A educação interprofissional é uma estratégia pedagógica na qual duas ou mais profissões trabalham e aprendem juntas, priorizando a integração, com a finalidade de mudar o perfil profissional na área da saúde e melhorar a qualidade do cuidado e resolutividade na atenção primária. A disciplina Atenção em Saúde é uma proposta interprofissional, desenvolvida em sete cursos da área da saúde na Universidade Estadual de Maringá, oportunizando ao aluno vivência no Sistema Único de Saúde (SUS), utilizando a problematização de situações em equipe. Objetivou-se apresentar a avaliação dos estudantes de Odontologia sobre a disciplina Atenção em Saúde, por meio da análise das narrativas de seus portfólios avaliativos. Trata-se de um estudo exploratório e descritivo, com análise documental como técnica de investigação, com amostragem até saturação dos dados. A análise dos documentos permitiu mostrar que os acadêmicos ressaltam a importância do trabalho interprofissional, beneficiando trabalhadores e pacientes e a relevância do cirurgião-dentista na equipe de saúde. Constatou-se o conhecimento experienciado do aluno sobre o SUS, capaz de destacar falhas e qualidades, bem como o reconhecimento sobre a eficácia do modelo ativo de aprendizagem, para capacitá-los a enfrentar positivamente as adversidades. Conclui-se que os acadêmicos apontaram as práticas colaborativas interprofissionais e a utilização de metodologias ativas de ensino- aprendizagem como estratégias positivas para formação qualificada, estando em consonância com as Diretrizes Curriculares Nacionais de Odontologia quanto à formação profissional para o trabalho em equipe e compreensão das reais necessidades de saúde da população.
BackgroundTraumatic brain injuries (TBI) are a significant cause of mortality and morbidity for children globally. Adherence to evidence-based treatment guidelines have been shown to improve TBI outcomes. To inform the creation of a pediatric TBI management guideline for a low and middle income country context, we assessed the quality of available clinical practice guidelines (CPGs) for the acute management pediatric TBI.MethodsArticles were identified and retrieved from MEDLINE, EMBASE, Cochrane Library, LILACS, Africa-Wide Information and Global Index Medicus. These articles were screened by four reviewers independently. Based on the eligibility criteria, with the exception of literature reviews, opinion papers and editor’s letters, articles published from 1995 to November 11, 2016 which covered clinical recommendations, clinical practice or treatment guidelines for the acute management of pediatric TBI (within 24 hours) were included for review. A reference and citation analysis was performed. Seven independent reviewers from low, middle and high income clinical settings with knowledge of pediatric TBI management appraised the guidelines using the AGREE II instrument. Scores for the CPGs were aggregated by domain and overall assessment was determined.ResultsWe screened 2372 articles of which 17 were retained for data extraction and guideline appraisal. Except for one CPG from a middle income country, the majority (16/17) of the guidelines were developed in high income countries. Seven guidelines were developed specifically for the pediatric population, while the remaining CPGs addressed the acute management of TBI in both adult and pediatric populations. The New Zealand Guideline Group (NZGG, 2006) received the highest overall assessment score of 46/49 (93.88%) followed by the Scandinavian Neurotrauma Committee (SNC, 2016) with a score of 45/49 (91.84%) followed by the Scottish Intercollegiate Guideline Network (SIGN, 2009) and Brain Trauma Foundation (BTF 2012) both with scores of 44/49 (89.80%). CPGs from Cincinnati Children’s Hospital (CCH 2006) and Sao Paulo Medical School Hospital/Brazilian Society of Neurosurgery (USP/BSN, 2001) received the lowest score of 27/49 (55.10%) subsequently followed by the Appropriateness Criteria (ACR, 2015) with 29/49 (59.18%). The domains for scope and purpose and clarity of presentation received the highest scores across the CPGs, while applicability and editorial independence domains had the lowest scores with a wider variability in score range for rigor of development and stakeholder involvement.ConclusionsTo our knowledge, this is the first systematic review and guideline appraisal for pediatric CPGs concerning the acute management of TBI. Targeted guideline creation specific to the pediatric population has the potential to improve the quality of acute TBI CPGs. Furthermore, it is crucial to address the applicability of a guideline to translate the CPG from a published manuscript into clinically relevant local practice tools and for resource limited practice settings.
