Obstetrik nedenlerle acile gelen hastaların 10 dakika içinde değerlendirilip, aciliyetlerine göre sınıflandırılması için sağlık personeli tarafından yapılan işleme ise obstetrik triyaj denir. Obstetrik, gebelik ve doğum ile ilgili bilgileri karşılayan bir terim olmasının yanında, gelişmekte olan ülkelerde bu dönemdeki komplikasyonlar önde gelen morbidite ve mortalite nedenidir. Çok boyutlu bir kalkınma göstergesi olarak anne ölüm düzeyi, üreme sağlığı hizmet sunumu kalitesi ile yakından ilişkilidir. Türkiye’de obstetrik triyaj kullanılma durumu ve bunu geliştirmek için yapılan uygulamalar şu şekildedir; Günümüzde Sağlık Bakanlığı’nın uyguladığı üçlü triyaj sistemi geçerli olup ölüm sınıflandırması bunun dışında tutulmaktadır. Gebeler kapıda geliş sırasına göre değil, aciliyetine göre kırmızı, sarı ve yeşil renkler ile kod alırlar. Ebelerin prenatal ve intrapartum bakım yönetiminde tecrübeli olmaları obsterik triaj için ideal sağlık personeli olduklarını ortaya koymaktadır. Bu kapsamda Sağlık Bakanlığı tarafından düzenli aralıklarla “Acil Obstetrik Bakım Eğitimi” düzenlenmektedir. Obstetrik triyaj sisteminin bir ayağı da “Kurtarılan Anne” (Near Miss) yaklaşım programıdır. Türkiye’de anne ölümlerinin nedenleri arasında önlenebilir nedenlerle ölüm oranı %37.9’dur. Gebelerin sağlık kuruluşlarına ulaşmada ve ulaştırılmada ortaya çıkan problemleri belirlemede "Dünya Sağlık Örgütü Geçikme Modelleri"ne göre sınıflandırma yapılmaktadır. Ayrıca obstetrik triyaj; Sağlık Bakanlığı tarafından verilen "Acil Durum ve İlk Yardım Eğitimleri"nde obstetrik acillere yaklaşım temel modül içerisinde yer almaktadır. Tüm bu uygulamalar kapsamında ebeler ana faktör olarak yer almaktadır. Avrupa Birliği kriterleri doğrultusunda üniversitede eğitim verilen ebelerin eğitimlerinde "obstetrik aciller ve gebelerin risk tanılaması" tüm eğitim sürecinde yer almaktadı
Diastazis Rekti, karın bölgesinin sol ve sağ ön yüzeyini tutan rektus abdominis kasları arasında ayrılma ve üst orta hat karında yaygın bir çıkıntı ile karakterize bir bozukluktur. Tanısı sağlık profesyonelleri (fizyoterapist, ebe, hemşire ya da doktor gibi) tarafından konulabilir. Eğer, sağlık profesyoneli bu durumdan şüphe ediyorsa fizyoterapi konsültasyonu isteyebilir. Diastazis Rekti, umbilikusun üstünde 4.5 cm ve altında 4.5 cm uzunluğunda (bazı kaynaklara göre 3cm), genişliği ≥2 cm/parmak olarak palpe edilebilir. Uluslararası literatürde Diastazis Rekti'nin, gebelik döneminde %30-70, doğum sonu 6-8. haftalarda %52.4 ve 6. ayda %39.3 oranında görüldüğü belirtilmektedir. Türkiye'deki prevelansı gebelerde %24-55 olarak saptanmıştır. Diastazis Rekti gelişmesinde pek çok faktörün yanında özellikle gebeliğe bağlı ortaya çıkan hormonal değişiklikler, ileri anne yaşı, multiparite, sezaryen, büyüyen uterus ve uterusun fazla gerildiği durumlar ile buna bağlı artan intra-abdominal basınç etkilidir. Diastazi Rekti yaşam kalitesini düşürmektedir. Sağlık çalışanları doğum sonu muayene sırasında Diastazis Rekti saptar ise, solunum egzersizi, kegel egzersizi, pelvik-tilt, korse ya da abdominal bant kullanımı gibi koruyucu uygulamaları kadına öğretmeli, bunları uygulaması için motive edip izlemini yapmalıdır. Daha ciddi ayrılmalara ise farklı tedavi seçenekleri önerilmektedir. Bununla birlikte, doğum sonu bedenindeki değişimlerin takibi yanında yeni rol ve sorumluluklarına uyum sağlamaya çalışan kadının yaşam kalitesinin artması ve sağlıklı aile yapısının desteklenmesi için Diastazis Rekti önemlidir. Bu derleme ile, literatür sonuçlarına bağlı olarak Diastazis Rekti ve bakımı konusunda farkındalığın arttırılması amaçlanmaktadır.
