Die vorliegende Expertise hat zwei Zielsetzungen: <OL><LI>Überblick über Therapieangebote für Alkohol- und Drogenabhängige und die Dauer der Therapie in Deutschland und in ausgewählten europäischen Staaten (Frankreich, Italien, Großbritannien, Niederlande und Schweden) <LI>Darstellung der Therapieerfolge in Europa und Analyse des Zusammenhangs zwischen Therapiedauer und Therapieerfolg einschließlich der Berücksichtigung zusätzlicher Einflussvariablen</OL>
<span class="fett">Ziele:</span> </p><ol> <li>Abschätzung der »Bevölkerungsattraktivität« von Glücksspielen sowie</li> <li>der Prävalenz des Pathologischen Glücksspiels (PG) in Form von »Glücksspiel- und Bevölkerungsrisiken«</li> </ol><p> <span class="fett">Methodik:</span> Die Stichprobe betrug 7.817 Respondenten zwischen 18 und 64 Jahren aus dem Epidemiologischen Suchtsurvey 2006. Lebenszeit- und 12-Monats-Prävalenz, präferiertes Glücksspiel und PG Diagnosen (DSM-IVTR) wurden mittels Fragebögen und telefonischen Interviews erfasst. </p><p> <span class="fett">Ergebnisse:</span> 71,5 % der Deutschen haben schon einmal gespielt, 49,4 % innerhalb der letzten 12 Monate. Bei der »Bevölkerungsattraktivität« stehen die Spiele der Lottogruppe im Vordergrund (60,3 %), mit Abstand folgen Lotterien, Sportwetten, Casinospiele, Geldspielautomaten und illegales Glücksspiel. Das höchste »Glücksspielrisiko« zeigen Internetkartenspiele (7,0 %) und das kleine Spiel im Casino (6,7 %), die geringste Gefährdung besteht durch Lotto/Toto/Keno (0,1 %). Das »Bevölkerungsrisiko« für PG liegt bei knapp 0,2 % (etwa 103.000 Personen), mit etwa gleich großen Anteilen für Sportwetten, Casinospiele und Geldspielautomaten (0,05–0,06 %; 24.000–31.000) sowie mit Abstand für Lottospiele (0,02 %; 12.000). Unter Berücksichtigung der Differenzialdiagnose Manische Episode halbiert sich das Bevölkerungsrisiko (0,09 %; etwa 46.000). </p><p> <span class="fett">Schlussfolgerungen:</span> Die Prävalenz für PG in Deutschland liegt für die erwachsene Bevölkerung im unteren Bereich europäischer Untersuchungen. Für präventive Maßnahmen sind die Unterschiede zwischen in »Glücksspielrisiko« und »Bevölkerungsrisiko« zu beachten.
Fragestellung: Die regelmäßige Auswertung von statistischen Daten über die Merkmale und Tätigkeit ambulanter Einrichtungen der Suchtkrankenhilfe in Deutschland ermöglicht eine Beschreibung und Analyse der Versorgungsstruktur und deren eventuell vorhandene Mängel, der Klientencharakteristika sowie der im Bezugsjahr erzielten Ergebnisse. Sie erlaubt Trendanalysen zu langfristigen Veränderungen und dient zusätzlich als Grundlage für die Planung von Forschungsprojekten und Modellprogrammen zur Verbesserung einzelner Aspekte der Struktur und Qualität ambulanter Einrichtungen. </P><P>Methodik: Die Statistik für 2002 basiert auf den Daten von 161.553 Klienten aus 454 ambulanten Spezialeinrichtungen für Menschen mit substanzbezogenen Störungen in Deutschland. Ausgewertet werden Aggregatdaten nach den standardisierten Vorgaben des Bundesdaten- und Bundestabellensatzes, der den Deutschen Kerndatensatz und den Kerndatensatz der Europäischen Beobachtungsstelle für Drogen und Drogenprobleme (EBDD) einschließt. </P><P>Ergebnisse: Alkohol (schädlicher Gebrauch und Abhängigkeit) steht bei 67 % der Klienten mit eigener Symptomatik im Vordergrund der Betreuung (Hauptdiagnose), 14 % der Klienten haben eine opiatbezogene und 9 % eine cannabisbezogene Hauptdiagnose. Der Anteil der Klienten mit einer stimulanzienbezogenen Hauptdiagnose (Ecstasy u. ä.) liegt mit 2,1 % höher als der Anteil derer mit Kokain (1,7 %). Die weiteren Substanzen sind bei den Hauptdiagnosen mit weniger als einem Prozent der Klientel vertreten; Essstörungen und Pathologisches Spielverhalten werden zusätzlich in geringem Umfang behandelt. Im Verlauf der letzten Jahre nahmen alkohol-, opiat-, cannabis- und stimulanzienbezogene Hauptdiagnosen weiter zu. Für die anderen wichtigen Substanzen zeigt sich ein leichter Rückgang bzw. keine bedeutsame Veränderung. Der Anteil der ambulanten medizinischen Rehabilitation an der Gesamtfinanzierung hat sich in den letzten fünf Jahren fast verdoppelt (W: 10,8 %; O: 2,6 %). 39 % der Klienten mit alkoholbezogener Hauptdiagnose und 19 % derer mit opiatbezogener Hauptdiagnose werden planmäßig entlassen; 17 % bzw. 28 % werden weiter vermittelt. Nach wie vor ist der Anteil der Klienten, die die Betreuung abbrechen, mit 40 % bei Alkohol und 43 % bei Opiaten etwa gleich hoch. </P><P>Schlussfolgerungen: Insgesamt zeigen die Ergebnisse, dass das ambulante Suchtkrankenhilfesystem im Durchschnitt bei knapp 50 % der Klienten erfolgreich ist (planmäßige Entlassung oder Vermittlung). Im Vergleich zu den Vorjahren gibt es kaum relevante Veränderungen in den Einrichtungs-, Klienten- oder Betreuungsmerkmalen bis auf eine Ausnahme. Die seit einigen Jahren beobachtete Tendenz, dass es in der Bevölkerung immer mehr Cannabiskonsumenten gibt, setzt sich auch in diesem Jahr in den Behandlungszahlen fort. Dies sollte Anlass sein, Studien durchzuführen, die die cannabisbezogene Störung, ihre Behandlungsindikation sowie spezifische dafür notwendige Behandlungsinhalte genauer untersuchen.
