Die vorliegende Expertise hat zwei Zielsetzungen: <OL><LI>Überblick über Therapieangebote für Alkohol- und Drogenabhängige und die Dauer der Therapie in Deutschland und in ausgewählten europäischen Staaten (Frankreich, Italien, Großbritannien, Niederlande und Schweden) <LI>Darstellung der Therapieerfolge in Europa und Analyse des Zusammenhangs zwischen Therapiedauer und Therapieerfolg einschließlich der Berücksichtigung zusätzlicher Einflussvariablen</OL>
Fragestellung: Bildung stellt einen wesentlichen Faktor für das Gesundheitsverhalten dar. Während hohe Bildung als protektiver Faktor für die Gesundheit gesehen wird, scheint niedrige Bildung mit gesundheitsriskanterem Verhalten verbunden zu sein. Substanzkonsum gilt als eine dieser gesundheitsschädlichen Verhaltensweisen. Klienten aus ambulanten und stationären Suchthilfeeinrichtungen werden hinsichtlich ihres Bildungsstatus und den Hauptdiagnosen untersucht, um der Frage nachzugehen, inwieweit sich diese Zusammenhänge auch innerhalb der untersuchten Population widerspiegeln. Als globales Maß wird dabei der Zusammenhang zwischen Bildungsstand und Behandlungserfolg betrachtet. Methodik: Datengrundlage bilden die Routinedaten der Deutschen Suchthilfestatistik (DSHS) 2010, zwei Sonderauswertungen der DSHS sowie der Mikrozensus der deutschen Bevölkerung 2010. Innerhalb der DSHS-Daten werden zwei Gruppen (hoch und niedrig gebildete Personen) untersucht. Ergebnisse: Im Vergleich zur Allgemeinbevölkerung zeigt sich in der DSHS ein höherer Anteil von Personen mit niedrigem Bildungsstand. Innerhalb der untersuchten Population weisen Personen mit niedrigem Bildungsstand häufiger Hauptdiagnosen im Bereich illegaler Substanzen auf, während bei Personen mit hohem Bildungsstand Hauptdiagnosen im Bereich legaler Substanzen häufiger sind. Ein hohes Bildungsniveau ist zudem mit besseren Behandlungsergebnissen und geringeren Behandlungsabbrüchen assoziiert. Schlussfolgerungen: Die Ergebnisse sprechen für Bildung als wichtigen Faktor im Zusammenhang mit Substanzabhängigkeit. Unterschiede in der Bildungsverteilung innerhalb der Hauptdiagnosegruppen sowie Unterschiede im Verlauf und Ergebnis der Behandlung in Abhängigkeit des Bildungsstandes erweisen sich hier als bedeutsam für die Versorgung von Personen in Einrichtungen der Suchthilfe.
Fragestellung: Darstellung von Behandlungs- und Patientenmerkmalen ambulanter und stationärer Einrichtungen der Suchtkrankenhilfe in Deutschland für das Jahr 2009. Methodik: Die Auswertung basiert auf aggregierten Daten von N = 936 (779 ambulanten und 157 stationären) Einrichtungen, die sich im Berichtsjahr 2009 an der DSHS beteiligt und 316.075 Betreuungen in ambulanten und 38.618 Behandlungen in stationären Einrichtungen dokumentiert haben. Voraussetzung für die Teilnahme der Einrichtungen an der DSHS ist eine Dokumentation der Arbeit entsprechend der Vorgaben des KDS (Deutscher Kerndatensatz). Auf der Grundlage des am IFT geführten Registers der Facheinrichtungen der Suchtkrankenhilfe in Deutschland kann für den ambulanten Bereich eine Erreichungsquote von 69 bis 74 % angenommen werden, für den stationären Bereich von 43 bis 48 %. Ergebnisse: Wie bereits in den Vorjahren stellen sowohl im ambulanten als auch im stationären Bereich Patienten mit Störungen aufgrund des Konsums von Alkohol (ambulant 56 %, stationär 75 % der Betreuungen), Opioiden (ambulant 18 %, stationär 9 % ) und Cannabis (ambulant 14 %, stationär 5 %) die drei größten Hauptdiagnosegruppen. Männer weisen dabei hinsichtlich der Zahl der Betreuungen und Behandlungen wesentlich höhere Anteile bei (fast) allen Hauptdiagnosen auf. Die durchschnittlichen Behandlungsdauern sind für alle Hauptdiagnosegruppen im ambulanten Bereich länger (m = 168 [Pathologisches Glücksspiel, PG] – 336 [Alkohol, Opioide] Tage mittlere Behandlungsdauer) als im stationären Bereich (m = 78 [PG] – 112 [Stimulantien] Tage). Der Anteil planmäßiger Beender ist vor allem bei Patienten mit primär Alkohol-bezogenen Störungen oder pathologischem Glücksspiel in stationären Einrichtungen deutlich höher als in der ambulanten Versorgung. Bei Opioid-, Cannabis- und Stimulantienkonsumenten gibt es keine vergleichbar gravierenden Unterschiede. Sowohl in den ambulanten als auch in den stationären Einrichtungen kamen über 80 % (ambulant) bzw. 92 % (stationär) der Personen mit einem planmäßigen Betreuungs-/Behandlungsende auch zu einem positiven Gesamtergebnis der durchgeführten Intervention.
