RESUMOSessenta e seis adolescentes, sendo 38 obesos (25F/13M) e 28 não obesos (23F/5M), pareados pelo sexo, idade, cor; nível de escolaridade e estágio de maturação sexual, foram submetidos a um inquérito clínico-demográfico e à avaliação clínico-laboratorial. A relação cinturaquadril foi maior no grupo dos obesos do que nos não obesos (0,86±0,08 vs. 0,74±0,04; p< 0,01), assim como as medidas de pressão arterial sistóli-ca e diastólica (120,2±12,1 vs. 105,4±9,1mmHg; p<0,01 e 74,3±7,7 vs. 65,5±9,4mmHg; p<0,01, respectivamente). A lesão dermatológica acanthosis nigricans predominou no grupo dos obesos (n= 24, 63,2%). Os obesos apresentaram valores de HDL-colesterol inferiores aos dos não obesos (36,5+10,5 vs. 43,0±9,2mg/dl, respectivamente; p< 0,05). Os níveis de triglicerídeos e ácido úrico foram maiores no grupo dos obesos se comparado aos não obesos (124,6±80,0 vs. 74,2±31,4mg/dl, respectivamente; p< 0,01 e 5,8±1,4 vs. 4,5±1,0mg/dl, respectivamente; p< 0,01). Não houve diferença nos valores de colesterol, LDL colesterol e na glicemia basal entre os dois grupos. Os adolescentes obesos procuraram tratamento na maioria das vezes movidos por uma preocupação com a saúde (n= 15, 39,5%). Um número significativo desses adolescentes já havia feito uso de medicamentos visando a perda ponderal no passado. Concluímos que a obesidade na adolescência pode estar relacionada a um perfil clínico-metabólico desfavorável, caracterizado por níveis mais elevados de pressão arterial sistólica e diastólica, triglicerídeos, ácido úrico, e mais reduzidos de HDL-colesterol; e ainda pela presença de acanthosis nigricans. ■ AbstractMajor clinical studies have shown that the targets for blood glucose, lipid profile and blood pressure in type 2 diabetic patients are difficult to maintain in clinical practice. However, there are few data concerning South American populations. Using guidelines represented by the Brazilian Diabetes Society, we evaluated cardiovascular (CV) risk factors, glycemic control and the availability of screening for diabetes complications in 2233 (60% females) outpatients with type 2 diabetes aged 59.2 ± 11.3 yr and with a known duration of diabetes of 9.2 ± 7.2 yr, collected from 8 Brazilian cities. The outcome showed that less intensive clinical care available for diabetic patients in Brazil compared to western industrialized countries leads to widespread poor metabolic control and health status. Less than 30% of the patients reached the target for systolic (28.5%, < 130 mmHg) and diastolic (19.3%, < 80 mmHg) blood pressure, BMI (24.6%, < 25 kg/m 2 ), LDL cholesterol (20.6%, < 2.6 mmol/l) and only 46% reached the goal for HbA1c (one % point above the upper limits of normality for the method used). Only 0.2% of patients reached all the targets. A lower number of women reached the targets for HbA1c, LDL and HDL cholesterol than men (p < 0.001). Women were less likely than men to have funduscopy examinations and urine albumin testing (p < 0.001). We conclude that the national goals for glycemic control, blood pressure and lipid levels are rarely achieved in clinical practice, and that the availability for diabetic complication screening is low. The quality of diabetes care, in particular for women, is poor and should be further reviewed in developing countries.

ObjectiveTo evaluate the capability of intravoxel incoherent motion (IVIM) diffusion-weighted imaging (DWI) to assess steatohepatitis and fibrosis determined by histopathology in type 2 diabetic patients.MethodsFifty-nine type 2 diabetic patients (49 women, 10 men; mean age, 54 ± 9 years) were submitted to liver biopsy for the evaluation of non-alcoholic fatty liver disease (NAFLD) and underwent DWI on a 3.0T MR system using 10 b values. Institutional approval and patient consent were obtained. Pure molecular-based (D), perfusion-related (D*), and vascular fraction (f) were calculated using a double exponential model and least squares curve fitting. D, D*, and f were compared between patients with and without steatohepatitis and between patients with and without fibrosis. The variables were compared by using the Ranksum test and Student t-test.ResultsSteatohepatitis was observed in 22 patients and fibrosis in 16 patients. A lower D median (0.70 s/mm2 vs. 0.83 s/mm2, p<0.05) and a lower D* median (34.39 s/mm2 vs. 45.23 s/mm2, p<0.05) were observed among those with steatohepatitis. A lower D median (0.70 s/mm2 vs. 0.82 s/mm2, p<0.05) and a lower D* median (35.01 s/mm2 vs. 44.76 s/mm2, p=0.05) were also observed among those with fibrosis.ConclusionIVIM-DWI has the potential to aid in the characterization of steatohepatitis and fibrosis.

Arq Bras Endocrinol Metabvol 47 nº 1 Fevereiro 2003 62 RESUMOAvaliamos a influência da gordura corporal (GC) determinada por bioimpedância, índice de massa corpórea (IMC) e a medida da cintura abdominal (CA) em determinadas variáveis clínicas e laboratoriais em 43 pacientes com diabetes tipo 2 (DM2), 26F/17M, pareados pela idade, com 13,6±9,1 anos de duração do DM. As mulheres apresentaram maior IMC: 30,3±5,4 vs. 26,9±2,9kg/m 2 (p= 0,04), GC: 35,4±6,2 vs. 19,6±6,2% (p= 0,000), níveis de colesterol total (CT): 235±41 vs. 204± 39mg/dL (p= 0,017), triglicerídeos (TG): 146± 61 vs. 116±57mg/dL (p= 0,06) e HbA1c (HPLC): 7,1±1,7 vs. 6,9±1,4% (p= 0,02) do que os homens, não havendo diferença entre HDL e LDL colesterol. A GC correlacionou-se significativamente com os níveis de TG, HbA1c, pressão arterial diastólica (PAD), IMC e CA. Na regressão múltipla com GC, CA e IMC como variáveis independentes e TG (r= 0,34; r 2 = 0,11; p= 0,02) e PAD (r= 0,39; r 2 = 0,15; p= 0,008) como dependentes, a GC foi a variável independente significativa. No mesmo modelo com a HBA1c como variável dependente, a GC (r= 0,31; r 2 = 0,10; p= 0,037) e o IMC (r= 0,43; r 2 = 0,19; p= 0,01) foram as variáveis significativas. Concluímos que o aumento da GC no paciente com DM2 constituiu um importante fator de risco para piora do controle metabólico e dos níveis tensionais.
