Pendahuluan: Telah dilakukan produksi protease dari bakteri termofilik isolat LS-1 yang dikulturkan dalam media cair mengandung natrium sitrat. Tujuan: Penelitian ini bertujuan untuk mendapatkan kondisi optimum produksi protease pada isolat bakteri termofilik lumpur Sidoarjo. Metode: Produksi protease dilakukan pada berbagai waktu, sumber karbon dan nitrogen. Karakterisasi protease dilakukan dengan menggunakan substrat azokasein dengan pengaruh berbagai suhu, pH dan pengaruh ion logam. Hasil: Produk enzim mencapai maksimum pada 10 jam dengan aktivitas 1,85U/mg protein. Beberapa sumber karbon yang dibutuhkan untuk produksi protease dalam penelitian ini telah dioptimasi. Amilum adalah substrat terbaik, diikuti oleh natrium sitrat, asam sitrat dan sukrosa. Di antara berbagai sumber nitrogen organik dan anorganik NH4NO3 telah diketahui yang terbaik. Studi karakterisasi protease yang diperoleh dalam penelitian ini menunjukkan suhu optimum pada 60ºC. Enzim stabil selama 2 jam pada suhu 30ºC, sementara pada suhu 40ºC dan 80ºC, menurun masing masing 16% dan 86% dari aktivitas awal. Pencapaian pH optimum enzim diketahui 8,0. Setelah larutan enzim kasar dinkubasi selama 24 jam pada pH 5,5, 8,0 dan 9,0, terjadi penurunan sekitar masing-masing 49%, 15% dan 63% dari aktivitas sebelumnya. Pengaruh K+, Hg2+ dan Cu2+ pada konsentrasi 1mM sebagai inhibitor kuat sehingga mengakibatkan hilangnya aktivitas. Ion yang berkontribusi mempengaruhi aktivitas adalah Mn2+ dan Ca2+, yang menunjukkan bahwa ion ini memiliki peran fungsional dalam struktur molekul enzim. Kesimpulan: Produksi protease optimum oleh bakteri termofilik isolat LS-1 pada waktu 10 jam. Amilum adalah substrat terbaik untuk produksi protease. Suhu dan temperatur optimum aktivitas protease masing masing pada suhu 60ºC dan pH 8,0. Aktivitas Protease dipengaruhi oleh ion logam Mn2+ dan Ca2+.
The use of traditional medicine, in general, is considered safer than modern medicine. This study aims to determine the relationship between frequency of traditional medicines usage, the type and amount of traditional medicines consumed on traditional drug user's health-related quality of life.This research was an observational study conducted in a cross-sectional way. The study was conducted in the city of Surabaya in 2019. Calculation of sample size required for 400 people using the Slovin formula. Inclusion Criteria of respondents were domiciled in Surabaya, had or were taking traditional medicine, aged over 17 years. The research instrument was in the form of a Questionnaire and Short Form 6 Dimension (SF6D) sheet that had been validated for the measurement of the quality of life for health. The instruments were distributed using the Snowball Sampling technique with Google Forms. While manual distribution using random sampling techniques to the people of Surabaya.The collected data were analyzed with the Spearman correlation test and the Kruskal Wallis test. All analyses were carried out with the help of SPSS.The results show that the frequency, type and amount of traditional medicine consumed had a p>0.05 to health-related quality of life. The behavior of traditional medicine usage shows that there is no significant relationship to health-related quality of life to the health of the Surabaya community. Penggunaan sediaan farmasi yang tidak rasional merupakan masalah kesehatan. Dampaknya terjadi pada efek klinis hingga efek ekonomi. Pada masa Jaminan Kesehatan Nasional (JKN), Puskesmas berperan dalam memberi layanan kesehatan tingkat pertama diharapkan mampu memberikan kualitas layanan yang optimal sehingga masalah kesehatan masyarakat tidak sampai berlanjut pada