Краснопольский) Минздрава Московской области, Москва, россия Пренатальная диагностика патологических состояний является современным методом диспансеризации плода, позволяет определить медицинский и социальный прогноз для жизнеспособности и здоровья будущего ребенка. Поиск и практиче-ское использование знаний о достоверных пренатальных признаках частых врожденных заболеваний определяют эффек-тивность скрининговых программ. Основной целью настоящего исследования явилась оценка значимости известных и поиск новых ультразвуковых признаков, улучшающих диагностику частых хромосомных анеуплоидий для оптимизации скринингового обследования беременных в сроках беременности 11-14 нед. Материал и методы. Материалом для иссле-дования послужили результаты ультразвукового обследования 266 779 беременных, жительниц Московской области, за период 2011-2015 гг., проведенного в рамках раннего пренатального скрининга в сроки беременности 11-14 нед (строго при показателе копчико-теменного размера плода от 45 до 84 мм). Одновременно проводились биохимический скрининг для выявления патологических пренатальных маркеров и расчет индивидуального риска выявления хромосом-ных анеуплоидий на основе единого программного обеспечения. Результаты. Были выявлены 433 плода с трисомией 21, 166 плодов с трисомией 18, 64 плода с трисомией 13. Впервые в российской популяции установлены медианы известных пренатальных ультразвуковых признаков трех частых хромосомных анеуплоидий (трисомии 21, 18 и 13): толщины ворот-никового пространства, частоты сердечных сокращений и состояния носовой кости. Изучена распространенность ряда врожденных пороков развития и особенностей фенотипа плода. Заключение. В перечень дополнительных пренатальных ультразвуковых маркеров могут входить следующие: патология лимфатической системы и дефекты межжелудочковой перегородки при трисомии 21; врожденный порок сердца, микрогнатия, расщелины лица, патологическое положение кистей плода при трисомии 18; пороки развития центральной нервной системы, врожденные пороки сердца, пороки раз-вития и особенности строения лицевого черепа, полидактилия при трисомии 13. авторы информируют об отсутствии конфликта интересов. INTRODUCTIONThe first trimester screening in pregnancy is very effective in diagnosing chromosomal abnormalities and assessment of nasal bone is one of the effective secondary factors. Maternal age, Nuchal translucency (NT) and free β-hCG and PAPP-A (combined test) is found to have 85-95% detection rate for fetal aneuploidy with a false-positive rate of 5% whereas combined test along with nasal bone (NB) or tricuspid flow or ductus venosus flow has detection rate of 93-96% and false positive rate of 2.5% for euploid, 60% of trisomy 21, 53% for trisomy 18 and 45% for trisomy 13.1 Hence, NB absence or hypolplasia is a promising component of screening test.2 Ethnic differences in NB length have been reported. 3 To define NB hypoplasia, nomograms are set in Caucasian, African-American, Korean and Chinese populations. Nasal bone length of 323 choromosomal and structural normal fetuses at 11-14 weeks gestation were measured by transabdominal ultrasonography, prospectively. The correlation between nasal bone and other biometric parameters was assessed by regression analysis and average±SD of nasal bone lengths were measured.

Objective: The aim of the present study was to determine the correlation between fetal liver length (FLL) at the 11+0-13+6 week of pregnancy and crown-rump length (CRL).Methods: Pregnant women (n=63) referring to the Başkent University Konya Hospital Obstetrics and Gynecology Clinic between January and March 2014 for first trimester screening were included in the study. Patients were excluded for the following reasons: two for type II DM, one for metabolic disease, and three because they refused to share their information. FLL was measured in patients at the 11+0-13+6 week of gestation and in those who had normal ultrasonographic findings. Exclusion criteria were as follows: nuchal translucency of ≥3.5 mm, antenatal first trimester combined test results of ≥1/250, reverse flow of the ductus venosus and tricuspid valve, absence of nasal bone (NB), and fetal malformation in second trimester. Fiftyseven women who fulfilled inclusion criteria were included in the study. Statistical Package for the Social Sciences version 15.0 (SPSS, Inc., Chicago, IL, USA) was used for statistical analysis. Descriptive statistics and Pearson's correlation test was used for correlation between FLL and CRL.Results: Mean age of patients was 28. 18±4.7 (19-39). Mean gravida and parity were 2.05±1.2 (1-6) and 1±0.8 (0-5), respectively. Mean gestational age was 12.4 weeks (min: 11.1 -max: 13.6), and mean FLL of the study group was 11.2±1.1 mm (8.2-13.7 mm). Mean NT was 1.1±0.3 (0.5-2.4) mm. In the present study, it was established that FLL was correlated with CRL (r (57): 0.840; p<0.001).Conclusion: This study showed that there was a significant, strong, positive correlation between FLL and CRL at the 11 th -13 th week of pregnancy.
