OBJECTIVE: To identify the prevalence and the severity of malocclusions and to analyze factors
associated with the need for orthodontic treatment of Brazilian adolescents. METHODS: This exploratory, cross-sectional study was carried out based on secondary data from
the national epidemiological survey on oral health in Brazil (2002-2003).
Socio-demographic conditions, self-perception, and the existence and degree of
malocclusion, using the Dental Aesthetic Index, were evaluated in 16,833 adolescent
Brazilians selected by probabilistic sample by conglomerates. The dependent variable
need orthodontic treatment was estimated from the severity of malocclusion. The
magnitude and direction of the association in bivariate and multivariate analyzes from a
Robust Poisson regression was estimated. RESULTS: The majority of the adolescents needed orthodontic treatment (53.2%). In the
multivariate analysis, the prevalence of the need for orthodontic treatment was larger
among females, non-whites, those that perceived a need for treatment, and those that
perceived their appearance as normal, bad, or very bad. The need for orthodontic
treatment was smaller among those that lived in the Northeast and Central West
macro-regions compared to those living in Southeast Brazil and it was also smaller among
those that perceived their chewing to be normal or their oral health to be bad or very
bad. CONCLUSIONS: There was a high prevalence of orthodontic treatment need among adolescents in Brazil
and this need was associated with demographic and subjective issues. The high prevalence
of orthodontic needs in adolescents is a challenge to the goals of Brazil's universal
public health system.
Objetivo: Relatar os desafios de um exercício de calibração de examinadores para estudo epidemiológico sobre maloclusão envolvendo variáveis quantitativas e categóricas ordinais, apresentando os resultados da consistência diagnóstica intra e interexaminadores. Materiais e Métodos: Voluntários foram selecionados antecipando condições que os examinadores encontrariam em campo. Maloclusão foi avaliada pelo DAI, cujos critérios foram expostos no treinamento teórico e reforçados no prático. Os examinadores foram divididos em grupos e os dados coletados por exame. As concordâncias inter e intraexaminadores foram estimadas pelo CCI (Coeficiente de Correlação Intraclasse) para o DAI geral e o CCI ou Kappaponderado (Kw) para os componentes do DAI, utilizando-se os programas PASW e Excel. Os examinadores com concordância inferior a 0,60 foram considerados inaptos e deveriam ser submetidos a mais um exercíciode calibração. Resultados: Em três exercícios de calibração, demorados e dispendiosos, 33 examinadores em sete grupos, examinaram e re-examinaram 315 escolares. As concordâncias interexaminadores para oDAI geral, entre os pares de examinadores, variaram de -09 a 0,99 (pobre à excelente). A concordância de todos os examinadores em conjunto, por grupo, variou de 0,51 a 0,96, (razoável a excelente). Na análise porcomponentes, concordâncias entre os pares de examinadores >0,60 nos sete grupos foram encontradas para dentição, apinhamento e diastema. Discordâncias entre pares de examinadores para alguns dos componentesforam observadas mesmo em grupos cuja concordância entre o mesmo par de examinadores foi satisfatória para DAI geral. Pela concordância intra-examinadores para o DAI geral, de 0,37 a 1 (pobre a excelente), dois examinadores foram inaptos; na avaliação por componentes do DAI, 11 apresentaram CCI e Kw<0,60. Conclusão: A calibração de examinadores configura-se como um desafio para os pesquisadores demandandotempo e esforço. A concordância obtida por componentes do DAI identifica maior número de examinadores inaptos, garantindo assim maior acurácia aos resultados do levantamento epidemiológico.Descritores: Avaliação. Objetivo: desenvolver e verificar a validade, confiabilidade einterpretabilidade de um instrumento que se propõe a avaliaro Letramento em Saúde quanto ao Hábito Etilista (LSHE).Método: estudo metodológico realizado entre pessoas comdiabetes cadastradas na Estratégia de Saúde da Família econstituído pelas seguintes etapas: desenvolvimento doLSHE; verificação da validade de conteúdo por um comitê dejuízes; pré-teste (n=20); estimativa da confiabilidade (n=62):Alfa de Cronbach (AC), kappa de Cohen (K) e Coeficientede Correlação Intraclasse (CCI), resultados satisfatórios(≥ 0,60); estimativa da validade concorrente (n=212);interpretabilidade dos escores (n=212): variam de 0 a 18,sendo o ponto de corte ≤ 14 (LSHE inadequada). Utilizou-se oprograma SPSS para as análises estatísticas. Resultados: oLSHE apresentou aplicação dinâmica e adequada, mostrandoserelevante quanto ao seu conteúdo e ao construto propostos.As 18 palavras apresentaram K > 0,60, AC=0,82 e CCI=0,91.Houve correção do LSHE com a escolaridade (rs=0,537;p=0,000). Interpretabilidade: 31,6% (n=67) apresentaramLSHE inadequada. Conclusão: o LSHE foi consideradovalidado, confiável e com boa interpretabilidade.
