Arq Bras Endocrinol Metabvol 47 nº 1 Fevereiro 2003 62 RESUMOAvaliamos a influência da gordura corporal (GC) determinada por bioimpedância, índice de massa corpórea (IMC) e a medida da cintura abdominal (CA) em determinadas variáveis clínicas e laboratoriais em 43 pacientes com diabetes tipo 2 (DM2), 26F/17M, pareados pela idade, com 13,6±9,1 anos de duração do DM. As mulheres apresentaram maior IMC: 30,3±5,4 vs. 26,9±2,9kg/m 2 (p= 0,04), GC: 35,4±6,2 vs. 19,6±6,2% (p= 0,000), níveis de colesterol total (CT): 235±41 vs. 204± 39mg/dL (p= 0,017), triglicerídeos (TG): 146± 61 vs. 116±57mg/dL (p= 0,06) e HbA1c (HPLC): 7,1±1,7 vs. 6,9±1,4% (p= 0,02) do que os homens, não havendo diferença entre HDL e LDL colesterol. A GC correlacionou-se significativamente com os níveis de TG, HbA1c, pressão arterial diastólica (PAD), IMC e CA. Na regressão múltipla com GC, CA e IMC como variáveis independentes e TG (r= 0,34; r 2 = 0,11; p= 0,02) e PAD (r= 0,39; r 2 = 0,15; p= 0,008) como dependentes, a GC foi a variável independente significativa. No mesmo modelo com a HBA1c como variável dependente, a GC (r= 0,31; r 2 = 0,10; p= 0,037) e o IMC (r= 0,43; r 2 = 0,19; p= 0,01) foram as variáveis significativas. Concluímos que o aumento da GC no paciente com DM2 constituiu um importante fator de risco para piora do controle metabólico e dos níveis tensionais. As mulheres, por terem tido maior percentual de GC e níveis de lipídios, devem ter uma abordagem mais agressiva e diferenciada para sua redução. 116± 57mg/dL (p= 0.06) and HbA1c (HPLC): 7.1±1.7 vs. 6.9±1.4% (p= 0,02) than men, but HDL and LDL cholesterol were not different. In the stepwise multiple regression analysis using BA, WC and BMI as independent variables and TG (r= 0.34; r 2 = 0.11; p= 0.02) and DBP (r= 0.39; r 2 = 0.15; p= 0.008) as the dependent ones, BA was found to be statistically significant. By using the same model having HbA1c as the dependent variable, BA (r= 0.31; r 2 = 0.10; p= 0.037) and BMI (r= 0.43; r 2 = 0.19; p= 0.01) became significant. Diabetic Pregnancy: Outpatient Follow-up in a Brazilian University Hosp i t a l .In this study we assessed neonatal complications of diabetic in 50 pregnant women at a University Hospital during 2001-2002: 13 (26%) with type 1 diabetes (DM1), 16 with DM2, and 21 (42%) with gestational DM (GDM). The mean outpatient follow-up was at 16.3±8 wk for patients with DM1, 22.9±7.5 wk for DM2, and 26.0±8.9 wk for GDM. Mean HbA1c, fasting and 2-h post-prandial glycemia on first attendance were respectively: 6.1±1,1% (RV: 2.6-6.2%), 132±39mg/dL and 190±54mg/dL. 22 patients were on insulin and 15 were on oral antidiabetic agents (OA) at first evaluation. OA were taken on conception and during the first pregnancy trimester and no malformations were seen in the children. Their metabolic profile was similar to other pregnant women. Caesarean section was needed in 54.5% of deliveries. Complications: 56.1% were macrosomic babies, with a mean fetal weight of 3.48±0.73Kg, with no differences according to treatment (insulin vs. OA). We conclude that diabetic pregnant women begin their prenatal care at a later period, often taking OA that are not officially advised to be used during pregnancy and are not in a regular metabolic control. Routine use of Framingham score, which is feasible and noninvasive, could identify such patients and institute precocious and intensive measures in order to reduce their cardiovascular risk.
