This is a qualitative empirical study of descriptive ethics conducted with nurses and physicians working in the Family Health Program (FHP) in the city of Sao Paulo, Brazil. The objective was to identify ethical problems experienced by these professionals, who were asked to record ethical problems they had faced through a short written description of such events. The results revealed ethical problems involving patients and their families, the health team, organization, and the health system. The study addresses ethical issues permeating daily health care practice and requiring immediate solutions, rather than the classical dilemmas usually found in bioethics literature. The peculiarity of ethical problems in primary care may hinder their identification as such and jeopardize the relationships upon which the FHP is built.
This review included 21 research studies, representing various countries and healthcare settings. There were 223 findings, which were aggregated into 15 categories, and three synthesized findings: CONCLUSIONS: This review shows that health professionals experience teamwork and interprofessional collaboration as a process in primary health care settings; its conditions, consequences (benefits and barriers), and finally shows its determinants. Health providers face enormous ideological, organizational, structural and relational challenges while promoting teamwork and interprofessional collaboration in primary health care settings. This review has identified possible actions that could improve implementation of teamwork and interprofessional collaboration in primary health care.
ABSTRACT:This study aimed to identify the elements and strategies that facilitate the ethical decision-making of nurses faced with ethical problems, from publications on the theme. An integrative literature review was carried out. Data collection was performed between the months of April and May 2014, in the databases: SciELO, CINAHL, LILACS and MEDLINE. Original, theoretical and case study articles, with a health team that included nurses, published in Portuguese, English and Spanish were included. A total of 19 studies were selected, the analysis of which resulted in three categories: external factors of ethical decision-making in nursing, individual factors of ethical decisionmaking in nursing and facilitating strategies of ethical decision-making in nursing. It was concluded that nurses need to use strategies that develop sensitivity, ability and ethical competence, in order to make prudent ethical decisions, contributing to the quality of health care. Estudo quantitativo exploratório para identificar e verificar a freqüência dos problemas éticos vividos por enfermeiros e médicos no PSF. Coleta de dados com questionário auto-aplicado, contendo situações geradoras de problemas éticos e escala de quatro pontos para a freqüência. Desrespeito do profissional para com o usuário; problemas relacionados às informações para usuários e famílias e à preservação da privacidade e confidencialidade apareceram na maioria dos questionários, embora com freqüências diferentes. Os resultados indicam que a bioética na atenção básica lida com situações do cotidiano e não com dilemas de maior apelo dramático. A sutileza desse panorama pode levar a não percepção dos problemas, reforçando a necessidade de capacitação dos profissionais em comunicação e acolhimento, para instituir uma atitude ética baseada na cidadania, solidariedade e humanismo.
The aim of this study is to identify ethical problems experienced by nurses in primary health care and resources for coping based on publications on the subject. An integrative literature review was performed between the months of October and November 2013, using the databases: BDTD, CINAHL, LILACS, MEDLINE, Biblioteca Cochrane, PubMed, RCAAP and SciELO. Articles, dissertations and theses published in Portuguese, English and Spanish were included, totalling 31 studies published from 1992 to 2013. This analysis resulted in four categories: ethical problems in the relationship between team members, ethical problems in the relationship with the user, ethical problems in health services management and resources for coping with ethical problems. Results showed that nurses need to be prepared to face ethical problems, emphasizing the importance of ethics education during the education process before and during professional practice to enhance the development of ethical sensitivity and competence for problem resolution.
Palliative Care (PC) is presented as a public health problem, due to the substantial increase in the number of elderly and the prevalence of non-communicable chronic degenerative diseases. The importance of the reorganization of Primary Health Care Services (PHCS) to ensure the provision of this care has become evident. The objective was to identify cases of users, in order to inventory the ethical problems that the team experienced. This is a case study based on a Qualitative approach, using thematic analysis. Two cases were identified, that revealed, as ethical problems, the "responsibility of the PHCS towards the caregivers and families" and the "difficulty of frank and honest communication between the team and the family". The results indicate that the training of human resources with technical competence and the continuity of assistance in the transition from curative to palliative treatment are factors favorable to the integrality and the obtention of more adequate responses to the ethical challenges that the teams experienced. Os resultados indicam que a formação de recursos humanos com competência técnica e que a continuidade da assistência na transição do tratamento curativo para o paliativo são fatores propícios à integralidade e à obtenção de respostas mais adequadas aos desafios éticos que as equipes vivenciam. The ageing population has increased the incidence of chronic health conditions, requiring the inclusion of different levels of palliative care (PC) in different parts of the health system, including the Primary Health Care (PHC). This new reality might interfere with the ethical issues of PHC. This research aimed to identify, from the point of view of health professionals, the ethical problems which arise from the palliative care in PHC. We carried out a systematic review in PubMed, EMBASE, LILACS, CINAHL, using the descriptors 'ethics', 'bioethic's, 'Primary Health Care' and 'Palliative Care'. We found 3,915 articles of which 16 remained after analyses. The ethical problems found were: lack of resources; lack of knowledge about palliative care; lack of communication skills; difficulty in establishing limits in clinical relationship; work overload; lack of support from referral services. These problems, in general are similar to those experienced in the PHC but with differences in specific situations. Isso poderá interferir nas questões éticas da APS. A presente pesquisa teve como objetivo identificar, na visão dos profissionais de saúde, os problemas éticos decorrentes da prática dos CP na APS. Fez-se revisão sistemática nas bases PubMed, Embase, Lilacs, CINAHL, com os descritores "ética", "bioética", "atenção primária à saú-de" e "cuidados paliativos". Localizaram-se 3.915 artigos, restando 16, após a análise. Os problemas éticos detectados foram: escassez de recursos; desconhecimento sobre CP; falta de habilidades comunicacionais; dificuldade de estabelecer limites na relação clínica; sobrecarga de trabalho; falta de apoio dos serviços de referência. Na abrangência, esses problemas assemelham-se aos vividos na APS, com diferenças nas situações específicas.