layanan tingkat lanjutan. Penggunaan sediaan farmasi yang tidak rasional diharapkan seminimal mungkin terjadi sehingga keselamatan pasien dapat terjaga. Penelitian ini dilakukan secara observasional dan bertujuan untuk memberikan gambaran profil penggunaan obat rasional berdasarkan indikator yang telah ditetapkan World Health Organization (WHO). Data penelitian diambil di wilayah kabupaten pasuruan pada tahun 2020. Data yang dikumpulkan berasal dari laporan indikator peresepan di puskesmas pada Dinas Kesehatan Kabupaten selama kurun waktu tahun 2019 dan direkap kembali menggunakan form pelaksanaan POR. Hasil yang diperoleh menunjukan bahwa penggunaan sediaan farmasi telah memenuhi standar WHO dari parameter penggunaan antibiotik pada ISPA non pneumoni dan diare, penggunaan injeksi pada myalgia, namun belum memenuhi pada rata-rata item obat per lembar resep. Penggunaan sediaan farmasi di puskesmas secara rata-rata Kabupaten Pasuruan telah memenuhi standar WHO namun masih perlu dilakukan tindaklanjut perbaikan terhadap puskesmas yang pelaksanaan penggunaan sediaan farmasinya belum memenuhi standar WHO
ABSTRAKMasalah ketersediaan obat selalu terjadi pada tiap Kabupaten/ Kota setiap tahun. Penelitian terkait pengelolaan obat khususnya yang berkaitan dengan evaluasi ketersediaan obat dan nilai obat rusak/ kadaluarsa telah banyak dilakukan dalam sepuluh tahun terakhir. Hampir seluruhnya menemukan adanya permasalahan pada ketersediaan obat. Dirjen Farmalkes melaporkan bahwa persentase Ketersediaan Obat Nasional di Propinsi Jawa Timur pada tahun 2017 telah mencapai 96,55% namun angka tersebut belum menggambarkan kondisi ketersediaan obat secara riil karena perhitungan tidak menggunakan data kuantitas obat. Penelitian ini bertujuan untuk menjelaskan kondisi riil ketersediaan 17 item obat-obat indikator nasional tahun 2017 di Propinsi Jawa Timur, serta mengidentifikasi masalah dan informasi seputar pengelolaan yang telah berjalan di tahun 2017. Penelitian ini adalah penelitian observasional yang bersifat deskriptif kuantitatif. Instrumen penelitian berupa lembar pengumpulan data ketersediaan 17 item obat tahun 2017 dan pedoman wawancara. Hasil penelitian menunjukan bahwa ketersediaan obat berada pada kondisi berlebih dengan rata-rata mencapai 22,5 bulan. Obat program yang diadakan langsung oleh Pemerintah Pusat sering mengalami kelebihan persediaan di tingkat Instalasi Farmasi Kabupaten/ Kota namun rata-rata persentase obat rusak/ kadaluarsa masih pada rentang yang cukup baik (0-3%). Hal ini dapat disimpulkan bahwa ketersediaan obat di Propinsi Jawa Timur pada tahun 2017 dipengaruhi oleh sistem perencanaan, sistem pengadaan, ketersediaan obat di tingkat penyedia (suplayer), ketersediaan sistem informasi manajemen dan kebijakan daerah masing-masing. Research related to drug management, especially those related to the evaluation of drug availability and the value of damaged/expired drugs, has been carried out in the last ten years. Almost all the research found problems with the availability of the drugs. The Director General of Pharmacy and Medical Devices reported that the percentage of the availability of National Medicines in East Java in 2017 had reached 96.55%, but had not yet described the condition of the availability of medicines in real terms because the calculation did not use data on drug quantity. This study aims to explain the real conditions for the availability of 17 items of national indicator drugs in East Java in 2017, as well as identify problems and information about management that has been running in 2017. This research is an observational research that is descriptive quantitative.