A forma não-clássica da hiperplasia adrenal congênita (HAC) por deficiência da 21-hidroxilase (D21OH) é rastreada com a 17-hidroxiprogesterona basal (17OHPb) e seu diagnóstico confirmado pelo teste do ACTH. Como o nível de 17OHPb que indica a realização do teste ainda é motivo de discussão, este estudo teve o objetivo de avaliar este valor no diagnóstico de HAC por D21OH. O teste do ACTH (0,25 mg IV) foi realizado em 87 mulheres de 24,8±0,8 anos com hiperandrogenismo clínico. O valor de 17OHP ≥1.500 ng/dl após 60min do ACTH foi considerado sugestivo de D21OH, a qual foi diagnosticada em 11 casos (12,6%). Nos outros 76, o teste não foi compatível com o diagnóstico. A sensibilidade, especificidade e valores preditivos positivo e negativo para 17OHPb >200 ng/dL foram 0,91, 0,62, 0,26 e 0,98, respectivamente. Para 17OHPb >350 ng/dL estes valores foram 0,91, 0,91, 0,59 e 0,99 e para 17OHPb >500 ng/dL foram 0,82, 0,99, 0,9 e 0,97. Os achados clínicos (hirsutismo, irregularidade menstrual e obesidade) e as dosagens de androstenediona e testosterona total foram semelhantes entre os dois grupos. Concluímos que os achados clínicos e os níveis de androgênios não permitem diferenciar os pacientes com e sem D21OH. Sugerimos realizar o teste do ACTH quando a 17OHPb for >350 ng/dL, valor com alta sensibilidade e bom valor preditivo negativo. Screening for non-classic congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency (21OHD) is performed with baseline 17-hydroxyprogesterone (17OHPb) and confirmed with the ACTH test. Because the cutoff level of 17OHPb that prompts an ACTH testing is still a matter of discussion, we evaluated the levels of 17OHPb in diagnosing CAH caused by 21OHD. An ACTH (0.25 mg IV) test was performed in 87 hyperandrogenic women, 24.8±0.8 years old. 17OHP levels greater than 1,500 ng/dL, 60min after administration of ACTH were considered diagnostic. 21OHD was confirmed in 11 patients (12.6%); in the remainder 76 the test was negative. Sensitivity, specificity, positive and negative predictive values for 17OHPb >200 ng/dL were 0.91, 0.62, 0.26 and 0.98, respectively. For 17OHPb >350 ng/dL these values were 0.91, 0.91, 0.59 and 0.99 and for 17OHPb >500 ng/dL, 0.82, 0.99, 0.9 and 0.97. RESUMOObjetivo: Avaliar a presença de microalbuminúria em indivíduos não diabéticos, associando-a à presença de fatores de risco cardiovasculares como hipertensão arterial, tabagismo, dislipidemia e obesidade. A taxa de excreção urinária de albumina (EUA) foi avaliada em relação aos índices de secreção e resistência insulínica (RI). Pacientes e Métodos: 105 indivíduos com idade de 33,4 ± 1,4 anos (57,1% mulheres) foram submetidos ao TOTG com 75 g de dextrose, sendo avaliadas as curvas de glicose e insulina: valores basais e em 2h, valores de pico e áreas sob a curva (ASC). Para a avaliação da secreção e RI, utilizamos os índices: insulinogênico, delta, HOMA, QUICKI, relação glicose/insulina e relação entre os índices insulinogênico e HOMA. As amostras para avaliação da albuminúria foram colhidas overnight. Os indivíduos foram divididos em dois grupos: 1) tolerância normal à glicose e 2) alteração do metabolismo glicídico. Resultados: Houve diferença entre os 2 grupos para idade, IMC, PA, cintura, RCQ, colesterol, triglicerídeos (TG), glicemias (GJ e G2h), ASCg, índices HOMA e QUICKI, e relação entre os índices insulinogênico e HOMA. A EUA foi de 4,28 ± 2,73 µg/mL, apresentando correlação com PAD, GJ, G2h, ASCg, VPG, HOMA, I2h, VPI e ASCi. Após regressão em stepwise, apenas ASCg foi preditora de EUA. Na comparação da amostra estratificada em quartis de EUA, o 1º e o 4º quartis foram estatisticamente diferentes para IMC, PAS, PAD, cintura, quadril, G2h, TG, LDL, ASCg, ASCi, VPG e índices HOMA e QUICKI. Conclusão: Embora não houvesse nenhum indivíduo com microalbuminúria, encontramos diferença entre a EUA em indivíduos com diferentes graus de tolerância à glicose e diferenças entre as variáveis clínicas e laboratoriais entre o 1º e o 4º quartis de EUA. Nossos achados sugerem que em indivíduos não diabéticos o aumento da EUA está relacionado a algumas características da sín-drome metabólica, o que pode conferir uma maior suscetibilidade aterogênica. Sua patogênese está relacionada ao aparecimento de anticorpos anti-insulina, e o tratamento baseia-se em imunossupressão. Apresentamos aqui o caso de uma paciente de 33 anos, com diagnóstico de diabetes desde a infância, que referia uso de hipoglicemiantes orais durante a adolescência. Durante o acompanhamento em nosso serviço, a dose de insulina foi progressivamente reduzida até ser substituída por hipoglicemiantes orais. Permaneceu 11 meses com esquema de glibenclamida, metformina e acarbose até ser internada em coma hiperosmolar não-cetótico. Após internação prolongada, recebeu alta usando insulina NPH, sendo necessário o aumento da dose nos meses subseqüentes. Quando atingiu a dose de 2,7U/Kg/dia, foi investigada e excluída a possibilidade de diabetes secundário, sendo diagnosticada resistência insulínica imunológica. Foram tentados diversos esquemas imunossupressores sem sucesso. A paciente está atualmente em uso de bomba de infusão subcutânea de insulina Lispro, micofenolato mofetil e prednisona com melhora do controle glicêmico. Immunologic insulin resistance is an entity recognized in the clinical practice for many years. Its pathogenesis is related to the presence of antiinsulin antibodies and treatment is based upon immunossuppression. We hereby present a case of a 33-year-old patient with the diagnosis of diabetes since the childhood that reported using oral hypoglicemic drugs during adolescence. During follow up, the insulin dose was gradually reduced until she was switched to oral hypoglicemic agents. She remained for 11 months in a regimen of glyburide, metformin and acarbose when she was admitted in a hyperosmolar non-ketotic coma. At discharge she was using insulin NPH and in the following months the dose needed to be gradually increased, as she remained hyperglycemic, until we reached 2.7U/Kg/day. Secondary diabetes was investigated and discarded, and she was diagnosed with immunologic insulin resistance. Several immunossuppressive regimens were tried with disappointing results. She is actually using a continuous subcutaneous infusion pump with insulin lispro, micophenolate mophetil and prednisone with improving glycemic control.
