a b s t r a c tPurpose: To give a first description of the perception of late effects among long-term survivors after Allogeneic Haematopoietic Stem Cell Transplantation (HSCT) and to validate the German Brief Illness Perception Questionnaire (BIPQ). Methods: This is a secondary analysis of data from the cross-sectional, mixed-method PROVIVO study, which included 376 survivors from two Swiss HSCT-centres. First, we analysed the sample characteristics and the distribution for each BIPQ item. Secondly, we tested three validity types following the American Educational Research Association (AERA)Standards: content validity indices (CVIs) were assessed based on an expert survey (n ¼ 9). A confirmatory factor analysis (CFA) explored the internal structure, and correlations tested the validity in relations to other variables including data from the Hospital Anxiety and Depression Scale (HADS), the number and burden of late effects and clinical variables. Results: In total, 319 HSCT recipients returned completed BIPQs. For this sample, the most feared threat for post-transplant life was long lasting late effects (median ¼ 8/10). The expert-survey revealed an overall acceptable CVI (0.82), three itemseon personal control, treatment control and causal representationeyielded low CVIs (<.78). The CFA confirmed that the BIPQ fits the underlying construct, the Common-Sense Model (CSM) (c 2 (df) ¼ 956.321, p ¼ 0.00). The HADS-scores correlated strongly with the item emotional representation (r ¼ 0.648; r ¼ 0.656). Conclusion: According to its overall content validity, the German BIPQ is a promising instrument to gain deeper insights into patients' perceptions of HSCT late effects. However, as three items revealed potential problems, improvements and adaptions in translation are therefore required. Following these revisions, validity evidence should be re-examined through an in-depth patient survey.

Determinants of methotrexate adherence in rheumatoid arthritis patients

In rheumatoid arthritis (RA) patients, weekly intake of methotrexate (MTX) is the basic drug treatment. This observational study aims to investigate how many RA patients are adherent in terms of MTX intake and to identify determinants of non-adherence. Intake of MTX (orally or via injection) was recorded in 129 RA patients with an electronic monitoring system (MEMS ® ) during 16 weeks. In addition, two adherence questionnaires, the Medication Adherence Report Scale (MARS-5) and the Compliance-Questionnaire-Rheumatology (CQR) as well as a visual analogue scale (VAS) measuring MTX adherence, were administered to these patients. As possible determinants of adherence, data on demographics, disease and treatment characteristics, depression, illness cognitions, motivation, and social support were collected. Of all participants, 58 % were fully adherent and 75 % skipped at most one dose during 16 weeks. A better mental health status and suffering from comorbidities had a positive effect on adherence, while living alone had a negative effect. These three predictors explained 30 % of the variance in MTX adherence. Of the three self-report medication adherence measures, the VAS correlated the highest with the results of the electronic monitoring system (r = 0.552, p = 0.01). A relatively high adherence rate was observed in RA patients treated with MTX. The determinants identified by this study could be used to screen patients at risk for non-adherence. A simple VAS scale seems to be an acceptable way for a preliminary screening of MTX adherence.

RESUMO -O objetivo deste estudo foi proceder à análise fatorial exploratória da versão brasileira do Questionário de Percepção de Doenças Versão Breve (Brief IPQ). Participaram 325 pessoas adultas portadoras de enfermidades crônicas. A análise pelo método dos eixos principais, com rotação varimax, extraiu dois fatores: o Fator 1 (quatro itens; alfa de Cronbach = 0,80) denominado representação emocional e o Fator 2 (três itens; alfa de Cronbach = 0,52) nomeado representação cognitiva. Embora a solução fatorial encontrada no presente estudo tenha diferido, em parte, da proposta teórica do Modelo de Autorregulação de Leventhal, a versão brasileira do Brief IPQ pode ser considerada válida e confiável para avaliar a percepção de doenças no contexto de enfermidades crônicas.Palavras-chave: doença, doenças crônicas, percepção de doenças, análise fatorial, Brief IPQ Exploratory Factor Analysis of the Illness Perception Questionnaire Brief Version (Brief IPQ)ABSTRACT -The objective of this study was to conduct an exploratory factor analysis of the Brazilian version of the Brief Illness Perception Questionnaire (Brief IPQ). Participants were 325 adults with chronic illnesses. A principal axis factoring analysis with varimax rotation extracted two factors: Factor 1 (four items, alpha = 0.80) named emotional representation and Factor 2 (three items, alpha = 0.52) named cognitive representation. Although the factor solution found in the present study differed in part from the Leventhal's Self-Regulation Model, the Brazilian version of the Brief IPQ can be considered a valid and reliable measure to assess the perception of illness in the context of chronic diseases.
