OBJECTIVE:To evaluate the quality of information registered on fetal death certifi cates. METHODS:Records were reviewed from 710 fetal deaths registered in the consolidated database of deaths from the State System for Data Analysis and the São Paulo State Secretary of Health, for deaths in São Paulo municipality (Southeastern Brazil) during the fi rst semester of 2008. Completeness was analyzed for variables on fetal death certifi cates issued by hospitals and autopsy service. The death certifi cates from a sub-sample of 212 fetal deaths in hospitals of the National Unifi ed Health System (public) were compared to medical records and to the records from Coroner's Offi ce. RESULTS:Among death certifi cates, 75% were issues by Coroner's Offi ce, with Coroner's greater frequency in public hospitals (78%). Completeness of variables on death certifi cates issued by hospitals was higher among nonpublic hospitals. There was greater completeness, agreement and sensitivity in death certifi cates issued by hospitals. There was low agreement and high specifi city for variables related to maternal characteristics. Increased reporting of gender, birth weight and gestational age was observed in certifi cates issued by Coroner's Offi ce. Autopsies did not result in improved ascertainment of cause of death, with 65.7% identifi ed as unspecifi ed fetal death as 24.3% as intrauterine hypoxia, while death certifi cates by hospitals reported 18.1% as unspecifi ed and 41.7% as intrauterine hypoxia. CONCLUSIONS:Completeness and the ascertainment of cause of fetal death need to be improved. The high proportion of autopsies did not improve information and ascertainment of cause of death. The quality of information generated by autopsies depends on access to hospital records.
O monitoramento da mortalidade perinatal depende da qualidade dos dados dos sistemas de informação. As diferentes definições para a notificação e cálculo da mortalidade perinatal podem afetar a magnitude e análise dos seus componentes. Comparou-se a disponibilidade de informações sobre nascidos vivos, óbitos fetais e neonatais precoces no Registro Civil, SIM e SINASC para oito unidades da federação com cobertura de eventos acima de 90%, em 2002. Verificou-se que o SINASC apresenta maior cobertura de eventos que o registro civil e excelente completude de dados (superior a 99%). O SIM apresenta situação distinta, há elevada ausência de informação sobre peso ao nascer (23,4%), idade gestacional (9,1%), idade da mãe (18,5%), tipo de gravidez (13,8%) e anos de estudo da mãe (40,6%), para os óbitos fetais. Os óbitos neonatais precoces apresentam comportamento semelhante, com ausência do registro do peso ao nascer em 22,6%, idade gestacional (17,8%), tipo de gravidez (19,1%), idade (27,9%) e escolaridade da mãe (38,5%). Não foi possível caracterizar se os óbitos fetais eram intra-parto ou ante-parto por falta de informação. No entanto, estes dados poderiam ser facilmente obtidos, pois mais de 95% dos eventos ocorreram em estabelecimentos hospitalares. Os critérios para notificação de óbitos fetais e nascidos vivos nos sistemas de informação dificultam a comparação internacional da magnitude e da participação de seus componentes da mortalidade perinatal. A ausência de informações compromete a obtenção de indicadores específicos, dificultando as atividades de monitoramento. Algumas atividades são indicadas para o aprimoramento do SIM.
O objetivo foi analisar o perfil dos recém-nascidos, mães e mortalidade neonatal precoce, segundo complexidade do hospital e vínculo com o Sistema Único de Saúde (SUS), na Região Metropolitana de São Paulo, Brasil. Estudo baseado em dados de nascidos vivos, óbitos e cadastro de hospitais. Para obter a tipologia de complexidade e o perfil da clientela, empregaram-se análise fatorial e de clusters. O SUS atende mais recém-nascidos de risco e mães com baixa escolaridade, pré-natal insuficiente e adolescentes. A probabilidade de morte neonatal precoce foi 5,6‰ nascidos vivos (65% maior no SUS), sem diferenças por nível de complexidade do hospital, exceto nos de altíssima (SUS) e média (não-SUS) complexidade. O diferencial de mortalidade neonatal precoce entre as duas redes é menor no grupo de recém-nascidos < 1.500g (22%), entretanto, a taxa é 131% mais elevada no SUS para os recém-nascidos > 2.500g. Há uma concentração de nascimentos de alto risco na rede SUS, contudo a diferença de mortalidade neonatal precoce entre a rede SUS e não-SUS é menor nesse grupo de recém-nascidos. Novos estudos são necessários para compreender melhor a elevada mortalidade de recém-nascidos > 2.500g no SUS.
