Hematogenous thrombophilia is often encountered in the clinical practice of doctors of all specialties. The article presents current data on the diagnostic of this pathology. In most cases pulmonary embolism is a clinical onset of hereditary hematogenous thrombophilia. According to the authors of this article, pulmonary embolism occurred in 60% of surveyed patients with hereditary thrombophilia hematogenous. Current antithrombotic therapy is quite effective in this category of patients. Three cases of personal experience of the authors when there was diagnosed a combined form of hematogenous thrombophilia with serious thrombotic complications in anamnesis are shown. Adequate secondary prevention of thrombosis contributed to further disease-free course of the disease.
Membranous nephropathy (MN) is one of the most common causes of nephrotic syndrome in adults and is classified as either primary (idiopatic) or secondary MN according to underlying etiology (the later result from some known disease such as systemic autoimmune diseases, infections, malignancies, drugs, etc). In recent years, phospholipase A2 receptor 1 (PLA2R) and thrombospondin type-1 domain-containing 7A (THSD7A) were identified as two major podocytic antigens involved in the pathogenesis of idiopatic MN (IMN). And the discovery of circulating antibodies specific for these target antigens has transformed the diagnostic workup and significally improved management of IMN. However why do such antibodies develop is not conclusively established. The role of underlying genetic factors is discussed. The review presents the results of recent studies, that have shown significant associations of specific genetic factors (particularly human leucocyte antigen class II and PLA2R1 genes) with IMN.
In 185 patients with chronic hemoblastosis (chronic lymphocytic leukemia, chronic myeloid leukemia, idiopathic myelofibrosis, multiple myeloma) after autopsy, the pathology of the bronchopulmonary system was studied. It was found out that in addition to immunodeficiency, an important role in the occurrence of respiratory diseases in chronic lymphocytic leukemia, as well as in chronic myeloid leukemia and idiopathic myelofibrosis in the stage of blast crisis is played by specific leukemic infiltration of the lungs, bronchi, pleura and diaphragm; the presence of leukostasis in the vessels of medium and small caliber with violation of microcirculation; compression of the diaphragm by significantly increased spleen and liver; in some cases (especially in chronic lymphocytic leukemia) hyperplasia of the lymphoid follicles of the bronchial tree. In chronic myeloid leukemia and idiopathic myelofibrosis, hyperthrombocytosis with the development of the sludge syndrome in small vessels of the lungs is essential. Pulmonary localization of inflammatory processes in patients with multiple myeloma is facilitated by lymphoid and plasma cell infiltration of the lungs, paraproteinosis of the lungs, localization of myeloma nodes in the ribs, lung tissue and bronchi.
Мембранозная нефропатия (МН) -наиболее частая причина развития нефротического син-дрома у взрослых. В обзоре отражена более чем 60-летняя история изучения данного забо-левания, представлена эволюция в понимании его патогенеза -от эксперимента к клинике. Благодаря созданию в 1959 г. животной моде-ли МН (активный и пассивный Хеймановский нефрит) был идентифицирован почечный аутоантиген -белок мегалин, экспрессируе-мый в ножковых отростках подоцитов. В ходе экспериментальных исследований подтверж-дено формирование иммунных комплексов in situ из циркулирующих антител к мегалину, что ведет к активации комплемента с обра-зованием в субэпителиальном пространстве мембраноатакующего комплекса, вызываю-щего сублетальное повреждение подоцитов с реорганизацией их актинового цитоскелета, диссоциацией белков щелевидной диафрагмы, приводящих к увеличению проницаемости фильтрационного барьера и развитию про-теинурии. Так понимание природы идиопа-тической МН эволюционировало от иммуно-комплексного гломерулярного повреждения к подоцитопатии, и был открыт путь для по-иска других подоцитсвязанных антигенов. Механизмы подоцитарного повреждения были признаны ключевыми в развитии идиопатиче-ской МН и у человека, но в течение многих лет проводился поиск отличных от мегалина анти-генных мишеней, поскольку человеческие по-доциты этот белок не экспрессируют. В первом десятилетии XXI в. такие аутоантигены были идентифицированы -нейтральная эндопепти-даза, трансмембранный М-типа рецептор фос-фолипазы А 2 , домен тромбоспондина 1-го типа, содержащий 7А, -и установлена ведущая роль аутоантител, направленных на эти подоци-тарные мишени. Новые знания легли в основу создания современных методов диагностики и лечения МН. М ембранозная нефропатия (МН), или мембранозный гломерулонефрит -вариант иммунокомплексного по-ражения клубочков почек с разви-тием большой протеинурии. Из всех вариантов гломерулярного повреждения МН служит самой частой причиной развития нефротического син-дрома у взрослых. Название заболевания отража-ет патологические изменения, выявляемые в тка-ни почки, -утолщение гломерулярной базальной мембраны вследствие депозиции иммунных ком-плексов под ножками подоцитов и интрамем-бранозного отложения матриксного материала, продуцируемого пораженными подоцитами. Гистологические признаки этого заболевания впервые описаны более 60 лет назад. Но и в новом тысячелетии оно продолжает активно изучаться. Основу формирования иммунных комплексов при идиопатической МН составляет образование аутоантител к внутренним антигенам клубочка; за последние десятилетия достигнуты большие успехи в их идентификации. Вторичная МН представляет собой результат специфического поражения клубочков иммунными комплексами, в которые включены экзогенные (вирусные, опу-холевые и др.) антигены. The aim of the study was to assess the incidence of arterial lesions of various localizations in patients with giant cell arteritis (GCA) according to positron emission and computed tomography (PET/CT) with 18F-fluorodeoxyglucose (18F-FDG).Methods. Patients over 50 years of age diagnosed with GCA (with or without polymyalgia rheumatica) were included in a retrospective study. Damage to extracranial arteries was assessed based on the accumulation of 18F-FDG in the walls of large vessels according to PET/CT data.Results. The study included 47 patients, 14 (30%) men and 33 (70%) women, with a median age of 65 [57; 68] years. In 32 (68%) patients, 18F-FDG-PET/CT was performed before the start of therapy with glucocorticosteroids (GCS), in 15 (32%) after the start of treatment. In 40 (91%) patients, signs of damage to extracranial vessels of various localization were revealed, most often in the thoracic and abdominal aorta (72% and 64%, respectively).Conclusions. According to 18F-FDG-PET/CT data, in most patients with GCA, not only the arteries of the head and neck, but also vessels of other localization are involved in the pathological process.
