Benign paroxysmal positional vertigo (BPPV) is a most common cause of dizziness and usually a self-limited disease, although a small percentage of patients suffer from a permanent form and do not respond to any treatment. This persistent form of BPPV is thought to have a different underlying pathophysiology than the generally accepted canalolithiasis theory. We investigated 5 patients who did not respond to physical treatment, presented with an atypical concomitant nystagmus or both with high-resolution three-dimensional magnetic resonance imaging of the inner ear. This method provides an excellent imaging of the inner ear fluid spaces. In all 5 patients, we found structural changes such as fractures or filling defects in the semicircular canals which we did not find in control groups. One patient clinically presented with the symptoms of a ‘heavy cupula’. Whereas crosssections through the ampullary region and the adjoining utricle showed no abnormalities, there were significant structural changes in the semicircular canals, which are able to provide an explanation for the symptoms of a heavy cupula.
According to the canalolithiasis theory, benign paroxysmal vertigo (BPPV) is caused by gravity-dependent movements of otoconial debris that collects in the endolymph of the posterior semicircular canal. Other parts of the vestibular organ are rarely affected, and it is mainly the horizontal canal that is affected by this atypical form of BPPV. Canalolithiasis of the superior semicircular canal must be considered an anomaly because the superior semicircular canal is the highest point of the vestibular organ and debris normally cannot collect in this special location. Until now, BPPV of the superior canal has mainly been dealt with theoretically in the literature. The authors present three patients with canalolithiasis of the superior semicircular canal and offer direct proof of the condition using high-resolution 3D MRI.
The development of a radiolabelled somatostatin analogue Indium-111-Pentetreotide makes the detection of somatostatin receptor-bearing tumours by scintigraphic techniques possible. The existence of high-affinity binding sites for somatostatin has been described previously for most endocrine active tumours of the gastroenteropancreatic system (GEP), malignant lymphomas, small cell lung carcinomas, a subgroup of breast tumours and several types of neuroendocrine related human tumours. Using this new diagnostic tool we investigated some head and neck tumours of neuroendocrine origin (carcinoid of larynx, Merkel cell carcinoma, paragangliomas) with the newly developed radiolabelled somatostatin analogue Indium-111-Pentetreotide whether in vivo visualisation of somatostatin receptors might be possible. In cases not accessible for surgery but with a positive receptor status we started a specific therapy with the somatostatin analogue octreotide. The preliminary results suggest that this new isotopic scanning technique in a diagnostic tool and a predictive method for an effective therapy of those head and neck tumours which revealed highly specifically a positive receptor status. The therapeutical results using the somatostatin analogue octreotide indicate that this new concept is an ideal therapeutic strategy for those neuroendocrine head and neck tumours which cannot be controlled by surgical procedures.
Es wird Über eine 35jÄhrige Patientin mit beidseitigen Tumoren des Glomus caroticum und Glomus jugulare berichtet. Die Wertigkeit der verschiedenen diagnostischen Verfahren (B-Bildsonographie, Farb-Doppler-Sonographie, hochauflÖsende Computertomographie, Angiographie, Kernspintomographie und Somatostatinrezeptorszintigraphie) sowie die unterschiedlichen Therapiekonzepte werden diskutiert. Trotz fehlender positiver Familienanamnese erscheint auch bei Patienten mit sporadisch auftretenden Glomustumoren eine lebenslange Kontrolle erforderlich.
Thirty patients were examined with CT of the menisci. All findings were confirmed by arthroscopy. One false positive diagnosis of a torn meniscus was made, caused by partial volume effect. In seven patients with irregular degenerative changes of the menisci, the abnormality could be clearly demonstrated. One meniscus ganglion was shown. Seven meniscus lesions, which were not shown by arthrography or arthroscopy, were clearly demonstrated by CT. The computer tomographic demonstration of menisci can replace knee arthrography as the examination of choice and should be performed before carrying out arthroscopy.
Ein korrektes prÄoperatives Tumorstaging – und hier insbesondere die Beurteilung des Lymphknotenstatus – ist bei der Erarbeitung eines optimalen Therapieplanes bei Patienten mit Plattenepithelkarzinomen des Kopf-Hals-Bereiches von entscheidender Bedeutung. RoutinemÄssig wurden morphologisch orientierte Verfahren wie Computertomographie und/oder die Kernspintomographie (MRT) eingesetzt, die bei einer malignen Infiltration von Lymphknoten, deren Aussendurchmesser noch nicht verÄndert ist, methodisch bedingt versagen mÜssen. In der vorliegenden Studie soil die Wertigkeit der in der Routinediagnostik neu eingefÜhrten Positronenemissionstomographie (PET) und der Somatostatinszintigraphie (Octreoscan) zum PrimÄrtumor- und Lymphknotenstaging bei Patienten mit gesichertem Plattenepithelkarzinom des Kopf-Hals-Bereiches der MRT und der diagnostischen Endoskopie gegenÜbergestellt werden. Als «Goldstandard» diente hierbei das Ergebnis der Endoskopie und der postoperativen histopathologischen Aufarbeitung. Es wurden 4 Laryngektomien und 4 Tumorresektionen durchgefÜhrt. Alle Operationen waren anhand der histologischen Aufarbeitung der OperationsprÄparate in sano erfolgt. Die MRT konnte das T-Stadium lediglich bei 4 Patienten richtig einordnen. Vier Patienten wurden falsch einem hÖheren T-Stadium zugeordnet. Auch die 18F-Fluordesoxyglukose (FDG)-PET zeigte eine korrekte Stadieneinteilung lediglich bei 2 von 8 Patienten. Bei 6 Patienten lag ebenfalls eine falsche, zu hohe Stadieneinteilung vor. Die Somatostatinszintigraphie ergab nur bei 2 Patienten eine korrekte Stadieneinteilung. Bei alien Übrigen Patienten wurde das T-Stadium als zu niedrig eingestuft. Von 197 histologisch aufgearbeiteten Lymphknoten waren 40 maligne befallen. Die MRT konnte das N-Stadium in nur 5 von 8 Fallen richtig einordnen. Drei von 8 Patienten wurden einem hÖheren N-Stadium zugeordnet. Die 18F-FDG-PET unterschÄtzte das N-Stadium lediglich in 1 Fall. Sieben von 8 Patienten wurden richtig eingeordnet. In der Somatostatinszintigraphie wurde das N-Stadium bei lediglich 1 Patienten richtig erkannt, bei 7 von 8 Patienten wurde das N-Stadium unterbewertet. In der korrekten Bewertung der T-Stadieneinteilung ist die MRT der 18F-FDG-PET Überlegen. MRT und PET fÜhren z.T. zu einer Überbewertung der T-Stadien. Das peritumorale Ödem mit vermehrter Durchblutung macht dies erklÄrlich. Die Anreicherung von markiertem Somatostatin in 2 grossen PrimÄrtumoren (T3, T4) erscheint in Anbetracht der gestÖrten DiffusionsverhÄltnisse unspezifisch. Die diagnostische Endoskopie und Palpation ist zur Beurteilung des PrimÄrtumors unverzichtbar. Hinsichtlich der N-Stadieneinteilung ergab die MRT wegen benigner LymphknotenvergrÖsserung bei der HÄlfte der Patienten eine falsche Stadieneinteilung. Bei der 18F-FDG-PET war lediglich 1 Patient mit einem kleinen ipsilateralen Lymphknoten (Durchmesser 0,9 cm) nicht erkannt worden. Alle anderen Patienten wurden in ihrem N-Stadium richtig zugeordnet.
