Unaccustomed smoking may elicit transient nystagmus, dizziness, unsteadiness, and nausea. Infrared videonystagmography and posturography were performed simultaneously to study the differential effects of nicotine on the association of ocular motor and postural disturbances in 25 non- or occasional smokers. Sixteen showed nicotine-induced nystagmus (NIN) of various directions (mainly horizontal or upbeat) which was associated with a significant increase in postural sway after smoking a cigarette (total sway path (SP) before smoking 2.22 +/- 0.82 m/min (mean +/- s.d.), 1 min after smoking 3.83 +/- 1.41 m/min; p < 0.0004, ANOVA); nine showed neither effect. There was a high correlation between the intensity of the nystagmus (measured as peak slow phase velocity) and the increase in total SP (correlation coefficient 0.78) as well as the time courses of both. Visual fixation of an LED integrated in the mask not only caused a suppression of NIN but also a decrease in body sway. Transient ocular motor and postural effects are compatible with simultaneous nicotine-induced effects on the vestibulo-ocular and vestibulo-spinal functions.
In spite of the fact that musical hallucination have a significant impact on patients' lives, they have received very little attention of experts. Some researchers agree on a combination of peripheral and central dysfunctions as the mechanism that causes hallucination. The most accepted physiopathology of musical hallucination associated to hearing loss (caused by cochlear lesion, cochlear nerve lesion or by interruption of mesencephalon or pontine auditory information) is the disinhibition of auditory memory circuits due to sensory deprivation. Concerning the cortical area involved in musical hallucination, there is evidence that the excitatory mechanism of the superior temporal gyrus, as in epilepsies, is responsible for musical hallucination. In musical release hallucination there is also activation of the auditory association cortex. Finally, considering the laterality, functional studies with musical perception and imagery in normal individuals showed that songs with words cause bilateral temporal activation and melodies activate only the right lobe. The effect of hearing aids on the improvement of musical hallucination as a result of the hearing loss improvement is well documented. It happens because auditory hallucination may be influenced by the external acoustical environment. Neuroleptics, antidepressants and anticonvulsants have been used in the treatment of musical hallucination. Cases of improvement with the administration of carbamazepine, meclobemide and donepezil were reported, but the results obtained were not consistent. Key words: musical hallucination, auditory hallucination, hearning loss, deafness.Alucinações musicais associadas a perda auditiva RESUMO Apesar das alucinações musicais causarem grandes repercussões na vida dos pacientes, sempre foram pouco valorizadas e estudadas pelos profissionais. Alguns investigadores sugerem uma combinação de disfunções periféricas e centrais como o mecanismo causador das alucinações. A fisiopatologia mais aceita entre os pesquisadores de alucinação musical associada à hipoacusia ou anacusia (causada por lesão coclear, de nervo coclear ou interrupção de informação na ponte ou mesencéfalo) é a desibinição de circuitos de memória auditiva devido à deprivação sensorial. Em relação às áreas corticais envolvidas na alucinação musical, há evidência de que um mecanismo excitatório no córtex temporal superior, como nas epilepsias, seja responsável pela alucinação musical. Finalmente, considerando a lateralidade, estudos funcionais de percepção e imagética em indivíduos normais mostraram que canções com letras levam a ativação temporal bilateral e melodias ativam apenas o lobo temporal direito. É bem documentado o efeito de aparelhos auditivos na alucinação musical através de uma melhora da perda auditiva.
RESUMO -A vertigem de posicionamento paroxística benigna é um distúrbio vestibular de diagnóstico fácil, baseado no exame neurológico e tratado com manobras de liberação. With a simple maneuver one can easily diagnose this condition and treatment is based on liberatory maneuvers. Essas freqüências são explicadas pela relação anatômica dos canais com o utrículo: a posição do CSC posterior facilita o depósito de otólitos soltos no seu interior, enquanto a entrada de partí-culas no canal anterior é muito mais difícil e portanto incomum. O CSC horizontal por sua vez tem uma abertura mais larga e movimentos de rotação lateral (por exemplo, ao virar na cama durante o sono) facilitam a saída das partículas. FISIOPATOLOGIA Cupulolitíase x canalolitíaseA teoria da cupulolitíase foi proposta por Schuknecht em 1969 ao encontrar, em estudos anátomo-patológicos, depósitos basofílicos aderidos à cúpu-la do CSC de indivíduos que apresentavam VPPB antes do óbito 3 . O autor argumentou que estes depó-sitos basofílicos, compostos por partículas inorgâ-nicas, se desprenderiam da mácula do utrículo e em virtude da ação da gravidade cairiam no CSC posterior, cuja abertura está situada inferiormente ao utrí-culo, e ficariam aderidas à cúpula. A teoria da canalolitíase também admite que pequenos fragmentos de carbonato de cálcio se soltam da mácula utricular e caem nos CSCs, mas contrariamente à teoria da
