The subjects revealed that the team also suffers with the death of a child and, like the family, moves toward the construction of coping mechanisms for the elaboration of mourning. Paradoxically, the team shares knowledge to determine the foundations of a singular therapeutic project and inserts the family in this process so that it can be the protagonist of the child's care.
RESUMO O cuidado paliativo à família da criança em oncologia pediátrica perpassa vários olhares e saberes, num processo compartilhado de construção. Ouvir a equipe de enfermagem acerca de suas experiências com famílias pode ressignificar a práxis do cuidado sensível e competente. Este estudo objetivou conhecer as experiências e as percepções da equipe de enfermagem em relação à família da criança em cuidados paliativos na Oncologia Pediátrica do Hospital de Clínicas de Porto Alegre e consiste numa pesquisa qualitativa, exploratória e descritiva. Seus dados foram coletados junto a quinze profissionais de enfermagem por meio de entrevista semiestruturada e submetidos à análise de conteúdo. O estudo possibilitou compreender que, diante da facticidade existencial da família ao enfrentar a situação de cuidados paliativos do (da) filho(a), questionamentos filosófico-existenciais afloram e permeiam este processo singular de cuidado, trazendo algumas inquietações difíceis de enfrentar, porém, paradoxalmente, impulsionam a equipe a não esmorecer e a colocar-se como referência para a criança e a família. O cuidado está intimamente relacionado ao vínculo estabelecido entre o profissional, a família e a criança, o que faz com que o profissional, movido pela compaixão, centre seus cuidados não somente no biológico, explicitando uma genuína valorização da vida.
RESUMOEste estudo caracteriza-se por ser uma pesquisa qualitativa que objetivou desvelar a percepção e a vivência em relação ao tratamento antirretroviral do adolescente com síndrome da imunodefi ciência adquirida. A pesquisa foi realizada em serviços de referência em dois municípios na região sul do Brasil. A produção dos dados foi desenvolvida com a dinâmica de criatividade e sensibilidade mapa falante, por cinco participantes. Foi aplicada a técnica de análise temática do conteúdo. Das produções artísticas e depoimentos emergiram o cotidiano do uso dos medicamentos e o silêncio do diagnóstico da doença e do tratamento que implicam o cuidado à saúde. Conclui-se que é necessário o envolvimento dos profi ssionais de saúde, possibilitando espaços para a família e promovendo diálogos desta com o adolescente, visando à adesão ao tratamento. ABSTRACTThis study is characterized as a qualitative research with the objective of unveiling the perception and experience regarding the antiretroviral treatment of the adolescent with acquired immunodefi ciency syndrome. The research was carried out within reference health care centers in two municipalities from the southern region of Brazil. The data production was developed through the creativity and sensitiveness dynamics of talking map type by a group of fi ve participants. One applied the technique of content thematic analysis. The artistic productions and testimonies evidenced their daily experience with use of medications and the silence on the diagnosis of the disease and its treatment that implies the health care. The conclusion drawn is that the involvement of health professionals is needed since it opens spaces for the family besides promoting the dialogue of its members with the adolescent aiming at the adhesion to the treatment.
Introdução: a proposta dos Cuidados Paliativos é implementada para pacientes em situações de vulnerabilidade ante uma doença grave, limitante e que pode, inevitavelmente, se encaminhar para a terminalidade. Objetivo: conhecer as percepções e vivências dos profissionais de enfermagem frente ao cuidado à criança em cuidados paliativos em unidades pediátricas. Metodologia: estudo qualitativo, exploratório e descritivo, realizado em unidades pediátricas de um hospital universitário de grande porte do Sul do Brasil. Os dados foram obtidos mediante entrevistas semiestruturadas com nove participantes, enfermeiros e técnicos de enfermagem, submetidas à análise de conteúdo temática. Resultados: a partir da análise temática emergiram quatro temas: Criança em cuidados paliativos: olhar da enfermagem; Cuidado centrado na família; Vivências, sentimentos e percepções da equipe de enfermagem; e Enfrentamento da equipe de enfermagem: desafios para o cuidado. O estabelecimento dos princípios de cuidados paliativos pediátricos é fundamental para subsidiar o cuidado qualificado. Os profissionais de enfermagem revelam a necessidade de serem incluídos nas reuniões multidisciplinares realizadas para decidir condutas em relação ao paciente e sentem necessidade de apoio psicológico. Considerações finais: desvela-se a compreensão de que a filosofia de Cuidados Paliativos precisaser ampliada para uma rede integrada de atenção e respeito às decisões para o final da vida.
Objective: To report the experience of the training in home parenteral nutrition (PN) directed to family members of children and adolescents participating in a multidisciplinary intestinal rehabilitation program of a tertiary public hospital.Methods: Cross-sectional descriptive study with family caregivers of patients from the Intestinal Rehabilitation Program of Hospital de Clínicas de Porto Alegre, RS, Brazil, from July/2014 to January/2017. Inclusion criteria: family members of children aged 30 days to 17 years and estimated PN use ≥8 weeks; and family members motivated to care for the child. The training covered: hand washing and disinfection; infusion pump handling; and central venous catheter (CVC) and PN care. Outcomes assessed: catheter-related bloodstream infection (CRBSI) rate, accidental CVC exit, end of PN infusion with more than 60minutes of delay or advance compared to the time predicted, mechanical obstruction, bleeding in the CVC insertion site, and death.Results: Twenty-seven family members of 17 children were trained. Their median age was 28 (18-60) years, and 63% were mothers. The mean CRBSI rate was 1.7/1,000 days of CVC use, and 29.4% of patients had at least one episode of accidental CVC exit. There were no complications related to PN infusion, bleeding, or death.Conclusions: The training of family caregivers allowed the safe implementation of home PN, with the active participation of families, making the procedure feasible in the public health system in Brazil.
Objective To know the dehospitalization of technology-dependent children from a multiprofessional perspective. Method A qualitative, exploratory and descriptive study, carried out in 2018 at a Teaching Hospital in the city of Porto Alegre. The participants were 15 members of the multidisciplinary health team. In data production, we used the Dynamics of Creativity and Sensitivity, “Creative Storm”, which is part of the Sensitive Creative Method. Data interpretation occurred based on Thematic Content Analysis. The study was approved by the Ethics Committee. Results Dehospitalization is influenced by the absence of planning according to the reality of the child and the family. In addition, there is predominance of health services with a physician-centered model, unscheduled discharge, overload of professionals, and communication problems between the staff, the health network and the family. Final Considerations Despite knowing the importance of dehospitalization, it still occurs in a fragmented manner, with hasty hospital discharge, preventing safe dehospitalization and with greater planning.
This article presents a cutting from the multicentric study carried out in the municipalities of Porto Alegre and Santa Maria/ RS with the objective of unveiling the perception and the life experience of the child regarding the antiretroviral treatment. With qualitative approach, the study was carried out with seven children of five to ten years of age, in the period from 2006 to 2010, after approval by Committee National for Ethics in research and the Committees of Ethics in research. Based on the thematic analysis was obtained the results: the dayto-day life of the child with medicines; the family care upon the adhesion to the antiretroviral treatment; the professional care: perception of children with infection. Observation showed that the children face adversities, know and appreciate the treatment in spite of the paradoxical movement of rejection/acceptance expressed by the fight against the syndrome.Descriptors: Acquired Immunodeficiency Syndrome.
