Objectives: to validate nursing care effectiveness indicators of patient safety dimension. Methods: quantitative survey, using the electronic Delphi sampli, with 52 participants selected by the Snowball sampling. Eight indicators were evaluated regarding the attributes: availability, reliability, simplicity, representativeness, sensitivity, comprehensiveness, objectivity, cost, utility, stability and timeliness. For validation, the minimum agreement criterion was 70%. Results: Cronbach’s alpha (0.942) evidenced the high internal consistency among the attributes. The indicators fall with damage, hip fracture, and postoperative hip fracture, incidents related to equipment, incidents due to failures in patient identification, and pressure injury were validated in all attributes, and those of medication error and hand Hygiene were not validated. Conclusions: the validated indicators allow assessment of the effectiveness of hospital nursing care. Unavailability of data is an obstacle to monitoring patient safety.
Objective: to analyze the safety culture in surgical units of a teaching hospital in different periods of hospital management. Method: this is a descriptive and analytical study developed with health professionals working at a surgical center and at five surgical inpatient units in two different periods of hospital management in southern Brazil. The Hospital Survey on Patient Safety Culture questionnaire was answered by 73 professionals in period I (2014/2015) and by 158 professionals in period II (2017). The analysis was based on descriptive statistics and on inferential analysis and reliability analysis using Cronbach's alpha coefficient; the dimensions were considered strong when the percentage of positive answers was ≥75%. Results: the administrative transition had a positive influence on the “Staffing” and “Handoffs and transitions” dimensions and a negative influence on “Teamwork within units” (p<0.001), with no difference in the remaining nine dimensions. In both periods, no dimension was considered strong; there were no changes with regard to the notification of adverse events and to the overall safety assessment; and the overall reliability of the instrument was satisfactory (0.89). Conclusion: the administrative transition of hospital management had little impact on the dimensions of the organizational safety culture, and there are still challenges in the progressive development of this predictor of patient safety.
Pesquisa documental sobre a trajetória de um hospital público de ensino na construção de Programa de Segurança do Paciente. Foram consultados 200 documentos do período de 2004 a 2012, em um hospital público federal do sul do Brasil, com dados coletados entre maio de 2012 a abril de 2013. A análise documental evidencia estratégias essenciais à implantação de um programa. Entre elas, diagnóstico situacional com utilização de ferramentas da qualidade; auditorias internas com instrumentos customizados; profissionais com dedicação exclusiva e expertise na área da qualidade; capacitações direcionadas à equipe multiprofissional; incentivo à construção coletiva, o apoio da direção e o envolvimento de lideranças. O programa foi vanguardista entre os hospitais públicos federais. Desenvolvido previamente à legislação brasileira que normatiza a matéria, cumpriu em grande parte com as demandas da Resolução de Diretoria Colegiada n° 36 de 25 de julho de 2013 do Ministério da Saúde.
Background: Indicators of effectiveness in the dimension of patient-centered care. Objective: To validate indicators for determining attributes that make it possible to measure the effectiveness of hospital care in the dimension of patient-centered care. Methodology: Quantitative survey type study, carried out in 2017 to judge eight indicators, considering 11 attributes, by 52 specialists, using the Delphi technique and the electronic Survey Monkey® platform. The internal consistency was tested by Cronbach's Alpha 0,98. A valid indicator was considered when there was a minimum consensus of 70% between the responses of the participants, in all attributes. Results: Four indicators were validated: patient satisfaction; surgeries canceled on the scheduled day; hospital recommendation; and the patient's involvement in their own care, with high internal consistency (0.98). Conclusion: The validated indicators favor the evaluation of the effectiveness of hospital care oriented to the valorization of the person as a care center. Non-validated indicators are the subject of future studies. Objetivo: Validar indicadores quanto à deteção de atributos que possibilitem medir a efetividade da assistência hospitalar na dimensão do cuidado centrado no doente. Metodologia: Estudo quantitativo do tipo survey, realizado em 2017 para julgamento de 8 indicadores, considerando 11 atributos, por 52 especialistas, utilizando-se técnica Delphi e a plataforma eletrónica Survey Monkey®. A consistência interna foi testada pelo alfa de Cronbach de 0,98. Considerou-se indicador válido quando houve concordância mínima de 70% entre as respostas dos participantes, em todos os atributos. Resultados: Foram validados 4 indicadores (satisfação do doente; cirurgias canceladas no dia agendado; recomendação do hospital pelo doente; e envolvimento do doente com o próprio cuidado), com alta consistência interna (0,98). Conclusão: Os indicadores validados favorecem a avaliação da efetividade da assistência hospitalar orientada para a valorização da pessoa enquanto centro do cuidado. Os indicadores não validados são objeto de estudos futuros.