Übersicht über psychotherapeutische Interventionen bei cannabisbezogenen Störungen <B>Ziel:</B> Übersicht über die Literatur zur psychotherapeutischen Behandlung von Cannabismissbrauch und Abhängigkeit. </P><P> <B>Ergebnisse:</B> Beschränkungen von acht identifizierten Studien werden beschrieben. Als wirksame Therapiekomponenten stellten sich die Motivationssteigerungs-Therapie, die kognitive Verhaltenstherapie, das Notfallmanagement und verschiedene Arten der Familientherapie für jüngere gefährdete Cannabiskonsumenten heraus. </P><P> <B>Schlussfolgerungen:</B> Insgesamt belegen die Studien, dass es sowohl einen steigenden Bedarf als auch eine potenzielle Wirksamkeit von Behandlungen gibt, die speziell auf cannabisabhängige und gefährdete Konsumenten zugeschnitten sind. Trotzdem besteht immer noch ein erheblicher Forschungsbedarf zu spezifischen psychologischen Interventionen auf diesem Gebiet. Insbesondere in Europa herrscht ein eklatanter Mangel an Interventionsstudien und der Entwicklung von Behandlungsprogrammen für den routinemäßigen Einsatz in Einrichtungen für Suchthilfe.
<span class="fett">Zielsetzung:</span> Ziel der vorliegenden Publikation ist die Auswertung der Deutschen Suchthilfestatistik für ambulante Einrichtungen, die seit 1980 die Aufgaben der Dokumentation von Einrichtungs-, Patienten- und Behandlungsmerkmalen, der Angebotsevaluation und Qualitätssicherung erfüllt, Trendanalysen ermöglicht und zudem als praktische Arbeitshilfe dient. </p><p> <span class="fett">Methodik:</span> Die Auswertung 2006 basiert auf Daten von 267.496 Klienten (2005: 265.245) aus 743 ambulanten Suchthilfeeinrichtungen (2005: 749) in Deutschland. Es werden aggregierte Daten zu Patienten-, Behandlungs- und Ergebnischarakteristika auf der Basis der beiden deutschen Standarddatensätze (Bundesdatensatz bzw. Deutscher Kerndatensatz) ausgewertet. Analysiert werden Vergleiche zum Vorjahr. </p><p> <span class="fett">Ergebnisse:</span> Mit 55 % (2005: 54 %) der erfassten Fälle ist die alkoholbezogene Hauptdiagnose (Abhängigkeit oder schädlicher Gebrauch nach ICD-10) am häufigsten. Diagnosen aufgrund anderer Substanzen umfassen unterschiedliche Anteile an den Behandlungsfällen (Opiate: 21 %; 2005: 22 %; Cannabis: 13 %; 2005: 13 %; Kokain: 3 %; 2005: 3 %; Stimulanzien: 3 %; 2005: 2 %). Der Anteil erfasster Erstbehandelter ist im Vergleich zu 2005 in allen Klientengruppen deutlich gesunken. Unter allen abhängigen Klienten hat ein deutlich höherer Anteil eine Opiat-Hauptdiagnose als unter den Klienten mit schädlichem Gebrauch (21 % vs. 3 %; 2005: 23 % vs. 4 %). Umgekehrt hat unter allen Klienten mit einer Hauptdiagnose für schädlichen Gebrauch ein deutlich höherer Anteil eine Cannabis-Hauptdiagnose als unter denjenigen mit einer Abhängigkeit (2006: 28 % vs. 10 %; 2005: 31 % vs. 9 %). 42 % der Klienten mit einer Alkohol- (2005: 43 %), 24 % derjenigen mit einer Opiat- (2005: 23 %) und 44 % derjenigen mit einer Cannabis-Hauptdiagnose (2005: 45 %) beenden ihre Behandlung planmäßig. Unter diesen planmäßigen Beendern erreichen wie im Vorjahr 84 % der Alkohol-, 71 % der Opiat- und 80 % der Cannabis-Klienten ein positives Ergebnis im Sinne einer Behebung oder Besserung der Problematik. </p><p> <span class="fett">Schlussfolgerungen:</span> Die weitgehend stabile Stichprobe im Vergleich zum Vorjahr schlägt sich in einer hohen Konstanz der Verteilung der Hauptdiagnosen, der soziodemografischen sowie behandlungsbezogenen Daten nieder.
