Resumo Este estudo investiga os fatores associados à adesão terapêutica em 150 idosos diabéticos assistidos em serviço gerontogeriátrico de natureza ambulatorial no Nordeste do Brasil. Dentre os idosos, 27,3% autorreferiram adesão integral à terapêutica. Na análise bivariada a adesão esteve associada com a autopercepção da saúde, crenças no uso dos remédios, entendimento das explicações sobre o diabetes e profissional responsável pelas orientações sobre o tratamento. Após análise ajustada, apenas as crenças no medicamento foram significativas ao comparar a não adesão com a adesão integral (OR = 9,65; IC95% 1,6;56,6) e a não adesão com a adesão parcial (OR = 18,15; IC95% 3,5;95,4). Conclui-se que a adesão integral ao tratamento para o diabetes é baixa e está associada às crenças nas medicações utilizadas para o controle da doença. Faz-se necessário o desenvolvimento de estudos adicionais para melhor definir o papel das crenças em saúde e as práticas de cuidados entre idosos assistidos na atenção básica de saúde.
This study aimed to assess the knowledge about diabetes, the attitude for self-care and associated factors through a cross-sectional study, the baseline of a randomized clinical trial with elderly diabetic in primary health care in Recife, Northeastern Brazil. We used the Diabetes Knowledge Scale (DKN-A) and Diabetes Attitudes Questionnaire . Of the 202 elderly, 77.7% had insufficient knowledge of the disease, especially for ketonuria, food replacement and were unaware of the causes and care of hypoglycemia. As for attitude, 85.6% had a negative psychological adjustment for diabetes. The logistic regression model showed that living alone was a protective factor (OR = 0.24; 95% CI 0.09-0.65; OR = 0.22; 95% CI 0.07 to 0.71), and low education, a risk factor (OR = 7.78; OR = 13.05; for the insufficient knowledge and the negative attitude for self-care, respectively. The findings reinforce the need for interdisciplinary educational actions that include socioeconomic, psycho-emotional and educational aspects in diabetes management to maintain elderly autonomy and functionality.
This is an integrative literature review which aims to identify the multi-professional scientific production on educational practices for individuals with diabetes available in the databases: Latin American Literature in Health Sciences (Literatura Latino Americana em Ciências da Saúde-LILACS), Medical Literature and Retrieval System online (Medline), Spanish Bibliographical Index in Health Sciences (Indice Bibliográfico Español en Ciencias de la Salud-Ibecs) and the Database on Nursing (Base de Dados em Enfermagem-BDENF), from 1999 to 2009. Results show that educational practices are developed mainly for adults and seniors up to 80 years of age, and involve themes that reflect the daily ife of living with diabetes. These practices are spread mainly through groups, bringing benefits not only for the individual with diabetes but also for the healthcare professional. Thus, we can see the process of changing the traditional education paradigm to a problem-based dialogical education, with a view for promoting health.