O sistema vestibular tem a função de estabilização da imagem na retina, manutenção da postura e do equilíbrio, orientação espacial e percepção do movimento, requerendo a integração com os sistemas motor ocular, visual e proprioceptivo 1 . Os órgãos otolíticos, utrículo e sáculo, fornecem informações ao sistema vestibular sobre movimentos de aceleração linear da cabeça e contribuem para a percepção de orientação espacial, que é a capacidade de um indivíduo perceber a vertical e horizontal gravitacionais 2 . As síndromes vestibulares são diagnosticadas por meio da anamnese e do exame físico dirigidos ao sistema vestibular 3 . Os exames complementares de avaliação da função vestibular colaboram com a confirmação diagnóstica, fornecendo dados e/ou registros que muitas vezes são capazes de quantificar o comprometimento deste sistema, tal como o teste da Vertical Visual Subjetiva (VVS).Em situações habituais, o julgamento de verticalidade e horizontalidade é realizado principalmente através das informações visuais, pois há, em qualquer ambiente, linhas verticais e horizontais que auxiliam nesta função. Quando as informações visuais são excluídas, por exemplo, em uma sala completamente escura, uma pessoa normal é capaz de julgar se um bastão iluminado está na posição vertical ou horizontal na ausência de qualquer outra aferência visual. Subjective visual vertical evaluation in normal Brazilian subjectsABSTRACT -Otolith function can be evaluated by subjective visual vertical (SVV) that determine the capacity of a subject to judge if the objects are on vertical position with absence of any visual reference. The aim of this study was to evaluate the SVV in a sample of normal Brazilian subjects using a portable device. Measurements of SVV were performed in 160 normal subjects (aged from 16 to 85). SVV mean value was obtained after ten adjustments. SVV mean values ranged from -2.0º to +2.4º (mean=0.18º, and SD=0.77). Considering all age groups, there was no difference of SVV mean values (Kruskal-Wallis test; p=0.40), but older groups had a greater variance (Levene test; p=0.016). SVV values observed in this study are comparable to those described in previous studies. Although there was no difference in mean SVV-inclination according to age, there was a greater variance in older subjects.
RESUMO -Os objetivos deste estudo foram identificar as síndromes vestibulares mais comuns nos ambulatórios de vertigem, suas características clínicas e semiológicas, e observar a resposta ao tratamento específico. F o r a m estudados retrospectivamente 515 pacientes atendidos em ambulatórios de duas instituições e avaliados aspectos da anamnese, exame físico e a resposta ao tratamento. As síndromes mais freqüentes foram: vertigem de posicionamento paroxística benigna (VPPB) (28,5%), vertigem postural fóbica (11,5%), vertigem central (10,1%), neurite vestibular (9,7%), doença de Menière (8,5%), enxaqueca (6,4%). Houve boa resposta ao tratamento nos pacientes com enxaqueca (78,8%), VPPB (64%), neurite vestibular (62%), doença de Menière (54,5%) e paroxismia vestibular (54,5%), enquanto pacientes com nistagmo para baixo e vestibulopatia bilateral não tiveram resposta satisfatória (52,6% e 42,8% respectivamente). As síndromes vestibulares foram diagnosticadas através da anamnese e exame físico com testes clínicos específicos para avaliação da função v e s t i b u l a r. A identificação destas síndromes permitiu o tratamento adequado levando a uma boa evolução.PALAVRAS-CHAVE: vertigem, etiologia, diagnóstico, tratamento. Diagnosis and treatment of the most frequent vestibular syndromesABSTRACT -The aims of this study were to identify the most common vestibular syndromes in a dizziness unit, and to observe their clinical aspects and response to treatment. Five hundred and fifteen patients were studied retrospectively in two institutions. Aspects of anamnesis, physical examination and the response to treatment were evaluated. The most frequent syndromes were: benign paroxysmal positioning vertigo (VPPB) (28.5%), phobic postural vertigo (11.5%), central vertigo (10.1%), vestibular neuritis (9.7%), Menière disease (8.5%), and migraine (6.4%). A good response to treatment was observed in most patients with migraine (78.8%), VPPB (64%), vestibular neuritis (62%), Menière disease (54.5%) and vestibular paroxismia (54.5%). On the other hand, patients with downbeat nystagmus and bilateral vestibulopathy had poor response (52.6% and 42.8%, respectively). The diagnosis of these most frequent vestibular syndromes were established through anamnesis and physical examination (with specific clinical tests for evaluation of the vestibular function). The correct diagnosis and adequate treatment are important since these syndromes may have a good prognosis.
Two- and three-dimensional analyses of nicotine-induced eye movements were performed in 53 subjects to evaluate whether they were primarily of vestibular or ocular motor origin. Nicotine-induced nystagmus was detected in 27 subjects (51%); in 25 of these (93%) it was modulated by otolith input. Three-dimensional analysis of nicotine-induced nystagmus revealed that it violates Listing's law. Taken together, these findings suggest that nicotine induces an imbalance in the vestibulo-ocular reflex.
Cannabinoids comprehend endocannabinoids, phytocannabinoids, and synthetic cannabinoids, with actions both in the central and peripherical nervous systems. Scientific Departments from the Brazilian Academy of Neurology described evidence for medical use in their areas. Literature is constantly changing, and possible new evidence can emerge in the next days or months. Prescription of these substances must be discussed with patients and their families, with knowledge about adverse events and their efficacy.