Zusammenfassung Ziel Dieser Beitrag untersucht anhand der Daten der Deutschen Suchthilfestatistik (DSHS) welche Veränderungen sich in der ambulanten Suchthilfe hinsichtlich Klientel, Hauptdiagnosen und Betreuungsergebnis im Zeitraum von 2007 bis 2016 ergeben haben. Die beobachteten Trends werden vor dem Hintergrund sich wandelnder gesamtgesellschaftlicher und versorgungspolitischer Rahmenbedingungen reflektiert. Material und Methoden Aggregierte Daten aller ambulanten Suchthilfeeinrichtungen, die sich im genannten Zehnjahreszeitraum an der DSHS beteiligt haben, wurden deskriptiv ausgewertet. Neben der Entwicklung des Betreuungsvolumens beschreiben die Analysen soziodemografische, störungs- und betreuungsrelevante Parameter im Zeitverlauf. Ergebnisse Die Anzahl teilnehmender Einrichtungen (2007: n=720; 2016: n=863) ist im untersuchten Zeitraum gestiegen. Neben einigen soziodemografischen Merkmalen (zunehmendes Durchschnittsalter, steigender Frauenanteil, höheres Bildungsniveau) hat sich insbesondere die Zusammensetzung der Hauptdiagnosen verändert. Trotz stark rückläufigem Anteil (2007: 57,3%; 2016: 48,9%) repräsentieren alkoholbezogene Störungen nach wie vor den häufigsten Betreuungsanlass. Bei opioidbezogenen Störungen ist ebenfalls ein Rückgang zu verzeichnen (2007: 18,6%, 2016: 13,7%), während cannabis-bezogene Störungen deutlich zugenommen haben (2007: 12,1%, 2016: 17,8%). Der Anteil positiver Betreuungsergebnisse war konstant hoch (2007: 64,5%, 2016: 64,1%). Diskussion Die konstant positiven Betreuungsergebnisse deuten darauf hin, dass auf veränderte Bedarfe der Suchthilfeklientel in richtigem Maße reagiert wurde. Perspektivisch ist von einem Bedeutungszuwachs der älteren Klientel sowie von Menschen mit Migrationshintergrund in der Suchthilfe auszugehen, was annahmegemäß weitere Anpassungen des Angebots nach sich ziehen dürfte.
Zusammenfassung. Ziel: Cannabis ist in Deutschland die am häufigsten konsumierte illegale Droge. In Suchthilfeeinrichtungen machen Personen mit einer cannabisbezogenen Störung die zweitgrößte Gruppe aller Klienten aus. Ziel ist es, Veränderungen in der Inanspruchnahme von Suchthilfe aufgrund cannabisbezogener Probleme darzustellen und Besonderheiten der behandelten Cannabiskonsumenten, insbesondere Unterschiede zwischen ambulant und stationär Behandelten, herauszuarbeiten. Methodik: Es handelt sich um eine deskriptive Analyse der Daten von Personen aus ambulanten und stationären Suchtberatungs-/-behandlungseinrichtungen in Deutschland, die im Rahmen der Deutschen Suchthilfestatistik (DSHS) dokumentiert wurden. Im Datenjahr 2013 lag die Zahl der betrachteten Fälle im ambulanten Bereich bei n = 163.865 aus 822 Einrichtungen und im stationären Bereich bei n = 41.395 Fällen aus 200 Einrichtungen. Ergebnisse: Der Anteil der Personen mit einer primären Cannabisproblematik in Einrichtungen der Suchthilfe ist im Verlauf ambulant von 6,4 % (2000) auf 15,4 % (2013) und stationär von 1,1 % (2000) auf 7,1 % (2013) gestiegen. Personen mit einer cannabisbezogenen Störung zeichnen sich durch junges Alter, geringes Bildungsniveau und hohe Arbeitslosigkeit aus. Im Vergleich zu anderen Hauptdiagnosen (HD) zeigt sich ein früher Erstkonsum und Störungsbeginn. Vor allem stationär weisen Cannabispatienten eine hohe Zusatzbelastung durch multiplen Substanzkonsum auf. Ein vergleichsweise hoher Anteil kommt aufgrund gerichtlicher Auflagen in die Behandlung (ambulant: 29,9 %, stationär: 27,6 %) und etwa zwei Drittel zeigen nach Behandlungsende eine Verbesserung ihrer Suchtproblematik (ambulant: 64,0 %, stationär: 64,0 %). Schlussfolgerungen: Der deutlich gestiegene Anteil der Klienten mit HD Cannabis an allen Klienten ist vermutlich durch die gestiegene Zahl spezieller Programme zur Behandlung cannabisbezogener Störungen und ein stärkeres Bewusstsein über die Behandlungsbedürftigkeit problematischen Cannabiskonsums bedingt. Das geringe Bildungsniveau, die problematische berufliche Situation sowie die geringe Selbstständigkeit der Cannabisklienten sind vermutlich auf das unterdurchschnittliche Alter zurückzuführen, weshalb besonders Maßnahmen zur Erarbeitung einer psychosozialen Perspektive, wie z. B. Kooperationen mit Sozialämtern, Jugendhilfe und Arbeitsagenturen notwendig sind.
