Nursing leaders should use a validated instrument to investigate the quality of nursing records after implementation of standardised terminologies.
Objetivo: Identificar a frequência dos diagnósticos de enfermagem e características definidoras de pacientes com doenças cardiovasculares e caracterizá-los quanto às variáveis sociodemográficas e clínicas. Método: Estudo descritivo transversal realizado com 30 pacientes hospitalizados em um hospital de grande porte. Utilizou-se instrumento próprio validado para coleta de dados, que foram analisados por 5 peritos;, havendo concordância de 50%, sofreram análise estatística descritiva e inferencial. Resultados: Foram encontradas associações significativas com fatores Presença da Insuficiência Cardíaca, do Infarto Agudo do Miocárdio, da Dor, Sexo e Idade. Os diagnósticos mais frequentes foram: Ansiedade (76,7%), Dor Aguda (70,7%), Débito Cardíaco Diminuído (56,7%), Percepção Sensorial Perturbada - Visual (53,3%), Insônia (46,7%), Intolerância à Atividade (36,7%), Disfunção Sexual (36,7%) e Eliminação Urinária Prejudicada (36,7%). Conclusão: a descrição dos diagnósticos de enfermagem contribui para a análise das respostas à doença cardiovascular, com foco no objeto de trabalho do enfermeiro, apresentando respostas à doença cardiovascular por meio de investigação holística.
Objective: Identifying Nursing Diagnoses of fatigue, activity intolerance and decreased cardiac output in hospitalized patients with heart failure and verifying the association between the defining characteristics and the Nursing Diagnoses. Method: A longitudinal and prospective study that followed hospitalized patients with heart failure for three weeks. The data collected through interviews and physical examinations were sent to expert nurses for diagnostic inference. Descriptive and inferential statistical analyses were carried out. Results: Of the 72 patients, 68.0% were male and presented the nursing diagnosis of decreased cardiac output (62.5%) in the first week, reducing to 52.8% and 38% in the second and third weeks, respectively. Fatigue only appeared in one patient. Activity intolerance was the diagnosis that had the greatest discrepancy among the experts. Decreased cardiac output was associated to the defining characteristics: dyspnea, edema, jugular venous distension and reduced ejection fraction during all three weeks of evaluation. Conclusion: Decreased cardiac output was more prevalent in hospitalized patients with heart failure, and the associated defining characteristics were determining factors for this nursing diagnosis.
Trata-se de um estudo exploratório-descritivo e retrospectivo com os objetivos de identificar, a partir dos dados contidos nos prontuários, diagnósticos de enfermagem; e caracterizar esses diagnósticos através de um mapeamento utilizando a Taxonomia II da North American Nursing Diagnosis Association. A coleta de dados foi realizada em outubro e novembro de 2006, aplicando-se um roteiro, com oito perguntas fechadas, em todos os prontuários (196) existentes no Programa Interdisciplinar de Geriatria e Gerontologia da Universidade Federal Fluminense, Niterói - RJ. Os dados foram agrupados e organizados para tratamento estatístico descritivo simples. Obteve-se como principais diagnósticos de enfermagem: risco de integridade da pele, memória prejudicada e dentição prejudicada. Concluiu-se que o mapeamento realizado mostrou-se em concordância com a taxonomia diagnóstica, auxiliou na construção de um instrumento padrão e validou a necessidade do uso dessa tecnologia como um sistema padrão de linguagem.
