The prevalence of common mental disorders has increased in many countries. Cases are often not identified and adequately treated because traditional health care services are rarely prepared to deal with this problem. The Family Health Program (FHP) has been implemented in Brazil since 1995-1996 and provides a new primary health care model with the potential for better care for these patients. This study investigates common mental disorders prevalence according to FHP coverage and associated socio-demographic factors. A large health and health care survey was conducted from January to March 2001 in areas partly covered by the FHP in a peripheral area of the city of Sao Paulo and included common mental disorders screening in 2,337 individuals > 15 years of age. There was no significant difference in common mental disorders prevalence according to FHP. Common mental disorders prevalence was significantly higher among females (PR = 1.34), elderly (PR = 1.56), and individuals with lower income (PR = 2.64) or less schooling (PR = 2.83). Common mental disorders was associated with indicators of social disadvantage, implying the need to focus on specific health problems and risk groups to improve the impact of care.
The Strategic Advisory Group of Experts (SAGE) on Immunization advises WHO on global policies for vaccines. In April, 2016, SAGE issued recommendations on the use of the first licenced dengue vaccine, CYD-TDV. In November, 2017, a retrospective analysis of clinical trial data, stratifying participants according to their dengue serostatus before the first vaccine dose, showed that although in high seroprevalence settings the vaccine provides overall population benefit, there was an excess risk of severe dengue in seronegative vaccinees. SAGE's working group on dengue vaccines met to discuss the new data and mainly considered two vaccination strategies: vaccination of populations with dengue seroprevalence rates above 80% or screening of individuals before vaccination, and vaccinating only seropositive individuals. We report on the deliberations that informed the recommendation of the pre-vaccination screening strategy, in April, 2018. Important research and implementation questions remain for CYD-TDV, including the development of a highly sensitive and specific rapid diagnostic test to determine serostatus, simplified immunisation schedules, and assessment of the need for booster doses.
INTRODUÇÃO: A mortalidade neonatal no Município de São Paulo, apesar da sua tendência decrescente, constitui em um importante problema para a saúde pública. Os principais fatores de risco podem ser agrupados em quatro categorias básicas de variáveis: características do recém-nascido, características maternas, condições socioeconômicas e características dos serviços de saúde. O peso ao nascer e a prematuridade constituem fatores dominantes, compondo complexas redes de articulação com os demais. METODOLOGIA: Este é um estudo caso-controle, com base em dados vinculados do SIM e SINASC no Município de São Paulo, no primeiro semestre de 1995. Foi utilizada análise hierárquica, considerando quatro blocos de variáveis (características socioeconômicas, do recém-nascido, maternas e serviços de saúde) para o conjunto de recém-nascidos e para três grupos de peso ao nascer: <1.500g, 1.500- 2.499g e 2.500g e mais. RESULTADOS: No modelo final para o conjunto de recém-nascidos mostraram associação com óbito neonatal, a idade materna inferior a de 20 anos, nascer em hospital vinculado ao SUS, peso ao nascer <2.500g e prematuridade. Os três grupos de peso ao nascer exibiram perfis distintos de fatores de risco. Os RN <1.500g tiveram menor mortalidade quando nasceram em hospital universitário e por cesárea. Nos RN de 1.500-2.499g a mortalidade estava associada a prematuridade, sexo masculino e mãe grande multípara. No grupo com peso acima de 2.500g, os fatores de risco para mortalidade foram a prematuridade, mãe adolescente, morar em área com qualidade de vida precária e nascer por cesárea e em hospital vinculado ao SUS. CONCLUSÃO: Esse estudo, além de apontar a enorme influência do peso ao nascer sobre a mortalidade neonatal, no MSP, demonstrou que as diferentes categorias de peso apresentam vulnerabilidades biológicas e sociais distintas, que constituem articulações complexas, e que os serviços de saúde têm um importante papel a desempenhar em cada uma delas.
OBJECTIVE:To assess risk factors for early neonatal mortality.
METHODS:A population-based case-control study was carried out with 146 early neonatal deaths and a sample of 313 controls obtained among survivals of the neonate period in the south region of the city of São Paulo, in the period of 8/1/2000 to 1/31/2001. Information was obtained through home interviews and hospital charts. Hierarchical assessment was performed in fi ve groups with the following characteristics 1) socioeconomic conditions of mothers and families, 2) maternal psychosocial conditions, 3) obstetrical history and biological characteristics of mothers, 4) delivery conditions, 5) conditions of newborns.
RESULTS:Risk factors for early neonate mortality were: Group 1: poor education of household head (OR=1.6; 95% CI: 1.1;2.6), household located in a slum area (OR=2.0; 95% CI: 1.2;3.5) with up to one room (OR=2.2; 95% CI: 1.1;4.2); Group 2: mothers in recent union (OR=2.0; 95% CI: 1.0;4.2), unmarried mothers (OR=1.8; 95% CI: 1.1;3.0), and presence of domestic violence (OR=2.7; 95% CI: 1;6.5); Group 3: presence of complications in pregnancy (OR=8.2; 95% CI: 5.0;13.5), previous low birth weight (OR=2.4; 95% CI: 1.2;4.5), absence of pre-natal care (OR=16.1; 95% CI: 4.7;55.4), and inadequate pre-natal care (block 3) (OR=2.1; 95% CI: 2.0;3.5); Group 4: presence of clinical problems during delivery (OR=2.9; 95% CI: 1.4;5.1), mothers who went to hospital in ambulances (OR=3.8; 95% CI: 1.4;10.7); Group 5: low birth weight (OR=17.3; 95% CI: 8.4;35.6) and preterm live births (OR=8.8; 95% CI: 4.3;17.8).
CONCLUSIONS:Additionally to proximal factors (low birth weight, preterm gestations, labor complications and unfavorable clinical conditions in gestation), the variables expressing social exclusion and presence of psychosocial factors were also identifi ed. This context may affect the development of gestation and hinder the access of women to health services. Adequate prenatal care could minimize the effect of these variables.
