| Neck pain is the most common symptom of cervical dysfunctions often being related to the maintenance of postures. Postural changes of the head are often associated with the occurrence of neck pain with an anterior approach was the most frequent. The purpose was to investigate the correlation between head posture, pain intensity and neck disability index. The study group (SG) was composed of women, aged between 20 and 50 years who complained of neck pain for more than three months, and the control group (CG) for asymptomatic women. Pain intensity was assessed by visual analogue scale (VAS), disability by neck disability index (NDI) and the head posture by the craniovertebral angle (CV). Normality of the data was verified by the Lilliefors test and the comparison between groups by the Student's t-test for independent samples, and the association between variables by Spearman correlation test. Significance level was 5%. The SG had lower average CV for the angle (p=0.02). The CV angle was negatively correlated with the VAS (r=-0.48) and NDI (r=-0.15) suggesting that the smaller the angle, the greater the intensity of neck pain and disability. The NDI and VAS showed a positive correlation (r=0.59). The angle CV in subjects with neck pain was significantly lower than in healthy individuals association with the neck disability index and pain.Keywords | neck pain; posture; head.RESUMO | A dor cervical é o sintoma mais comum das disfunções cervicais, frequentemente relacionado à manutenção de posturas inadequadas. As alterações posturais da cabeça estão associadas à ocorrência de dor cervical, sendo a anteriorização a alteração mais comum. O objetivo foi investigar a correlação entre postura da cabeça, intensidade da dor e índice de incapacidade cervical -neck disability index (NDI). O grupo estudo (GE) foi composto por mulheres na faixa entre 20 e 50 anos com queixas de dor cervical por mais de três meses, e o grupo controle (GC), por mulheres assintomáticas. A intensidade da dor foi avaliada pela escala visual analógica (EVA), a incapacidade pelo NDI e a postura da cabeça pelo ângulo craniovertebral (CV). A normalidade dos dados foi verificada pelo teste de Lilliefors, e a comparação entre os grupos pelo teste t de Student para amostras independentes e a associação entre as variáveis pela correlação de Spearman. O nível de significância foi de 5%. O GE apresentou mé-dias menores para o ângulo CV (p=0,02). O ângulo CV demonstrou correlação negativa com a EVA (r=-0,48) e o NDI (r=-0,15), sugerindo que quanto menor o ângulo, maior a intensidade da dor e a incapacidade cervical. O NDI e a EVA apresentaram correlação positiva (r=0,59). Este estudo analisa os efeitos da associação das terapias de imagem motora e de movimento induzido por restrição na reeducação funcional do membro superior (MS) de um paciente com deficit sensorial e motor determinado por acidente vascular encefálico (AVE). A terapia de imagem motora (IM) consistiu em: 1o, estimulo visual do espelho, em 3 sessões semanais de 30 a 60 minutos por 4 semanas; e 2o, IM com prática mental, em 3 sessões semanais de 15 minutos por 3 semanas. Por último foi aplicada a terapia de indução ao movimento por restrição do membro superior não-afetado por 14 dias, em 10 dos quais foi feita atividade funcional do membro parético por 6 horas diárias. Além da avaliação clinica da sensibilidade e medida da força de preensão palmar, antes do tratamento e após cada modalidade de terapia foi medida a amplitude de movimentos de ombro, cotovelo e punho e aplicada a escala de avaliação motora (EAM). Os escores dos quatro momentos da coleta foram comparados estatisticamente. Após o tratamento os resultados mostraram diferença significativa (p<0,05) com aumento da amplitude de movimentos em todas as articulações do MS e na força de preensão palmar; redução no tempo de execução de tarefas da função de braço e mão na EAM; e recuperação clinica da sensibilidade, especialmente tátil e sensação de pressão. No paciente estudado a associação da IM e da terapia de movimento induzido por restrição foi eficaz na recuperação funcional do membro superior parético pós-AVE.
Few studies have addressed early cerebrovascular lethality in Brazil. OBJECTIVE: To evaluate 10 and 28-day stroke case-fatality rates in three hospitals in three Brazilian cities. METHODS: We described the stroke registries in São Paulo, João Pessoa, and Natal. RESULTS: Out of a total of 962 first-ever events (mean age, 68.1 years-old; 53% men), 83.6% (804 cases) were classified as ischemic and 16.4% (158) as hemorrhagic stroke. Overall, the case-fatality rates and 95% confidence intervals (95%CI) for hemorrhagic stroke events were higher than for ischemic events, both at 10 (12.3%; 95%CI 7.2-17.4 versus 7.0%; 95%CI 5.3-8.8) and at 28 days (19.8%; 95%CI 13.6-26.0 versus 11.1%; 95%CI 8.9-13.3). CONCLUSIONS: We did not find any substantial differences in early case-fatality rates according to stroke subtypes, when comparing the three centers.
