Co-existing active pulmonary tuberculosis with aspergilloma in a diabetic patient: A rare case report

Abstract: Pulmonary aspergilloma is commonly associated with tuberculosis. Pulmonary aspergilloma is found in residual tuberculosis cavities and potential for other pathogens’ infections due to its sufficient oxygen and necrotizing tissue. A 48-year-old woman came with shortness of breath and cough for 7 months. She was diagnosed with pulmonary tuberculosis 9 months ago and was still under anti-tuberculosis drug therapy. “…In this country, due to the high incidence of tuberculosis, which is 301 per 100,000 population [6] , infection of tuberculosis mediastinal abscess was the first to be included in the differential diagnosis. Additionally, tuberculosis is known as a disease with many characteristics and may co-exist with another disease, such as aspergilloma [7] . Tuberculosis mediastinal abscess is a rare manifestation in immunocompetent adults.…”

“…According to the literature, pulmonary aspergillosis is often accompanies of tuberculosis, especially in residual tuberculous cavities [4,9], and type 2 [11] diabetes mellitus [1] is one of the most common premorbid backgrounds for Aspergillus invasion into the bronchi. It established, that hyperglycemia in diabetes mellitus contributes to the development of immunodeficiency [11], so the control of blood glucose levels is requirement for success in both the treatment of aspergillosis and its combination with other diseases [4,10,11].…”

“…The clinical picture of pulmonary aspergillosis is determined by the interaction between the fungus Aspergillus and the host [9]. Pulmonary aspergilloma is often associated with tuberculosis (TB) and is found in residual tuberculous cavities, as sufficient oxygen and caseous necrosis contribute its growth in this location [4].…”
