Aim. To analyze the effect of immunomodulator azoximer bromide on the serum cytokine profile (IL-2, IL-6, IL-4, IL-10) in the complex therapy for children with newly diagnosed tuberculosis (TB). Materials and methods. The study included 51 newly diagnosed TB children, who were divided into 2 groups: 26 children received immunomodulator azoximer bromide (main group) in combined therapy with concurrent antimycobacterial therapy (AMBT) and 25 children were assigned to only AMBT (control group). The main group children received azoximer bromide (immunomodulator) adjunctive therapy for immunological changes correction concurrently with the standard AMBT: children under the age of 10 years – orally 6 mg twice a day, children aged over 10 years – 12 mg twice a day; the treatment course – 14 days. The serum cytokine profile (IL-2, IL-6, IL-4, IL-10) was examined on the basis of the Immunological Department in the Training Medical and Laboratory Center of ZSMU by solid-phase enzyme-linked immunosorbent assay (ELISA) using a device ELISA – reader Sirio S with respective kits (Bender MedSystems GmbH, Austria, pcg/ml) before AMBT initiation and at the end of AMBT maintenance phase (MF). The study results were processed on a personal computer using the statistical package of the licensed program Statistica, version 13 (Copyright 1984–2018 TIBCO Software Inc. All rights reserved, License No. JPZ8041382130ARCN10-J). Results. At the AMBT initiation, newly diagnosed TB children of both groups showed a clear imbalance between pro- and anti-inflammatory cytokines towards pro-inflammatory ones with a predominance and high activity of Th1 type cellular immune response. The ratios of IL-2/IL-10 and IL-6/IL-10 pointed to the normalized balance in the regulatory system of pro- and anti-inflammatory cytokines in the main group children upon the MF of AMBT completion. The ratio of IL-2/IL-10 dropped 7.3-fold at the treatment completion relative to that at the initiation of treatment, however, compared to the control group at the end of treatment, it was 7.2 times less. The controls did not show significant on-treatment dynamics of the IL-2/IL-10 and IL-6/IL-10 ratios, the IL-2/IL-10 ratio was 12.8 times higher than in the healthy children group at the treatment completion. Conclusions. The use of immunomodulator azoximer bromide in combination therapy for newly diagnosed TB children helps to normalize all cytokine profiles and the balance in the regulatory system of pro- and anti-inflammatory cytokines after the AMBT completion. Therefore, the effect of immunomodulator azoximer bromide on the cytokine profile in the treatment for newly diagnosed TB children is substantial, which would increase the AMBT effectiveness among this patient group as a whole.
Aim. To substantiate the expediency of the immunocorrective therapy use in combined treatment by studying the effectiveness of the immunomodulator azoximer bromide aimed at correcting the immunological changes in children with new tuberculosis (TB) cases. Materials and methods. The study on the effectiveness of immunocorrective therapy in the complex treatment of children with new TB cases was conducted at the beginning of antimycobacterial therapy (AMBT) and at the end of the maintenance phase (MF) of AMBT. For this purpose, 51 children with new TB cases and immunologic changes were included in the study and divided into 2 groups: 26 children received immunomodulator azoximer bromide in the combined therapy on the background of AMBT (main group) and 25 children were assigned to receive only AMBT (control group). The groups were identical in age, sex, prescribed AMBT regimens, and severity of the specific process. The children from the main group along with the standard AMBT additionally used azoximer bromide (immunomodulator) to correct immunological changes: for children under 10 years of age – 6 mg twice a day, for children over 10 years – 12 mg twice a day; the treatment course – 14 days. The study results were processed on a personal computer using the statistical package of the licensed program Statistica, version 13 (Copyright 1984–2018 TIBCO Software Inc. All rights reserved, License No. JPZ804I382130ARCN10-J). Results. Given the results obtained, the use of immunomodulator azoximer bromide in the complex treatment for children with new TB cases, alongside normalization of all cytokine profile indicators and the balance in the regulatory system of pro- and anti-inflammatory cytokines, helped to achieve by the end of treatment: a shorter average time to culture conversion by 0.9 (1.5 (1.0; 2.0) months against 2.4 (2.0; 3.0) months; P < 0.01), positive radiological dynamics of 77 % (χ2 = 5.79; P < 0.01), reduced time of destruction healing by 2.1 (1.7 (1.0; 2.0) months against 3.8 (3.0; 4.0) months, P < 0.02), shorter average time of the basic AMBT course by 1.5 (6.2 (5.6; 6.8) months against 7.7 (6.0; 9.3) months; P < 0.01). Combined treatment tolerability was satisfactory in all 100 % of cases. Conclusions. Immunomodulator azoximer bromide as a part of the combined therapy for children with new TB cases can not only restore the body immune reactivity, but also reduce the specific process activity on the background of AMBT, shorten the average time to culture conversion by 0.9 months and destruction healing by 2.1 months, reduce the average duration of the main AMBT course by 1.5 months. In addition, this approach to therapy helps to conduct standard AMBT without changing the treatment regimen.