RESUMOO tratamento da dor é um dos assuntos mais presentes em pacientes internados nas Unidades de Terapia Intensiva Pediátrica (UTIP). O objetivo deste trabalho foi caracterizar o manejo da dor, realizado pelas equipes de enfermagem nas UTIPs. Foi aplicado um questionário, que analisou, de forma quantitativa, o conhecimento sobre o manejo da dor, em equipes de enfermagem de dois hospitais. Os sujeitos da pesquisa foram 35 profissionais de enfermagem (técnicos de enfermagem e enfermeiros). Dos entrevistados, 94,29% responderam ser importante avaliar a dor de forma rotineira na UTIP, 77,14% dos pesquisados nunca participaram de nenhum treinamento para avaliar a dor em pediatria. Vinte e sete participantes relataram que a presença de um acompanhante facilita a avaliação da dor na UTIP. A maioria disse que associam estratégias farmacológicas com não-farmacológicas e 94,29% avaliam novamente a dor após a implantação de ações para o alívio da mesma. Para as equipes de enfermagem pesquisadas, o tratamento da dor na UTIP é importante, contribuindo para a estabilidade clínica do paciente, porém a equipe não se mostra apta para realizar o manejo da dor. É necessário o treinamento contínuo da equipe de enfermagem visando habilitá-los para avaliar e tratar a dor na UTIP, beneficiando os pacientes e contribuindo para a assistência humanizada. Palavras-chave:Avaliação da dor. Pediatria. Cuidados intensivos. ABSTRACTPain treatment is one the most present issues among patients hospitalized at a pediatric intensive care unit (PICU). This work aims to characterize pain management as carried out by the nursing staff at PICUs. A survey was applied to nursing teams from two different hospitals, analyzing quantitatively their knowledge of pain management. The sample consisted of 35 nursing professionals -nursing technicians and nurses. Among the interviewed employees, 94.29% answered that it is important to routinely evaluate pain at the PICU, but 77.14% had never attended any training to evaluate pain in pediatrics. Twenty-seven participants said the presence of a patient companion at the PICU makes pain management evaluation easier. Most said they combine pharmacological with non-pharmacological strategies, and 94.29% evaluate the pain again after implementing different actions for pain relief. For the nursing staff interviewed, pain treatment at the PICU is important, contributing for the clinic stability of the patient, however, the staff has not shown to be able to carry out pain management. Continuous training by the nursing staff is needed, aiming to enable them to evaluate and treat pain at PICU to benefit the patients and contribute to the humanization of care.
Resumo Na educação interprofissional (EIP), duas ou mais profissões de saúde aprendem uma sobre a outra, aprimorando atitudes, conhecimento, habilidades e comportamentos para a prática colaborativa. Na Universidade Estadual de Maringá (UEM), os cursos de saúde apresentam currículos com disciplinas, hierarquizados e centrados no professor. Justificam-se mudanças em busca de uma formação integral. Este trabalho descreve a criação de disciplinas que atuam na Atenção Primária. Os componentes curriculares denominados Atenção em Saúde I e II foram introduzidos na matriz curricular. Nas atividades, foi utilizado o Arco de Maguerez, como metodologia ativa e um sistema de avaliação composto de eixos cognitivos, psicomotor e afetivo, articulados, contínuos e sistemáticos. Conclui-se que a EIP pode ser a integração entre universidade e os serviços de saúde trabalhando pela formação dos estudantes, na qual o conhecimento contribua de forma fundamental para o bem-estar da comunidade.
Este estudo tem por objetivo caracterizar o atendimento do profissional de Enfermagem no Programa Saúde da Família (PSF), direcionado ao atendimento pediátrico, de forma a integrar a equipe e qualificar as informações para os familiares. Trata-se de estudo observacional, descritivo de uma coorte transversal, em que participaram 102 indivíduos, o qual analisa, de forma quantitativa, os dados coletados por um questionário com 12 questões, sobre o atendimento de Enfermagem, obtendo opiniões dos responsáveis pelas crianças até cinco anos, atendidas no ambulatório de pediatria de uma Unidade Básica de Saúde. Dos entrevistados, 92,86% receberam atendimento de Enfermagem antes da consulta pediátrica. 96,94% se diziam seguros com o acompanhamento da enfermeira e 95,92% receberam orientações nos aspectos que precisavam, quanto ao cuidado da criança. Quando perguntados sobre a avaliação geral do atendimento da equipe, 46,94% responderam ser ótimo, 46,94% acharam bom e 6,12%, regular. O modelo interprofissional contribuiu para a melhoria da atuação da equipe e na promoção à saúde, visando a melhoria da qualidade no processo de trabalho e na assistência à criança.
