Characterization of the Running-in Period in Total Hip Resurfacing Arthroplasty: An in Vivo and in Vitro Metal Ion Analysis

Abstract: The simulator study allowed an exact characterization of the running-in period and showed a delayed onset of running-in wear. In contrast, the clinical data showed a slow increase in measured ion concentrations. These different wear patterns are probably due to the effects of distribution, accumulation, and excretion of particles and ions in vivo.

“…The titanium concentration of the serum was analysed using HR-ICP-MS (Element2, Thermo Fisher Scientific, Bremen, Germany) at intervals of 0, 1×10 6 , 2×10 6 , 4.5×10 6 , 7×10 6 and 10×10 6 cycles. In order to detect not only ions but also particles in the serum, the samples were first digested with high purity nitric acid (HNO 3 ) and hydrogen peroxide (H 2 O 2 ) following a standardised, well-established procedure [10]. The titanium mass was calculated based on the test medium's volume and the measured titanium concentration.…”

“…We included hips of several current-generation designs, all with monoblock cups and a large internal diameter ([ 38 mm) suitable for either resurfacing or large-diameter MOM THA. For the purposes of wear analysis, we required that all hips were implanted for a minimum of 12 months to minimize the confounding effect of the bedding-in process [18]. All patients revised with a diagnosis of infection were excluded to ensure all pseudotumors were sterile inflammatory masses.…”

“…This is supported by the experimental results presented by Wimmer (Wimmer et al, 2003), Liao (Liao et al, 2013;Liao et al, 2011), Yan (Yan et al, 2009), Hesketh (Hesketh et al, 2013) and Heisel et al (Heisel et al, 2008) who have all demonstrated that such films can reduce wear and corrosion, along with a stoichiometric release of metallic ions into the bulk environment. The formation of tribo-films within the articulation is -oxidation and improving lubrication.…”