Conclui-se que os maiores riscos de morte neonatal concentramse nos primeiros dias de vida e correspondem a recém-nascidos de baixo peso e de gestações de pré-termo, sendo muito elevado o número de mortes que ocorrem por doenças possíveis de evitar. São Paulo, SP, 1999.A cohort of live-boms and the correspondent under 28 days' deaths is described and the neonatal mortality rate is analyzed in the State of São Paulo, estimating death risks by the characteristics of the new-bom, of the pregnancy and the childbirth, of the mother, and death causes, according to the child age at dying. The linkage technique has been used in order to identify the deaths that occurred at the live-bom cohort, by chaining the death reports to the correspondent live-bom reports.Out of 167.174 researched live-boms, 51.2% were from the male gender; 9.2% were bom under-weight; 49.1% correspond to operatory childbirths; 17.1% were from teenage mothers; and 59.2% were from mothers that have not completed elementary school. Besides that, most of the 2.955 concatened deaths occurred at the early neonatal period (81.9%), where 39.6% of them were concentrated at the first 24 hours of living; while more than 70% of the deaths were caused by diseases classified as avoidable. The neonatal death probability has reached 17.7 per thousand, from which 14.5 per thousand corresponds to the early neonatal period and 3.2 per thousand to the late neonatal period. During the first 24 hours of living the very under weight new-born presented 26.6 times more chance to dying than the normal weight new-born; while among the pre-term live-born the death probability is 35.6 times bigger than the term pregnancy live-borns. O objetivo foi descrever as características do recém-nascido, da mãe e da mortalidade neonatal precoce, segundo local de parto, na Região Metropolitana de São Paulo, Brasil. Utilizou-se coorte de nascidos vivos vinculados aos respectivos óbitos neonatais precoces, por técnica determinística. Identificou-se o parto domiciliar a partir da Declaração de Nascido Vivo e os ocorridos em estabelecimentos a partir da vinculação com o Cadastro Nacional de Estabelecimentos de Saúde. Foram estudados 154.676 nascidos vivos, dos quais 0,3% dos nascimentos ocorreram acidentalmente em domicílio, 98,7% em hospitais e menos de 1% em outro serviço de saúde. A mortalidade foi menor no Centro de Parto Normal e nas Unidades Mistas de Saúde, condizente com o perfil de baixo risco obstétrico. As taxas mais elevadas ocorreram nos prontos-socorros (54,4 óbitos por mil nascidos vivos) e domicílios (26,7), representando um risco de morte, respectivamente, 9,6 e 4,7 vezes maior que nos hospitais (5,6). Apesar da alta predominância do parto hospitalar, há um segmento de partos acidentais tanto em domicílios como em prontos-socorros que merece atenção, por registrar elevadas taxas de mortalidade neonatal precoce.
Results The factorial analyses indicated a two-dimensional structure (perception of work ability/mental resources and diseases and health restrictions). The value of Cronbach's a and McDonald u was, respectively, 0.80 and 0.87. The theoretical hypothesis of the construct validity were confirmed with direct correlation and significant of the WAI with the scores of reward, control and selfevaluate of the health status; inverse correlation and significant with the scale of need for recovery, psychological distress, effort, overcommitment and demand. Conclusion We found good evidence for a high reliability and construct validity of WAI questionnaire, supporting its use in future analyses in similar populations.