OBJETIVO: Este estudo se propôs a verificar se a estimulação por meio da equoterapia é capaz de desencadear alterações no controle postural de portadores de esclerose múltipla (EM). MÉTODO: Fizeram parte deste estudo 11 portadores de EM divididos em Grupo Intervenção (GI) e Grupo Controle (GC). O GI foi inserido num programa de hipoterapia durante 4 meses, sendo conduzidas 2 sessões semanais com duração de 50 minutos cada. A estabilidade postural foi avaliada utilizando uma plataforma de força (para calcular o deslocamento do centro de pressão (COP), durante 30 segundos, em postura ereta quasi-estática, com olhos abertos e fechados, antes e após o treinamento com equoterapia. O tratamento estatístico foi feito através do ANOVA e Post hoc de Tukey com p<0,05. RESULTADOS: A amplitude de oscilação anteroposterior reduziu significativamente no GI após a estimulação, enquanto o GC manteve um comportamento inalterado entre as avaliações. Em ambos os grupos, a estabilidade postural foi reduzida com a supressão da informação visual. CONCLUSÕES: A adaptação funcional proporcionada pela equoterapia foi capaz de melhorar a estabilidade postural dos portadores de EM. Os resultados corroboram a literatura que suporta o uso da equoterapia como uma intervenção em potencial nas desordens de controle postural em portadores de EM.
Background. Priming results in a type of implicit memory that prepares the brain for a more plastic response, thereby changing behavior. New evidence in neurorehabilitation points to the use of priming interventions to optimize functional gains of the upper extremity in poststroke individuals. Objective. To determine the effects of priming on task-oriented training on upper extremity outcomes (body function and activity) in chronic stroke. Methods. The PubMed, CINAHL, Web of Science, EMBASE, and PEDro databases were searched in October 2019. Outcome data were pooled into categories of measures considering the International Classification Functional (ICF) classifications of body function and activity. Means and standard deviations for each group were used to determine group effect sizes by calculating mean differences (MDs) and 95% confidence intervals via a fixed effects model. Heterogeneity among the included studies for each factor evaluated was measured using the I2 statistic. Results. Thirty-six studies with 814 patients undergoing various types of task-oriented training were included in the analysis. Of these studies, 17 were associated with stimulation priming, 12 with sensory priming, 4 with movement priming, and 3 with action observation priming. Stimulation priming showed moderate-quality evidence of body function. Only the Wolf Motor Function Test (time) in the activity domain showed low-quality evidence. However, gains in motor function and in use of extremity members were measured by the Fugl-Meyer Assessment (UE-FMA). Regarding sensory priming, we found moderate-quality evidence and effect size for UE-FMA, corresponding to the body function domain (MD 4.77, 95% CI 3.25-6.29, Z = 6.15, P < .0001), and for the Action Research Arm Test, corresponding to the activity domain (MD 7.47, 95% CI 4.52-10.42, Z = 4.96, P < .0001). Despite the low-quality evidence, we found an effect size (MD 8.64, 95% CI 10.85-16.43, Z = 2.17, P = .003) in movement priming. Evidence for action observation priming was inconclusive. Conclusion. Combining priming and task-oriented training for the upper extremities of chronic stroke patients can be a promising intervention strategy. Studies that identify which priming techniques combined with task-oriented training for upper extremity function in chronic stroke yield effective outcomes in each ICF domain are needed and may be beneficial for the recovery of upper extremities poststroke.
El GE obtuvo la descarga de peso más grande en el sentido anterior y menor en el sentido posterior (p=0,02), además de menores valores de las presiones medianas de los pies derecho (p=0,00) e izquierdo (p=0,01) en relación al GC. La gran parte de los niños presentó el mismo tipo de pie bilateralmente, con prevalencia de pies planos en el GE y cavos en el GC. Los niños con PC espástica, que presentaron el desempeño locomotor preservado o con alguna disfunción, revelaron antepulsión corporal, menores presiones plantares medianas y prevalencia del pie plano, en comparación a los niños con DT.
