Yara yeri enfeksiyonları, hastane kaynaklı enfeksiyonlar arasında ilk üç sırada yer almaktadır. Yara enfeksiyonlarının tedavisi sırasında kültür ve antibiyogram duyarlılıklarının yapılması, tedavinin başarısını arttıracağı gibi uygunsuz antibiyotik kullanımını engelleyerek, etken olan bakterilerde direnç gelişimini de önleyecektir. Çalışmamızda, 2016-2018 yılları arasında polikliniklere ayaktan başvuran ya da servislerde yatarak tedavi gören hastaların yara yeri örneklerinden izole ettiğimiz mikroorganizmaların dağılımı ve antibiyotik duyarlılıkları belirlenerek, ampirik tedavi seçeneklerine yol gösterici olması amaçlandı. Gereç ve Yöntemler: Hastanemiz mikrobiyoloji laboratuvarına gönderilmiş olan, toplam 315 yara yeri örneği retrospektif olarak incelendi. Etken olarak kabul edilen mikroorganizmaların tür düzeyinde tanımlanmaları ve antimikrobiyal duyarlılıkları araştırıldı. Bulgular: İzole edilen mikroorganizmaların %46.4'ünü (n=146) Gram pozitif koklar, %53'ünü (n=167) Gram negatif bakteriler ve %0.6'sını (n=2) mantarlar (Candida albicans) oluşturdu. S. aureus %21 oranında en sık, Koagülaz Negatif Stafilokok %16 oranında ikinci sıklıkta, E. coli %15 oranında üçüncü sıklıkta görülen bakteri oldu (p<0.001). Gram pozitif bakterilerde, direnç durumuna bakıldığında, S. aureus suşlarının %16.7'sinin metisiline dirençli olduğu görüldü. Enterokoklar dahil Gram pozitif bakterilerde glikopeptid direncine rastlanmadı. E. coli ve K. pneumoniae suşlarında GSBL oranı (%43.8; %48.5) diğer Gram negatif bakterilere oranla daha yüksek bulundu (p=0.493). En yüksek antibiyotik direnç oranı %70.8 ile ampisiline, en az direnç oranı %2 ile imipeneme karşı görüldü. Sonuç: Yara enfeksiyonlarının sık etkenlerinden olan S. aureus ve E. coli'nin çalışmamızda, başta gastrointestinal karsinomu olan hastalarda olmak üzere oldukça yüksek oranda saptanması nedeniyle ampirik tedavide bu etkenlerin antibiyotik duyarlılıklarının göz önünde bulundurulması gerektiği düşünülmüştür.
The differential diagnosis of TOA caused by C. septicum can be difficult, due to the lack of the symptoms. Tissues infected with C. septicum can become necrotic. A combination of early, adequate antibiotic therapy and surgery is the key point of the treatment.
We aimed to evaluate the epidemiological data of malaria cases by using the data of Gaziantep Public Health Directorate between January 2005 and December 2015. Methods: Blood samples were taken from suspicious cases and temporary agricultural workers living in the Fırat Watershed and in areas in where malaria was seen before and in the tent cities of Islahiye, Nizip and Karkamış by request of the ministry. The cases were evaluated in terms of age, gender, detected malaria species, months when malaria was detected, and whether they were imported cases. Results: Thirty-one malaria cases were detected in blood samples taken from 184.305 patients. The malaria positivity rate was determined as 0.017%. Five of the patients (16.3%) were indigenous; 2 (6.5%) were cases with relapse and 24 cases (77.2%) were imported. Conclusion: In our study, we last saw a new indigenous case in 2005. In the following years , cases with relapses and cases originating from abroad were seen, which was linked to the ease of transportation and the increase in touristic and commercial relations. Although lack of malaria in the samples taken from people living in tent cities in our region is favorable, necessary measures should be taken against new epidemics.
The presented study comprises the one-pot synthesis and the characterization of quercetin- and caffeic acid-functionalized chitosan-capped colloidal silver nanoparticles (Ch/Q- and Ch/CA-Ag NPs), and their antibacterial and anticancer activities. The formation of Ch/Q- and Ch/CA-Ag NPs has been confirmed by ultraviolet–visible (UV–vis) spectroscopy, Fourier-transform infrared (FTIR) spectroscopy, and transmission electron microscopy (TEM). The characteristic surface plasmon resonance (SPR) absorption band has been found at 417 and 424 nm for Ch/Q- and Ch/CA-Ag NPs, respectively. The formation of a chitosan shell comprising quercetin and caffeic acid, which surround the colloidal core Ag NPs, was confirmed by UV–vis, and FTIR analyses, and monitored by TEM microscopy. The size of nanoparticles has been determined as 11.2 and 10.3 nm for Ch/Q- and Ch/CA-Ag, respectively. The anticancer activity of Ch/Q- and Ch/CA-Ag NPs has been evaluated against U-118 MG (human glioblastoma) and ARPE-19 (human retinal pigment epithelium) cells. Both NPs showed anticancer activity, but Ch/Q-Ag NPs seemed to be more effective on cancer cell lines (U-118 MG) in comparison to healthy ones (ARPE-19). Furthermore, the antibacterial activity of Ch/Q- and Ch/CA-Ag NPs against Gram-negative (P. aeruginosa and E. coli) and Gram-positive (S. aureus and S. epidermidis) bacteria was determined, and dose-dependent antibacterial effects were found.
Hem dünyada hem de ülkemizde Escherichia coli üriner sistem enfeksiyonlarının en sık nedenidir. Gelişen bakteriyel direnç mekanizmaları bu enfeksiyonların tedavisini de kısıtlamaktadır. Tedavide kullanılacak antibiyotiklerin doğru bir antibiyotik duyarlılık testi sonucuna göre belirlenmesi gelişen antibiyotik direncini kontrol altına almakta büyük önem taşımaktadır. Hastanemizde görülen üriner sistem enfeksiyonları etkeni olan E. coli izolatlarının çeşitli antibiyotiklere karşı duyarlılıklarının ve direnç durumlarının belirlenmesi amaçlanmıştır.