Objective: To identify the NANDA International nursing diagnoses from the terms found in the nursing records of hospitalized children with congenital heart defects, and verify the association between these terms and the mapped nursing diagnoses. Methods: Observational and cross sectional study, developed by mapping of the terms in the nursing records of hospitalized children up to two years of age with congenital heart disease. The association between the terms and the most frequent nursing diagnoses were evaluated using the Student's t-test or chi-square. Results: The most frequent nursing diagnoses in the 82 records analyzed were: risk for infection (81.7%), impaired gas exchange (46.3%) and activity intolerance (36.6%). The terms "cyanotic" and "pallor" had significant associations with the diagnosis, impaired gas exchange. Conclusion:The terms recorded in the records of children with congenital heart disease allowed for the identification of NANDA International nursing diagnoses, in addition to verification of associations. ResumoObjetivo: Identificar Diagnósticos de Enfermagem da NANDA International a partir dos termos encontrados nos registros de Enfermagem de crianças com cardiopatias congênitas hospitalizadas e verificar associação entre estes termos e os Diagnósticos de Enfermagem mapeados. Métodos: Estudo observacional, transversal desenvolvido por mapeamento dos termos nos registros de Enfermagem de crianças hospitalizadas de até 2 anos com cardiopatia congênita. A associação entre os termos e os Diagnósticos de Enfermagem mais frequentes foi avaliada pelo teste t de Student ou qui quadrado. Resultados: Nos 82 registros analisados, os Diagnósticos de Enfermagem mais frequentes foram Risco de infecção (81,7%), Troca de gases prejudicada (46,3%) e Intolerância à atividade (36,6%). O termo "cianótico" e "hipocorado" tiveram relação estatisticamente significativa com o diagnóstico Troca de gases prejudicada. Conclusão: Observou-se que os termos registrados em prontuários de crianças com cardiopatias congênitas permitiram a identificação dos Diagnósticos de Enfermagem da NANDA International, além da verificação das associações.
O objetivo deste estudo foi comparar os diagnósticos de enfermagem identificados em adultos e idosos no pós-operatório. Foram recrutados aleatoriamente 20 adultos e 20 idosos internados na clínica cirúrgica de um hospital geral, em Niterói-RJ-Brasil, de janeiro a abril de 2009. Como instrumento de coleta de dados optou-se pelo Protocolo de Carpenito. Realizou-se análise estatística descritiva. Nos idosos identificou-se: dentição prejudicada (85%), constipação (75%), e déficit de autocuidado para vestir-se e arrumar-se (70%). Nos adultos: ansiedade (85%); déficit de autocuidado para vestir-se e arrumar-se (70%); e constipação (65%). Identificou-se 100% de risco de infecção e de integridade da pele prejudicada para ambos os grupos. Verificou-se uma proporção maior dos diagnósticos dentição prejudicada (p=0,008) e risco de quedas (p=0,000) em idosos, e privação do sono em adultos (p=0,028). Embora ambos os grupos estejam em pós-operatório, suas necessidades de cuidados mostraram especificidades, considerando as alterações do processo de envelhecimento. Objectives: to analyze treatment adherence in heart failure (HF) patients followed up by the nursing staff at specialized clinics and its association with patients' characteristics such as number of previous appointments, family structure, and comorbidities. Methods: a cross-sectional study was conducted at two reference clinics for the treatment of HF patients (center 1 and center 2). Data were obtained using a 10-item questionnaire with scores ranging from 0 to 26 points; adherence was considered adequate if the score was ≥ 18 points, or 70% of adherence. Results: a total of 340 patients were included. Mean adherence score was 16 (±4) points. Additionally, 124 (36.5%) patients showed an adherence rate ≥ 70%. It was demonstrated that patients who lived with their family had higher adherence scores, that three or more previous nursing appointments was significantly associated with higher adherence (p<0.001), and that hypertension was associated with low adherence (p=0.023). Conclusions: treatment adherence was considered satisfactory in less than a half of the patients followed up at the two clinics specialized in HF. Living with the family and attending to a great number of nursing appointments improved adherence, while the presence of hypertension led to worse adherence.
Heart failure (HF) is a common cause of hospitalization and requires accuracy in clinical judgment and appropriate nursing diagnoses. Objective: to determine the accuracy of nursing diagnoses of fatigue, intolerance to activity and decreased cardiac output in hospitalized HF patients. Method: descriptive study applied to nurses with experience in NANDA-I and/or HF nursing diagnoses. Evaluation and accuracy were determined by calculating effi cacy (E), false negative (FN), false positive (FP) and trend (T) measures. Nurses who showed acceptable inspection for two diagnoses were selected. Results: the nursing diagnosis of fatigue was the most commonly mistaken diagnosis identifi ed by the nursing evaluators. Discussion: the search for improving diagnostic accuracy reaffi rms the need for continuous and specifi c training to improve the diagnosis capability of nurses. Conclusion: the training allowed the exercise of clinical judgment and better accuracy of nurses.
