BackgroundIntima-media thickness (IMT) of the common carotid artery is a surrogate end point of cardiovascular disease (CVD). Identifying the factors associated with a higher IMT may contribute to the identification of subjects with higher CVD risk. Our objective was to compare the common carotid IMT of type 1 diabetes patients to healthy control subjects. The secondary objective was to determine factors associated with a higher carotid IMT.MethodsWe conducted a cross-sectional study between March 2009 and October 2013, comprising 127 type 1 diabetes patients and 125 control subjects matched by age, gender and body mass index (BMI). Carotid IMT was measured using semi-automated edge detection software.ResultsType 1 diabetes patients had a higher median IMT compared with control subjects (0.538; IQR: 0.500-0.607 vs 0.513 mm; IQR: 0.481-0.557, respectively p = 0.001). Women with type 1 diabetes had a higher median IMT difference compared to the control group (0.537; IQR: 0.495-0.596 vs 0.502 mm; IQR: 0.472-0.543, respectively p = 0.003) than did men with type 1 diabetes (0.547; IQR: 0.504-0.613 vs 0.528 mm; IQR: 0.492-0.575, respectively p = 0.2). Age and diabetes duration had an additive effect on the IMT of type 1 diabetes patients. Multivariate gamma regression model analysis showed that in type 1 diabetes patients, the IMT was associated with age (Exp (β) = 1.006, p < 0.001), duration of diabetes (Exp (β) = 1.004, p = 0.001), BMI (Exp (β) = 1.005, p = 0.021), family history of type 2 diabetes (Exp (β) = 1.044, p = 0.033), total cholesterol (Exp (β) = 0.999, p = 0.001) and creatinine clearance (Exp (β) = 1.000, p = 0.043).ConclusionsPatients with type 1 diabetes have increased IMT, a marker of subclinical atherosclerosis. The CVD risk may be similar between men and women with type 1 diabetes, suggesting a loss of gender protection. Also, CVD risk may be higher in those with a family history of type 2 diabetes. Prospective studies are needed to confirm the predictive value of these findings and the causal effect between IMT and CVD in patients with type 1 diabetes.

Intensive insulin therapy in patients with type 1 diabetes is resulting in a better clinic and metabolic control. On the other hand, an increase in overweight and obesity prevalence, which could contribute to a higher risk of cardiovascular disease, has been observed. The aim of our study is to analyze the demographics, clinical and laboratorial factors associated to the presence of dyslipidemia in a group of patients with type 1 diabetes compared to a non-diabetic population. We have studied 72 type 1 diabetics: 52.8% female, aged 22.7 +/- 9.6 years old, with a body mass index (BMI) of 21.1 +/- 3.1 Kg/m2 and 66 non-diabetic patients: 60.6% female, aged 23.1 +/- 10.9 years old and a BMI of 22.12 +/- 3.7 Kg/m2. The group included 13 children (6 with type 1 diabetes), 47 adolescents (23 with type 1 diabetes) and 78 adults (43 with type 1 diabetes). We have found in the adult population with type 1 diabetes a lower overweight prevalence and lower levels of apoB (p< 0.01) and a higher apoA/apoB ratio (p< 0.01) when compared to the non-diabetic population. We have not found difference in the lipid profile in adult groups. Diabetic children and adolescents had higher frequency of total cholesterol (p= 0.02 and p< 0.01, respectively) and LDL-cholesterol (p= 0.02 and p= 0.01 respectively) above the upper limit when compared to the non-diabetic group. We concluded that the conventional methods for detecting lipids alterations in outpatients with type 1 diabetes under routine care are not sufficient to identify the lipid alterations, which could be related to the higher risk of cardiovascular disease in adult population.

