O PET-Saúde Niterói, proposto pela Universidade Federal Fluminense (UFF) e pela Fundação Municipal de Saúde de Niterói (FMS), iniciou suas atividades em abril de 2010, após a publicação da aprovação da proposta do Edital nº 18, de 16 de setembro de 2009, do Ministério da Saúde, tendo como objetivos: fortalecer os mecanismos de cooperação entre gestores da rede local de saúde e a UFF, visando ampliar os cenários de aprendizagem; intensificar, na formação, a abordagem integral do processo saúde-doença e da promoção da saúde; favorecer a adoção de metodologias ativas de aprendizagem e educação permanente do trabalhador envolvido na relação trabalho e ensino; ampliar as experiências de trabalho em equipe multidisciplinar na formação dos profissionais de saúde, possibilitando a construção de novos mecanismos de atenção na rede de saúde local; fortalecer o processo de regionalização e territorialização como instrumento de ampliação da resolutividade e da integralidade da atenção efetivada pela Estratégia de Saúde da Família.O projeto aposta na compreensão do processo de aprendizagem ativa, tendo como eixo central o trabalho cotidiano nos serviços de saúde, uma vez que é no âmbito do trabalho que se consolidam os comportamentos e formas de atuação profissional, individual e coletiva. O cotidiano dos serviços de saúde configura o espaço de exposição da complexidade das relações e das instituições humanas e da integração entre teoria e prática. Um local em que os atores das cenas de ensino e de aprendizagem − professor, aluno, cidadão, usuário, trabalhador − se encontram no processo de trabalho, numa equipe de saúde multiprofissional.É importante ressaltar que a relação entre a UFF, particularmente no que se refere aos cursos da área de saúde, e a rede de saúde pública do município de Niterói se intensificou entre o final da década de 1970 e o início dos anos 1980, sobretudo a partir do Projeto Niterói -expressão municipal das Ações Integradas de Saúde (1982). A articulação entre a universidade, os serviços de saúde e a comunidade, que se deu inicialmente de forma pontual por meio da inserção de docentes e estudantes em práticas de disciplinas, projetos de extensão e de pesquisa, se ampliou, refletindo sempre uma preocupação com a necessária integração entre formação e rede de serviços, ligando e fortalecendo a relação entre o mundo do trabalho e o mundo da aprendizagem. Atualmente, mantemos essa parceria com diversos projetos, entre eles o Pró-Saúde e, mais recentemente, o PET-Saúde.O PET-Saúde Niterói é formado por oito grupos, compostos por docentes da universidade, profissionais de saúde do município de diferentes categorias e alunos de distintos perí-odos dos cursos de Educação Física, Enfermagem, Farmácia, Medicina, Nutrição e Odontologia, configurando um aprendizado multiprofissional. A case of malignant neoplasm is described in which the initial manifestations were mental dysfunction and meningeal irritation, mimicking chronic or subacute meningitis. Physical examination showed cranial nerve involvement and a pelvic tumor. There was progressive deterioration, and death occurred in 2 weeks. The autopsy revealed a gallbladder adenocarcinoma, meningeal carcinomatosis, and ovarian metastasis presenting as a Krukenberg tumor. The authors emphasize the importance of including meningeal carcinomatosis as a possibility in the differential diagnosis of non-characteristic clinical pictures, as well as the importance of the cerebrospinal fluid cytologic examination, repeated as needed, in order to confirm this diagnosis.
The exogenous lipoid pneumonia, uncommon in adults, is the result of the inhalation and/or aspiration of lipid material into the tracheobronchial tree. This is often confused with bacterial pneumonia and pulmonary tuberculosis due to a nonspecific clinical and radiologic picture. It presents acutely or chronically and may result in pulmonary fibrosis. We describe here a case of lipoid pneumonia in a gas station attendant who siphoned gasoline to fill motorcycles; he was hospitalized due to presenting with a respiratory infection that was hard to resolve. The patient underwent bronchoscopy with bronchoalveolar lavage, which, on cytochemical (oil red O) evaluation, was slightly positive for lipid material in the foamy cytoplasm of alveolar macrophages. Due to his occupational history and radiographic abnormalities suggestive of lipoid pneumonia, a lung biopsy was performed to confirm the diagnosis. The patient was serially treated with segmental lung lavage and showed clinical, functional, and radiological improvement.
Resumo: Apesar da Lista Brasileira de Causas de Morte Evitáveis (LBE), na sua versão para crianças menores de 5 anos (LBE < 5), não contemplar óbitos fetais, alguns estudos a utilizaram sem ou com poucas adaptações. Apresentamos uma proposta de adaptação da LBE para óbitos fetais (LBE-OF) e os resultados de sua aplicação aos óbitos fetais no Estado do Rio de Janeiro, Brasil, em 2018, comparados aos da LBE < 5. Revisamos as categorias da 10ª revisão da Classificação Internacional de Doenças (CID-10) presentes na LBE < 5 e as realocamos nos grupos de evitabilidade, segundo momento do óbito em relação ao parto e as causas condizentes com óbitos fetais. Condições que não esclarecem os determinantes do óbito foram alocadas como causas mal definidas. Óbitos fetais no Estado do Rio de Janeiro - selecionados das bases do Sistema de Informações sobre Mortalidade (SIM) - foram classificados segundo LBE-OF e a LBE < 5. Ao classificar os 2.585 óbitos fetais do Estado do Rio de Janeiro ocorridos em 2018, observou-se que, segundo a LBE < 5, predominariam causas “reduzíveis por adequada atenção ao parto” (42,9%), já pela LBE-OF, aquelas “reduzíveis por adequada atenção à mulher na gestação” foram as mais frequentes (43,6%). As causas mal definidas ocuparam a segunda posição segundo a LBE-OF (35,4%) e a terceira posição segundo LBE < 5. Cerca de 30% dos óbitos fetais mudaram de grupos e subgrupos de evitabilidade, mostrando maior coerência com o perfil de atenção obstétrica. Embora identificando um maior percentual de causas mal definidas, a LBE-OF coaduna-se mais com a fisiopatologia dos óbitos fetais. Sua inserção no SIM avançaria no sentido de monitorar e qualificar a investigação de causas de morte fetal.