Особенностью течения туберкулеза в подростковом возрасте является склонность к прогрессированию, некротическим реакциям. Наиболее тяжелым исходом милиарного специфического процесса является развитие менингита, что нередко может быть его терминальной стадией. Цель работы — изучить особенности течения милиарного туберкулеза в сочетании с менингитом туберкулезной этиологии у подростка на примере клинического случая. Особенностями течения милиарного туберкулеза в сочетании с менингитом туберкулезной этиологии у подростка были: развитие побочных реакций на длительный прием комплексного противотуберкулезного лечения и гормонотерапии (токсический гепатит, болезнь Иценко — Кушинга вторичного генеза); осложнение туберкулезного менингита ишемическим инсультом в подкорковых отделах левой гемисферы большого мозга и нисходящей атрофией зрительных нервов положительная динамика при правильно подобранном режиме противотуберкулезной химиотерапии как проявлений милиарного туберкулеза, так и специфического менингита.
BACKGROUND. Tuberculosis (especially chemoresistant), coronavirus disease (COVID-19) and Churg-Strauss syndrome (CSS), both separately are serious illnesses, and in combination with each other (tuberculosis + COVID-19, COVID-19 + CSS). The analysis of the literature also indicates the difficulty of differential diagnosis between these diseases, as they have common clinical and radiological features. The outcome of treatment depends on timely and early diagnosis of each of these diseases with the prescription of corresponding therapy. We haven’t found in the available literature described cases of concomitant tuberculosis, COVID-19 and CSS, which may be interesting in terms of diagnostic vigilance of physicians of different specialties. OBJECTIVE. To demonstrate the features of the simultaneous course of tuberculosis and COVID-19 in a patient with CSS on the example of a clinical case of self-observation. RESULTS AND DISCUSSION. During the last 19 years of her life, the patient suffered from CSS. From the treatment she received only polcortolon, which was insufficient for such a serious disease. Whereas, according to the literature, for the treatment of eosinophilic granulomatous vasculitis not only glucocorticoids should be prescribed, but also immunosuppressants (cyclophosphamide for induction and azathioprine for maintenance therapy), mepolizumab and others. During these years, the patient developed lesions of the cardiovascular system (metabolic cardiomyopathy of ethanol and eosinophilic-granulomatous-vascular origin, myocardial fibrosis with arrhythmia, heart failure of 1st degree), gastrointestinal tract (chronic gastroduodenitis, peptic ulcer of the duodenum, gallstone disease, chronic calculous cholecystitis, chronic pancreatitis), urogenital system (chronic pyelonephritis), skin (autoimmune dermatitis, hemosiderosis), respiratory system (respiratory insufficiency of the 2nd degree, chronic allergic rhinosinusitis). COVID-19 and multidrug-resistant tuberculosis joined on the background of the depleted organism. COVID-19 treatment was effective. However, the treatment of multidrugresistant tuberculosis, which the patient received for 3 months, did not have a positive effect (negative radiological dynamics on the background of continued bacterial excretion). Renal dysfunction (creatinine >140 μmol/l) and peripheral eosinophilia also persisted during this period. According to the FFS lethal risk scale, the patient had two factors, which indicated a very severe flow of the disease and a high risk of death. Acute heart failure was the reason of death. CONCLUSIONS. CSS (eosinophilic granulomatous vasculitis) is a rare disease that requires timely, quality and proper treatment that will prevent the development of damage of various organs and body systems (especially cardiovascular and respiratory). The presented case demonstrates a very severe course of CSS with damage of many organs and body systems in the absence of proper treatment, which could lead to timely regression of vasculitis symptoms and improve the patient’s prognosis. On the background of this syndrome, COVID-19 and multidrug-resistant tuberculosis joined. However, death has occurred as a result of acute heart failure after 3 months, that was inevitable.
