Detection and quantification of hepatitis E virus in the absence of IgG and IgM anti-HEV in HIV-positive patients

Abstract: Hepatitis E virus genotype 3 (HEV GT3) has been associated with silent chronic hepatitis and cirrhosis in HIV-positive subjects worldwide, but there is a lack of data on this co-infection in Brazil.

“…Similarly, another study that included 5 cases of acute HEV infection diagnosed by HEV RNA found that HEV IgM could only be detected at diagnosis in two subjects (Rivero-Juarez et al, 2015b), and three of these patients lacked HEV IgM positivity, with a CD4+ cell count higher than 200 cells/mm 3 . Similarly, in a study of 10 cases of acute HEV infection in Brazil, none of the patients presented with HEV HEV IgM, or HEV IgG antibodies (Salvio et al, 2018). Of the 9 patients with an available CD4+ count, only one showed a CD4+ cell count lower than 200 cells/mL (Salvio et al, 2018).…”

These data show that the lack of HEV IgM during the acute phase may not be limited to those patients with severe immunosuppression.…”

“…However, the associated risk factors are not well known. Currently, only two studies have assessed HIV-infected individuals (Ferreira et al, 2018;Salvio et al, 2018). The aim of the present study was to investigate the occurrence and seroprevalence of HEV infection and associated risk factors in HIV-infected individuals and blood donors from the southernmost part of the country, a region where the consumption of pork meat is widespread.…”

“…Molecular characterization of the HEV strains detected in humans in Brazil showed that all belonged to HEV-3 [ 32 , 33 , 40 , 45 , 53 ]. Further characterization of some of the strains identified subtypes 3b [ 33 , 40 ] and 3i [ 45 ].…”
