Current evidence suggests nebulised 3% saline may significantly reduce the length of hospital stay among infants hospitalised with non-severe acute viral bronchiolitis and improve the clinical severity score in both outpatient and inpatient populations.
La utilización de los servicios de salud está determinada por diversos factores. Con la finalidad de estudiar cuales son los factores más importantes y consistentes, fue realizada una revisión sistemática entre los años 1970 y 1999. Se encontró que la media de consultas, la proporción de personas que consultan y la proporción que concentra el mayor número de consultas fueron similares. Entre los factores demográficos, niños, mujeres en edad fértil y ancianos utilizan más los servicios. Entre los socioeconómicos, la clase social baja y el grupo con menos educación se asociaron con la utilización. El mayor uso está mediado por una mayor necesidad en salud. Pero, según del tipo de sistema, estos grupos menos favorecidos pueden recibir una insuficiente atención. La necesidad en salud es uno de los factores más importantes y, si se desea analizar la equidad del sistema, es necesario tener en cuenta el padrón de utilización entre los grupos sociales para el nivel de mayor necesidad en salud. Entre los factores relacionados a los servicios de salud, tener un médico definido determina una utilización más adecuada. Esto puede servir para atenuar las diferencias en la atención médica entre los diferentes grupos sociales. Finalmente, se propone una jerarquía para los factores relacionados.

Abstract
ObjectiveTo identify factors that lead people to visit a doctor in Brazil and assess differences between socioeconomic groups. Methods A cross-sectional study comprising 1,260 subjects aged 15 or more was carried out in southern Brazil. Demographic, socioeconomic, health needs and regular source of care data were analyzed concerning visits to a doctor within two months from the interview. Adjusted prevalence ratios and 95% confidence intervals were calculated using Poisson regression.
ResultsAdjusted PR showed that women having stressful life events, health insurance, and a regular doctor increased the outcome. A dose-related response was found with selfreported health, and the probability of visiting a doctor increased with health needs. Analysis in the chronic disease group revealed that uneducated lower income subjects had a 62% reduction in the chance of visiting a doctor compared to uneducated higher income ones. However, as it was seen a significant interaction between income and education, years of schooling increased utilization in this group.
ConclusionsResults suggest the existence of health inequity in the poorest group that could be overcome with education. Specific measures reinforcing the importance of having a regular doctor may also improve access in the underserved group.

BACKGROUND AND OBJECTIVE: The mainstay of treatment for acute bronchiolitis remains supportive care. The objective of this study was to assess the efficacy and safety of nebulized hypertonic saline (HS) in infants with acute bronchiolitis.
Estudo de base populacional com objetivo de avaliar a cobertura da assistência pré-natal, de acordo com a renda familiar, em município do Sul do Brasil. As informações foram coletadas por questionário, com as mães nas primeiras 24 horas após o parto. Observa-se que a cobertura pré-natal, o inicio das consultas no primeiro trimestre, a realização de seis consultas ou mais, a execução do exame das mamas e do exame especular, a realização dos exames de laboratório da rotina pré-natal do Programa de Humanização no Pré-natal e Nascimento (PHPN) e a realização da ultra-sonografia, aumentam com a melhoria da renda familiar, sendo mais realizados nas gestantes do quartil de maior renda (p < 0,001). Apesar destes resultados, a prevalência de baixo peso ao nascer não apresentou diferença estatística significante entre os diferentes quartis. O serviço local de saúde mostrou-se pouco efetivo porque apenas 26,8% dos pré-natais foram classificados como adequados, segundo parâmetros do PHPN, e desigual porque as pacientes do menor quartil de renda tiveram o acompanhamento da sua gestação com qualidade inferior à das pacientes do quarto quartil.