Мета роботи-встановити особливості перебігу ко-інфекції туберкульозу/ВІЛ у дітей і підлітків у Запорізькій області. Матеріали та методи. Здійснили аналіз історій хвороб та амбулаторних карток 24 дітей і підлітків. Результати. Ко-інфекція туберкульоз/ВІЛ частіше діагностована в дітей віком 1-4 роки (41,7 %). Переважали пацієнти з уперше діагностованим туберкульозом легень (83,3 %), без бактеріовиділення (79,2 %) та без деструктивного процесу (92,8 %). 95,8 % хворих на ко-інфекцію народжені від ВІЛ-інфікованої матері. ВІЛ-інфекцію від народження діагностовано в 45,8 %. У 58,3 % випадків діти не були щепленими вакциною БЦЖ. Контакт із хворими на туберкульоз встановлено в 66,7 %. Вірогідними результатами реакції Манту були негативний (33,3 %) та позитивний (58,4 %). У 66,7 % випадків туберкульоз діагностовано на тлі ВІЛ-інфекції дитини, тому середня тривалість ВІЛ-інфекції в пацієнтів вірогідно переважала в 5 разів. 91,7 % хворих отримували антиретровірусну терапію. 70,8 % хворих завершили лікування з залишковими змінами після перенесеного туберкульозу. 79,2 % хворих страждали на тяжкі супутні захворювання. У щеплених дітей середній вік довший у 2,7 раза, а кількість лімфоцитів CD4 + удвічі нижча. У нещеплених дітей переважала позитивна реакція Манту (92,9 %), а в щепленихнегативна (70 %). Тривалість стаціонарного лікування туберкульозу в нещеплених дітей на 1 місяць довша. Щеплені діти в 1,5 раза частіше завершували лікування з залишковими змінами після перенесеного туберкульозу. Висновки. На ко-інфекцію туберкульоз/ВІЛ частіше страждають діти віком 1-4 роки. 95,8 % хворих на ко-інфекцію народжені від ВІЛ-інфікованої матері, а ВІЛ-інфекція від народження дітям діагностована у 45,8 % випадків. АРТ отримували 91,7 % хворих. 79,2 % хворих страждали на тяжкі супутні захворювання. З боку туберкульозу переважали: вперше діагностований процес без бактеріовиділення та без деструкцій. Контакт із хворими на туберкульоз встановлено у 66,7 %, а курси хіміопрофілактики отримували тільки у 41,7 % випадків. У 66,7 % випадків туберкульоз діагностований на тлі ВІЛ-інфекції дитини. 70,8 % хворих завершили лікування з переведенням до категорії 5.1. Стерпність до лікування у 91,7 % випадків була задовільною. Для дітей, які щеплені вакциною БЦЖ, характерними особливостями були: довший у 2,7 раза середній вік, нижча вдвічі кількість лімфоцитів CD4 + , негативний результат проби Манту (70 %) та у 1,5 раза частіше завершення лікування з залишковими змінами після перенесеного туберкульозу. Для нещеплених-переважання позитивної реакції Манту (92,9 %) та довша на 1 місяць тривалість стаціонарного лікування туберкульозу. Ключові слова: ко-інфекція туберкульоз/ВІЛ, діти, підлітки.
