Introdução: As lesões traumáticas são atualmente a terceira causa de mortalidade no mundo, gerando impacto na morbimortalidade, principalmente na população economicamente ativa. Objetivo: Avaliar os fatores de risco para óbito no trauma em um hospital escola de nível terciário. Métodos: Realizou-se estudo caso-controle retrospectivo, por meio da análise de prontuários eletrônicos e fichas de avaliação inicial de pacientes atendidos em um hospital escola de nível terciário da cidade de São José do Rio Preto, São Paulo, Brasil, em 2015. Os 21 pacientes que morreram, compuseram os casos, enquanto 613, que sobreviveram ao evento traumático, o grupo controle. Esses grupos foram comparados, considerando variáveis gerais, dados sobre o evento traumático, atendimento pré-hospitalar, avaliação hospitalar inicial e desfecho da internação. Resultados: Os fatores de risco para mortalidade, por evento traumático identificados pelo estudo, foram: idade superior a 30 anos, traumas por agressão física, tempo de internação do paciente superior a 72 horas, perda de consciência no momento do evento traumático, presença no atendimento inicial do traumatizado de hipotensão arterial (pressão sistólica abaixo de 108,7 mmHg), escore da escala de coma de Glasgow inferior ou igual a 9 e revised trauma score (RTS) inferior a 5,7. Conclusão: Os fatores de risco que determinam maior probabilidade de óbito, após um traumatismo são causados por agressão física ao paciente com idade superior a 30 anos, com perda de consciência após o evento, hipotensão arterial, escore da Escala de Coma de Glasgow e RTS baixos no atendimento inicial, além de tempo de internação prolongado. Dessa forma, apesar de protocolos cada vez mais consolidados de atendimento ao politraumatizado, ainda há limitações em propiciar uma melhor sobrevida para os pacientes com pior prognóstico

Therefore, they need recre-ational activities, which promoteemotional well-being and reduce pain sensation. Ludic activities, especially the clown therapy, are presented as a low-cost alternative to remedy this problem. Objectives: Characterize the hospitalized elderly in a tertiary hospital, evaluate the presence of anxious and depressive symptoms in this sample, and verify if there is an improvement in the emotional state and a decrease in the perception of pain in these patients after the intervention of the clowns-doctors. Patients and Methods:The study included 35 elderly patients admitted to the Hospital de Base of São José do Rio Preto, who answered a questionnaire, which was applied by a previously trained academic. The questionary contained the following: identification, Geriatric Anxiety Inventory, Geriatric Depression Scale, Unidirectional Pain Scale, and self-assessment of emotional state. The questionnaire was applied 1 hour before and 1 hour after the interventions. Data were subsequently compared and evaluated descriptively and quali-quantitatively using the Wilcoxon statistical test. Results: It was observed in the profile of elderly, a prevalence of male subjects (57.5%), a frequent history of previous admissions (85.7%), and the presence of anxiety symptoms (40%). Regarding the changes observed before and after the intervention, there was a significant decrease in pain sensation (P = 0.001) and an improvement in the emotional state of the respondents, decreasing worry autoanalysis, anxiety, and sadness, but increasing the joy and excite-ment. Conclusion: Data suggest that the clown therapy is a method to reduce pain sensation and promote the improvement of the emotional state in hospitalized elderly patients.

RESUMO Introdução: Crianças com Síndrome de Down (SD) apresentam risco aumentado de múltiplos problemas de saúde, incluindo disfunções tireoidianas (DT). Mesmo com a evolução das técnicas diagnósticas e da abordagem clínica, o desenho do rastreamento e do manejo das disfunções tireoidianas em pacientes com SD ainda apresenta desafios. Ampliar o conhecimento das particularidades desses indivíduos pode levar a um delineamento mais adequado de estratégias de rastreamento, diagnóstico e tratamento, melhorando o atendimento à saúde. Objetivo: Verificar o perfil epidemiológico das disfunções tireoidianas, alterações laboratoriais e suas associações com variáveis demográficas e clínicas em pacientes com Síndrome de Down. Materiais e Métodos: Estudo observacional, retrospectivo, realizado em um hospital público universitário. Foram incluídos 177 pacientes de 0 a 25 anos de idade com SD. As variáveis analisadas foram: idade, sexo, presença e classificação de DT, tratamento e associação com autoimunidade antitireoidiana e extratireoidiana, genótipo, cardiopatia, dados antropométricos e história familiar de tireoidopatia. Resultados: A prevalência de DT foi de 88,7% e grande parte das alterações foi diagnosticada nos primeiros 6 meses de vida (48%). Em pacientes com menos de 3 anos, DT predominou no sexo masculino. Não houve associação entre DT e as variáveis avaliadas, exceto genótipo. A disfunção mais prevalente foi o hipotireoidismo subclínico com hormônio tireoestimulante (TSH) entre 4,2-10 μIU / mL, encontrado em 120 dos 157 pacientes com DT.
