Psoriasis is a chronic inflammatory skin disorder that may drastically impair the quality of life of a patient. Among the various modes of treatments for psoriasis, topical therapy is most commonly used in majority of patients. The topical formulations based on conventional excipients could serve the purpose only to a limited extent. With the advent of newer biocompatible and biodegradable materials like phospholipids, and cutting-edge drug delivery technologies like liposomes, solid lipid nanoparticles (SLNs), microemulsions, and nanoemulsions, the possibility to improve the efficacy and safety of the topical products has increased manifold. Improved understanding of the dermal delivery aspects and that of designing and developing diverse carrier systems have brought in further novelty in this approach. Substantial efforts and the consequent publications, patents and product development studies on the subject are the matter of interest and review of this article. However, majority of the work is related to the preclinical studies and demands further clinical assessment in psoriasis patients.

Liposomes are vesicles consisting of spherical phospholipid bi-layers with specific properties making them useful for topical application of drugs. Liposome research has expanded considerably over the last 30 years and nowadays, it is possible to construct a wide range of liposomes varying in size, phospholipids composition and surface characteristics to suit the specific application for which they are intended. In dermatology, the topical application of liposomes has proven to be of therapeutic value. Liposomes can be used as carriers for hydrophilic as well as lipophilic therapeutic agents because of their amphipathic character. They may improve stabilization of instable drugs by encapsulating them and serve as penetration enhancers facilitating the transport of compounds that otherwise cannot penetrate the skin. Liposomes help in reducing skin irritation by sustaining the release of drugs and by hydration of the epidermis. They also have the potential to target drugs into the pilosebaceous structures and hence they have an additional advantage for treatment of hair follicle-associated disorders. Clinical data indicate that 5-ALA encapsulated in liposomes improves the quality of Fluorescence Diagnosis by ALA-induced Porphyrins (FD) and optimizes the results of Photodynamic Therapy (PDT).

Nanotechnology and nanomedicine provide a platform for advanced therapeutic strategies for dermal and transdermal drug delivery. The focus of this review is on the current state-of-art in lipid-based nanotechnology and nanomedicine for (trans)dermal drug therapy. Drug delivery nanosystems based on the (phospho)lipid constituents are characterized and compared, with the emphasis on their ability to assure the controlled drug release to the skin and skin appendages, drug targeting and safety. Different types of liposomes, biphasic vesicles, particulate lipid-based nanosystems and micro-and nano-emulsions are discussed in more details. Extensive research in preclinical studies has shown that numerous parameters including the composition, size, surface properties and their combinations affect the deposition and/or penetration of carrier-associated drug into/through the skin, and consequently determine the therapeutic effect. The superiority of the most promising nanopharmaceuticals has been confirmed in clinical studies. We have selected several common skin disorders and provided overview over promises of nanodermatology in antimicrobial skin therapy, anti-acne treatment, skin oncology, gene delivery and vaccines.