We evaluated the influence of body adiposity (BA), which was measured by bioelectrical impedance, body mass index (BMI) and waist circumference (WC), in clinical and laboratorial parameters of 64 patients with type 1 diabetes (DM1), 33 females, matched for diabetes duration. Women had greater BA than men. Fourteen patients were overweight. In the whole group, we found correlations between BA and BMI (r= 0.50; p= 0.001), BA and WC (r= 0.30; p= 0.001) and BA and fasting glucose (r= 0.24; p= 0.048). There were 11 patients with abnormal BA; among them, there were 6 with overweight and abnormal WC. In those patients with abnormal BA, we found higher HbA1c, respectively [(9.8 +/- 2.4) vs. (8.1 +/- 1.5%); p= 0.03], WC [(82.9 +/- 11.4) vs. (72.9 +/- 8.3 cm); p = 0.01] and BMI [(26.1 +/- 2.7) vs. (22.1 +/- 2.5 Kg/m2); p= 0.0001]. We conclude that some DM1 patients can have some characteristics of the metabolic syndrome and the influence of these findings on clinical and laboratory control and on the cardiovascular risk must be analysed in prospectives studies.

A forma não-clássica da hiperplasia adrenal congênita (HAC) por deficiência da 21-hidroxilase (D21OH) é rastreada com a 17-hidroxiprogesterona basal (17OHPb) e seu diagnóstico confirmado pelo teste do ACTH. Como o nível de 17OHPb que indica a realização do teste ainda é motivo de discussão, este estudo teve o objetivo de avaliar este valor no diagnóstico de HAC por D21OH. O teste do ACTH (0,25 mg IV) foi realizado em 87 mulheres de 24,8±0,8 anos com hiperandrogenismo clínico. O valor de 17OHP ≥1.500 ng/dl após 60min do ACTH foi considerado sugestivo de D21OH, a qual foi diagnosticada em 11 casos (12,6%). Nos outros 76, o teste não foi compatível com o diagnóstico. A sensibilidade, especificidade e valores preditivos positivo e negativo para 17OHPb >200 ng/dL foram 0,91, 0,62, 0,26 e 0,98, respectivamente. Para 17OHPb >350 ng/dL estes valores foram 0,91, 0,91, 0,59 e 0,99 e para 17OHPb >500 ng/dL foram 0,82, 0,99, 0,9 e 0,97. Os achados clínicos (hirsutismo, irregularidade menstrual e obesidade) e as dosagens de androstenediona e testosterona total foram semelhantes entre os dois grupos. Concluímos que os achados clínicos e os níveis de androgênios não permitem diferenciar os pacientes com e sem D21OH. Sugerimos realizar o teste do ACTH quando a 17OHPb for >350 ng/dL, valor com alta sensibilidade e bom valor preditivo negativo. Screening for non-classic congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency (21OHD) is performed with baseline 17-hydroxyprogesterone (17OHPb) and confirmed with the ACTH test. Because the cutoff level of 17OHPb that prompts an ACTH testing is still a matter of discussion, we evaluated the levels of 17OHPb in diagnosing CAH caused by 21OHD. An ACTH (0.25 mg IV) test was performed in 87 hyperandrogenic women, 24.8±0.8 years old. 17OHP levels greater than 1,500 ng/dL, 60min after administration of ACTH were considered diagnostic. 21OHD was confirmed in 11 patients (12.6%); in the remainder 76 the test was negative. Sensitivity, specificity, positive and negative predictive values for 17OHPb >200 ng/dL were 0.91, 0.62, 0.26 and 0.98, respectively. For 17OHPb >350 ng/dL these values were 0.91, 0.91, 0.59 and 0.99 and for 17OHPb >500 ng/dL, 0.82, 0.99, 0.9 and 0.97. Clinical findings (hirsutism, menstrual irregularities and obesity) and levels of androstenedione and total testosterone were similar between both groups. T1D patients have an increased VA, a marker of subclinical atherosclerosis. The use of VA age may contribute to the identification of high CVD risk in T1D. In patients with T1D, a younger chronological age, particularly in women, might not be a protective factor for CVD.

Caries experience, plaque amount, diet frequency and fluoride sources are valuable predictors of caries risk; microbiological tests are not necessary for evaluating caries risk in children, which can be assessed similarly by CS without SM and FBC.