RESUMO: Avaliar a influência da correção da idade na detecção de riscos no desenvolvimento motor de prematuros. Trata-se de um estudo observacional retrospectivo, com 59 prematuros, atendidos no Ambulatório de Seguimento de Prematuros de um hospital-escola de uma cidade da região Central do Rio Grande do Sul, nos anos de 2014 e 2015. As crianças foram avaliadas a partir da Albert Infant Motor Scale aos quatro e oito meses. A normalidade dos dados foi verificada pelo teste de Shapiro-Wilk e a comparação dos grupos foi realizada a partir do Teste de Wilcoxon. Utilizando a idade corrigida, 39% dos prematuros avaliados aos quatro meses apresentaram alto risco para atraso do desenvolvimento motor, assim como 33% dos prematuros avaliados aos oito meses. Este resultado foi mais acentuado quando a idade cronológica foi utilizada (p=0,000). A correção da idade gestacional se mostrou como a melhor forma de avaliar o desempenho real dos bebês nascidos pré-termo, sem superestimar riscos que não se configuram de fato em problemas no desenvolvimento das crianças.ABSTRACT: Current paper assesses the influence of age correction when risks in the motor development of premature children are detected. A retrospective and observational study has been conducted with 59 premature children attended to at the Premature Children Clinic of a school hospital in the central region of the state of Rio Grande do Sul, Brazil, between 2014 and 2015. The children were evaluated by the Albert Infant Motor Scale, at ages four and eight months. Data normality was verified by the Shapiro-Wilke´s test and group comparison was undertaken by Wilcoxon´s Test. 39% of the premature children evaluated on month four revealed high risk levels for delay in motor development; similar results were obtained for 33% of premature children evaluated on month eight. Results were more significant when chronological age was employed (p=0.000). The correction of gestation age proved to be the best way in evaluating the true performance of pre-term children, without highly estimating risks which do not actually emerge into problems in children´s development.
A Organização Mundial de Saúde considera prematuro aquele nascido com idade gestacional entre 20 a 37 semanas. Com os avanços tecnológicos existentes nas Unidades de Terapia Intensiva Neonatal, tem-se maior sobrevida destes, que apresentam desenvolvimento neuromotor inferior aos recém-nascidos a termo. Este estudo objetivou caracterizar o desenvolvimento motor e variáveis clínicas de prematuros nascidos em um hospital público. Tratase de um estudo misto. A pontuação na Escala Motora Infantil de Alberta, peso ao nascimento, idade gestacional, tempo de internação na Unidade de Terapia Intensiva Neonatal e Apgar de 1 o e 5 o minuto de prematuros com idade corrigida entre 0 e 18 meses foram coletadas dos prontuários, entre maio de 2013 a abril de 2014. Para análise estatística, utilizou-se o teste do Qui-Quadrado (nível de significância p<0,05). Foram incluídos 267 prematuros, com mediana de idade gestacional de 32(30 -33) semanas, de tempo de internação de 35 (25 -58,5) dias e de peso médio de 1430 (11-75 -1690) gramas. Foi encontrado em 10,49% das crianças o Apgar no 5 o minuto inferior a 7. Verificou-se que 36,89% dos prematuros entre 0 e 5 meses, 39,6% entre 06 e 12 meses e 23,5% entre 13 e 18 meses estavam com riscos ou atrasos motores evidentes. Não foi observada correlação entre o Apgar do 5 o minuto e o desenvolvimento motor. A maioria das crianças entre 06 e 12 meses apresentou risco para atrasos motores, justificando a importância do seguimento de prematuros em serviços de referência, mesmo após o primeiro ano de vida. ABSTRACTThe World Health Organization considers preterm those born with a gestational age between 20 and 37 weeks. With the technological advances in Neonatal Intensive Care Units, we have seen a higher survival rate among preterms, who present lower neuromotor development than full term newborns. The aim of this study was to characterize the motor development and clinical variables of preterm infants born in a public hospital. It is a mixed study. The score on the Alberta Infant Motor Scale, birth weight, gestational age, time of stay in the Neonatal Intensive Care Unit, and the 1st and 5th minute Apgar of preterm infants with ages between 0 and 18 months corrected age, were collected from their files between May 2013 and April 2014. For statistical analysis, the Chisquare test (significance level of p < 0.05) was used. A total of 267 premature babies were included, with median gestational age of 32 (30-33) weeks, duration of stay of 35 (25-58.5) days, and mean weight of 1,430 (1,175-1,690) grams. A 5th minute Apgar score below 7 was found in 10.49% of the children. It was found that 36.89% of the infants between 0 and 5 months, 39.6% between 6 and 12 months, and 23.5% between 13 and 18 months of age were at risk of or had evident motor delay. No correlation was observed between the 5th minute Apgar and motor development.