EBELİK BÖLÜMÜ UZAKTAN EĞİTİM ÖĞRENCİLERİNİN E-ÖĞRENME İÇİN HAZIRBULUNUŞLUK DURUMLARININ DEĞERLENDİRİLMESİ ÖZ EVALUATION OF MIDWIFERY DISTANCE EDUCATION STUDENTS E-LEARNING READINESS ABSTRACTDistance learning has become more widespread recently, many professional shave begun a question how remote distance education students are prepared to be successful in this environment. The aim of midwifery department is to evaluate the availability of distance learning students for e-learning. The research sample has been accepted to participate in the study (n=86). Sociodemographic characteristics of the participants, a questionnaire measuring distance education information and a self-assessment questionnaire for e-learning were used in the data collection. The data were analyzed in the SPSS 15.0 program. The average score scale of first class distance education students was 101.0±14.17 (min: 68, max: 125) while the average score of second grade students was 105.10±12.38 (min: 74 max: 125). As a result of our study; all students were found to be ready for e-learning.
This study aimed to identify the factors affecting primiparous pregnant women’s prenatal attachment levels, childbirth self-efficacy beliefs, and labor worry levels. This cross-sectional study was conducted with 351 pregnant women. The data were collected by using a Personal Information Form, the Prenatal Attachment Inventory, the Oxford Worries about Labour Scale, and the Childbirth Self-Efficacy Inventory. The mean Prenatal Attachment Inventory, Oxford Worries About Labour Scale and Childbirth Self-Efficacy Inventory scores of the participants were 40.53 ± 10.32, 21.82 ± 6.8, and 201.35 ± 23.21, respectively. The data obtained in the study showed that the care offered in the preconception period affected the participating pregnant women’s prenatal attachment, labor worry, and childbirth self-efficacy levels. With individualized preconception care provided by health personnel, not only will awareness be increased but also women will be able to have a more positive pregnancy and labor experience.
Introduction: The study was conducted to determine the relationship between body image, genital self-image and self-esteem in the postpartum period. Materials and Method: The study was carried out with 145 women in the postpartum period. The data of the study were collected using the Personal Information Form, Body Cathexis Scale (BCS), Female Genital Self-Image Scale (FGSIS) and Rosenberg Self-Esteem Scale (RSES). Results: It was determined that the mean BCS total score of the women was 143.56±26.82, the total mean score of the FGSIS was 17.23±4.97, and the total mean of the RSES score was 20.23±4.52. It has been determined that as the genital self-image and body perception level of women increase in the postpartum period, their self-esteem also increases. Conclusion: Health professionals should inform women about body changes that may occur in the postpartum period, warn them not to be deceived by media images, and emphasize the importance of realistic postpartum expectations.
Aim The aim of this study was to determine prenatal attachment and foetal health locus of control in pregnant women and factors affecting these parameters. Methods This cross‐sectional study was conducted with 156 pregnant women (77 low‐risk and 79 high‐risk pregnancies). The study was carried out between August 2015 and February 2016. The data collection instruments included a Participant Information Form, the Prenatal Attachment Inventory and the Foetal Health Locus of Control Scale. Results Pregnant women's education and foetal health locus of control levels had positive and significant effects on prenatal attachment, and education had an effect on foetal health locus of control. Foetal health locus of control explained 6.3% of the total variance in prenatal attachment levels. Conclusion As the educational statuses of pregnant women affected their prenatal attachment and foetal health locus of control, information, counselling and care should be prioritized for women with low education levels, in line with their needs. This is particularly important in terms of improving the control of pregnant women with lower prenatal attachment levels or internal locus of control over foetal health.