RESUMO:Estudo descritivo, exploratório, de abordagem qualitativa que objetivou identificar fatores que interferem na adesão ao tratamento hemodialítico na percepção dos pacientes renais crônicos. Participaram 35 pacientes assistidos por um serviço público de hemodiálise em uma capital do Nordeste do Brasil, com coleta das informações entre maio e agosto de 2014. As informações foram coletadas por meio de questionário semiestruturado e analisadas pelo software ALCESTE. Os principais fatores que influenciaram na adesão foram: dificuldade de transporte, déficit de conhecimento sobre a doença, limitações do tratamento, transtornos vivenciados nas sessões de hemodiálise, controle das taxas pelos exames laboratoriais, fé, máquina de hemodiálise e suporte social. Considera-se, portanto, que os fatores que interferem na adesão são complexos, tornando-se necessária atuação multidisciplinar para a compreensão desse fenômeno e o desenvolvimento de estratégias educativas com vistas à conscientização dos indivíduos sobre a importância da adesão ao tratamento e possível melhoria da qualidade de vida. APS. 2018; out./dez.; 21 (4): 516 -533 RESUMO Este artigo objetivou avaliar o grau de conhecimento e a aderência às atividades de autocuidado de indivíduos com diabetes mellitus tipo 2. Trata-se de estudo exploratório, quantitativo, com 176 indivíduos com diabetes assistidos na atenção primária à saúde e selecionados por amostragem não probabilística. Foi utilizada a versão brasileira dos questionários de Conhecimento sobre o Diabetes (DKN-A) e de Autocuidado em Diabetes (QAD). Na análise dos dados, foi utilizada a estatística descritiva, por meio das medidas de tendência central e dispersão e, na análise bivariada, os testes Qui-Quadrado de Pearson ou Exato de Fisher. Observou-se que os entrevistados possuem conhecimento sobre o diabetes, sendo influenciados pela idade e renda. Nas atividades de autocuidado, a maior aderência foi ao uso de medicação e a menor a monitorização da glicemia. Objective: To identify the prevalence of diabetes and its relation to associated factors in elderly persons receiving care at a specialist gerontology-geriatric outpatient service. A descriptive cross-sectional study of 301 elderly persons of both genders was performed. Method: The bivariate analysis was performed using the Pearson's Chi-square test for Independence and Fisher's exact test, considering 5% of significance and 95% confidence. A multivariate logistic regression model was adjusted to identify factors that may influence the occurrence of diabetes. Result: There was no statistically significant association between the prevalence of diabetes and socioeconomic and demographic variables, behavioral factors and health conditions. Only the Body Mass Index variable was close to being statistically significant ( p=0.059). Conclusion: The result confirms the need to develop new studies on the subject to consider the particularities and specificities of the elderly population to enable the creation of educational intervention strategies aimed at this group.
RESUMO Objetivo: analisar a prevalência e os fatores associados para a ocorrência de eventos adversos no serviço de hemodiálise. Método: estudo descritivo longitudinal com abordagem quantitativa, cuja amostra foi composta de 51 pacientes atendidos na unidade de hemodiálise entre maio e julho de 2016. Os dados foram obtidos por meio do registro diário de eventos adversos em um checklist e submetidos análise e, para as variáveis categóricas, foi realizada a análise bivariada, por meio do teste Qui-Quadrado de independência de Pearson. Resultados: foram registrados 1110 eventos adversos com prevalência em 98,03% dos pacientes, sendo os mais frequentes: erro de medicação (45,2%) e fluxo sanguíneo inadequado (42,9%). Quanto ao grau de dano, a maioria dos eventos foi de grau leve (93,8%), seguido do moderado (5,9%). A ocorrência do evento adverso esteve associada ao sexo feminino, idade menor que 40 anos, ausência de comorbidades, tempo de tratamento menor que um ano e uso de cateter venoso central de longa permanência. Conclusão: a prevalência de eventos adversos foi considerada alta, com predominância do grau leve. O estudo reforça a necessidade de estratégias de gestão e assistência para implantação de medidas de controle da ocorrência dos eventos adversos em hemodiálise, resultando em benefícios econômicos e sociais para a instituição e sua clientela.
Objective: To identify the instruments used to evaluate the functional health literacy of elderly persons with chronic kidney disease, and to assess whether they are suitable for this population. Method: An integrative review of the Lilacs, PubMed, Ibecs, Scopus and Cochrane databases was carried out in July and August 2018, seeking original articles dealing with functional health literacy and chronic kidney disease in the elderly. Results: A total of 15 articles were evaluated and seven different instruments used in the research were identified, with the Rapid Estimate of Adult Literacy in Medicine (REALM) and Test of Functional Health Literacy in Adults (TOFHLA) the most prevalent. Conclusion: The instruments identified are applicable for the elderly, but are not specific for this population. The construction and/or adaptation of new instruments is suggested considering the specific characteristics of this age group.
