We investigated the role of comorbid posttraumatic stress disorder (CO-PTSD) in substance use disorder (DSM-IV/ICD-10 substance abuse or dependence, SUD) treatment. We conducted a systematic review of treatment outcome studies in SUD patients with and without CO-PTSD from 2000 to 2011. The N = 22 studies found differed clearly with regard to methodology and the sufficiency of provided study information, limiting their comparability. Remarkably, no consistent indication of a negative effect of CO-PTSD on the reported SUD treatment outcome was found. In addition to CO-PTSD, we point out possible further effect modifiers which need to be better understood. Finally, we emphasize the requirements for future research such as methodological standardization and a focus on long-term studies.
Zusammenfassung. Fragestellung: Das Beratungsangebot [U25] Deutschland bietet Hilfe für Jugendliche und junge Erwachsene in Krisensituationen oder bei Suizidgefahr. Die Ratsuchenden bleiben anonym, die Beratung wird durch ehrenamtlich tätige Gleichaltrige (Peers) durchgeführt. Durch den Online-Zugang ist das Angebot besonders niederschwellig. Die vorliegende Untersuchung befasst sich mit der Frage, ob junge Menschen in Krisensituationen und bei Suizidgefahr durch die Online-Peerberatung [U25] erreicht werden und welche Merkmale die erreichte Klientel auszeichnen. Methodik: Datengrundlage bietet die Auswertung der Dokumentation von 1062 Beratungsprozessen aus dem Jahr 2017 sowie eine Interviewstudie mit Peerberaterinnen und -beratern des [U25]-Standortes Nürnberg (n = 13). Ergebnisse: Die Daten deuten darauf hin, dass das Ziel, junge Menschen in (suizidalen) Krisen zu erreichen, durch [U25] gut erfüllt wird. Dies spiegelt sich auch in der hohen Inanspruchnahme des Angebots wider. Allerdings nehmen junge Männer das Angebot weniger häufig wahr. Es liegen auch erste Hinweise darauf vor, dass junge Menschen aus bildungsfernen Familien weniger gut erreicht werden. Schlussfolgerungen: [U25] bietet einen vielversprechenden Ansatz, um junge Menschen in Krisensituationen niederschwellig zu erreichen und bei Bedarf zur Inanspruchnahme von Präsenzangeboten hinzuführen. Konzepte zur gezielten Ansprache von jungen Männern und Personen aus bildungsfernen Schichten müssen erarbeitet und evaluiert werden.
Ziele: Es werden Struktur- und Patientenmerkmale ambulanter und stationärer Einrichtungen der Suchtkrankenhilfe in Deutschland im Jahre 2007 und die Erreichungsquote von Personen mit Substanzstörungen in der Bevölkerung durch die Einrichtungen der Suchtkrankenhilfe untersucht. </p><p> Methodik: Die Auswertung basierte auf aggregierten Daten von 220.669 Patienten aus 720 ambulanten Suchthilfeeinrichtungen und 34.186 Patienten aus 147 stationären Einrichtungen in Deutschland. Bezogen auf alle Einrichtungen entsprach dies einer Erreichungsquote von 77 bzw. 60 %. </p><p> Ergebnisse: Ambulante Einrichtungen waren Anlaufstellen für ein breites Spektrum von Störungen. Patienten mit alkohol-, opioid- und cannabisbezogenen Diagnosen stellten sowohl im ambulanten (57 %, 19 % und 12 %) als auch im stationären (70 %, 14 % und 5 %) Bereich die größten Diagnosegruppen dar. Die Prozentwerte für planmäßige Beendigungen variierten erheblich, am höchsten lagen sie bei Patienten mit alkoholbezogenen Diagnosen (ambulant: 63 %, stationär: 83 %). Trendanalysen zeigen, dass die Anzahl der behandelten Patienten mit alkoholbezogenen Störungen und pathologischem Glücksspielverhalten seit 2002 leicht gestiegen ist. </p><p> Schlussfolgerungen: Auch wenn Parallelen zwischen der Versorgung von Suchtpatienten im ambulanten und stationären Bereich gezogen werden können, gibt es zahlreiche Unterschiede in den Patientenmerkmalen, der Behandlung und dem Behandlungserfolg. Hochrechnungen zeigen, dass nur ein kleiner Anteil der Personen mit einer Suchtproblematik betreut wird.
Zusammenfassung. Hintergrund: In Deutschland liegen nur vereinzelte Studien zur PTBS-Prävalenzquote bei Suchtpatienten und zu deren psychischen Befund vor. Fragestellung: Wie hoch ist die relative Häufigkeit einer PTBS bei Patienten mit substanzbezogenen Störungen und wie unterscheiden sich die Patienten mit und ohne PTBS hinsichtlich klinischer und psychosozialer Charakteristika? Methode: Mittels Chi-Quadrat- und t-Tests wurden 376 mittels Propensity Scores gematchte Patienten aus einer Stichprobe von 4105 konsekutiv aufgenommenen Abhängigen in diagnostischen und psychischen Merkmalen retrospektiv miteinander verglichen. Ergebnisse: Die relative Häufigkeit von PTBS lag bei den Patienten mit einer alkoholbezogenen Störung bei 3,8 %, bei den restlichen Suchtpatienten mit Störungen durch andere psychotrope Substanzen bei 10,5 %. Bei den PTBS Patienten lag häufiger eine Persönlichkeitsstörung vor. Außerdem waren die PTBS Patienten stärker psychisch belastet, in ihrem Interaktionsstil abweisender, introvertierter, und nachgiebiger sowie im Bindungsstil vermeidender. Schlussfolgerungen: Die Unterschiede verdeutlichen die Notwendigkeit von auf den Interaktions- und Bindungsstil individuell angepassten Interventionen im Rahmen der Standardbehandlungen für Suchtpatienten mit PTBS.
