Objective: To understand the contributing factors for the consolidation of the patient safety culture, from a management perspective, in an accredited hospital. Method: A qualitative study developed in a hospital institution of size IV, accredited by the National Accreditation Organization as level II, located in the northwest region of the State of Rio Grande do Sul/Brazil. The inclusion criteria were the following: having been in the leadership position of the institution for over a year and actively participating in the accreditation process. Leaders on vacation or absent due to illness in August 2018 were excluded. The collection was performed using the Focus Group technique in August 2018. Data were explored by thematic analysis. Results: The group reported teamwork, professional appreciation, management support, implementation of protocols, professional satisfaction, and working conditions as factors that contributed to the consolidation of the safety culture. Conclusions and implications for practice: The identified factors allowed for a cultural change in the institution through participatory management in processes and results that encourage workers to assume significant roles in advancing patient safety by assimilating and taking responsibility for change, which plays a crucial role in developing safe care.

Objective: to understand the development of the clinical reasoning of nurses working in an emergency hospital service. Methods: qualitative research, with methodological reference of the Grounded Theory. Data collection took place through interviews, with a theoretical sample of 21 participants and analysis using substantive and theoretical coding. Results: the central category was identified as Knowing what to do, and four conceptual categories: 1) Uncovering the nurse's duties in the emergency service; 2) Improving clinical reasoning through professional experience; 3) Providing care with quality and safety for the patient; and 4) Conquering professional recognition from the health team. Conclusion: the development of nurses' clinical reasoning is a cumulative, experiential, and gradual process of knowing what to do in an emergency. It is based on the collaborative work with the health team and on the acquisition of technical-scientific knowledge to perform qualified assistance to patients.

Resumo OBJETIVO Conhecer a cultura de segurança do paciente em unidades de terapia intensiva. MÉTODO Estudo qualitativo, exploratório-descritivo, realizado com cinco médicos, cinco enfermeiros e 24 técnicos de enfermagem atuantes em unidades de terapia intensiva de duas instituições hospitalares do sul do Brasil, uma pública e uma filantrópica. Realizaram-se entrevistas semiestruturadas no período de setembro e outubro de 2016, analisadas por meio da análise textual discursiva. RESULTADOS Emergiram duas categorias: Percepção sobre o erro e Gestão do erro. CONCLUSÃO Evidenciou-se que os profissionais reconhecem a existência do erro na assistência à saúde e atribuem a sua ocorrência a falhas individuais e do sistema organizacional, além de estimularem uma cultura não punitiva e o aprendizado coletivo.
