratamiento. En el presente artículo analizamos sus indicaciones, detalles técnicos, resultados y principales complicaciones, para intentar comprender el por qué se ha impuesto como la técnica quirúrgica más utilizada. Material y método: nuestros pacientes son optimizados preoperatoriamente. El paciente es colocado con las piernas abiertas y en posición semisentada en la mesa de quirófano convencional. Habitualmente utilizamos cinco trócares y realizamos una gastrectomía vertical (GV) respetando el antro, tutorizada con sonda de Fouchet de 34 Fr, comenzando la sección gástrica a 6 cm del píloro, con endograpadora Flex de 60 mm mecanizada protegidas con Seamguard®. Se coloca un drenaje de Blake a lo largo de la sección gástrica y nunca dejamos sonda nasogástrica. Discusión: se discuten las múltiples ventajas de la técnica, como son su estandarización, su corto tiempo operatorio, sus múltiples y variadas indicaciones (especialmente en pacientes de alto riesgo y en las edades extremas), el mantenimiento de la continuidad digestiva, haciendo accesible el tracto digestivo superior a exploraciones endoscópicas, su eficacia en cuanto a pérdida de peso y resolución de comorbilidades, su tasa aceptable de complicaciones, sin complicaciones nutricionales importantes a largo plazo y sus diferentes posibilidades de conversión en caso de cirugía de revisión. También se discuten los puntos débiles de la técnica, como son la fuga a nivel del ángulo de His, la falta de resultados a largo plazo y su relación con el reflujo gastroesofágico. Las ventajas superan los inconvenientes, por lo que se ha convertido en la técnica bariátrica más realizada en la actualidad. Conclusiones: la GV es una técnica quirúrgica segura y eficaz, tanto en la pérdida de peso como en la resolución de comorbilidades, que mantiene la continuidad del tracto digestivo, con múltiples indicaciones, un número aceptable de complicaciones y con muchas posibilidades de conversión en caso de necesitar cirugía de revisión, motivos por lo que se ha hecho muy popular entre pacientes y cirujanos, hasta el punto de convertirse en el procedimiento bariátrico más realizado en la actualidad.
Resumen Introducción: La endosalpingiosis en apéndice cecal es una patología benigna muy rara. Caso clínico: Mujer de 39 años con dolor abdominal de 5 días de evolución acompañado de náuseas. Analíticamente destaca elevación de reactantes de fase aguda y en ecografía hallazgos compatibles con apendicitis aguda. Se realiza apendicectomía y en informe histológico se describe como hallazgo endosalpingiosis apendicular. Discusión: En la literatura médica sólo existen publicados 8 casos de esta entidad infrecuente. Es raro que la endosalpingiosis afecte al apéndice cecal , siendo confundida con apendicitis aguda o con un tumor apendicular. Su diagnóstico preoperatorio es difícil. Por tanto, se presenta una revisión sistemática de la literatura descrita para esta patología tan específica e infrecuente, con posibilidad de transformación maligna.
Resumen La infección por Anisakidosis ha ido aumentando de forma insidiosa en España llegando a ser el segundo país con la incidencia más alta. Este tipo de infecciones no solo aparece por viajes a países extranjeros sino que también puede darse por ingesta de pescado crudo. Suele ser una infección insidiosa, sin síntomas específicos y es por ello que hay que tener una alta sospecha para su diagnóstico, sobretodo en los casos en los que se presenta en forma de urgencia intestinal como es nuestro caso. El órgano más comúnmente afectado es el estómago pero también puede afectar al intestino delgado, causando cuadros que simulan obstrucción intestinal e incluso perforación. Al no existir una prueba Gold estándar de urgencia, muchas veces será necesaria una laparotomía exploradora urgente, obteniendo el diagnóstico definitivo tras los hallazgos anatomopatológicos. Existen pocos casos en la literatura de anisakidosis intestinal, no obstante, creemos que hay que considerarla en los diagnósticos diferenciales ante cuadros de obstrucción intestinal dada su incidencia creciente. Presentamos el caso de un paciente de 37 años con cuadro de Obstrucción intestinal causada por una infección por Anisakis.
Resumen El mesotelioma maligno es un tumor poco frecuente pero está asociado a una alta mortalidad y se considera por tanto un tumor muy agresivo con una incidencia que va aumentando en todo el mundo. Se origina más frecuentemente en las membranas serosas pleurales, seguido del peritoneo, pericardio y la túnica vaginalis,suponiendo los mesoteliomas peritoneales un 10-15% de todos los casos de mesotelioma. La TAC es la prueba diagnóstica de elección pero para el diagnóstico definitivo es necesaria la inmunohistoquímica. El tratamiento del mesotelioma peritoneal maligno precisa un tratamiento multimodal por lo que es necesario un comité multidisciplinar. El tratamiento estándar para pacientes seleccionados con mesotelioma maligno incluye cirugía citorreductora y quimioterapia hipertérmica intraperitoneal, mientras que los mesoteliomas irresecables se pueden beneficiar de tratamiento combinado con quimioterapia, radioterapia e inmunoterapia. Actualmente hay varios ensayos clínicos para conocer mejor las características de dicha neoplasia.
Resumen Introducción: el adenocarcinoma de páncreas constituye la cuarta causa de mortalidad en España y en Estados Unidos, siendo la resección quirúrgica el único tratamiento que logra aumentar la supervivencia. Se trata siempre de un reto para el cirujano debido a la dificultad técnica del procedimiento, y a pesar de que la mortalidad, se ha reducido en los últimos años, aún es bastante elevada las complicaciones quirúrgicas. Métodos: el proceso de investigación se ha realizado a partir de una búsqueda sistemática a partir de guías clínicas, conferencias de consenso, revisiones y publicaciones de ámbito nacional e internacional sobre cirugía pancreática oncológica realizada en la última década. Resultados: se ha determinado la necesidad de establecer un Equipo Multidisciplinar para la toma de decisiones. La necesidad de superar una curva de aprendizaje en este tipo de cirugía tan exigente para disminuir la morbimortalidad. La importancia de establecer unos estándares de calidad para valorar la calidad del tratamiento prestado, homogeneizar la metodología de trabajo poder medir resultados y poder comparar éstos con otros grupos y unidades especializadas y la importancia de establecer protocolos de recuperación de mejora para una mejora rápida y precoz de los pacientes sometidos a cirugía pancreática. Conclusión: La resección oncológica pancreática es una intervención de gran complejidad técnica y con una elevada incidencia de complicaciones postoperatorias, por lo que la necesidad de tomas de decisiones de equipos expertos, identificación de estándares de calidad y la necesidad de aplicar unos protocolos comunes constituyen elementos de gran importancia para disminuir la variabilidad en la práctica quirúrgica, mejorar los resultados y obtener una máxima optimización de los recursos disponibles.
