The study goals present an overview of Hospitalizations for Ambulatory Care Sensitive Conditions (ACSC) in Guarulhos, SP, from 2008 to 2012. This is an ecological study based on secondary data obtained from the Brazilian Hospital Information System, and supported by the Praxical Theory of Intervention of Collective Health Nursing. Applied descriptive statistics for analysis. It was observed that Guarulhos shows an upward trend in hospitalizations by ACSC (20% increase), the most frequent causes of heart failure (11.8%), cerebrovascular disease (10.6%) and angina (9.7%), most frequently in the age group ≥ 65years old, for both sexes. The results are similar to other Brazilian studies, but their analysis should extrapolate the biological limits and the supply of healthcare resources, focusing on the social determinants of the health-disease process.

Objective: Analyzing the network of care for the amputee from the perspective of healthcare professionals inserted in high complexity services. Methods: A qualitative research with data collection through semi-structured interviews with professionals from different areas involved in care of the amputee. Results: It was observed that a part of the interviewed professionals knows and acts in a perspective of referral to rehabilitation, while others are unaware of the flow of referrals, as well as of the care network that supports these users. Conclusion: The data showed the multidisciplinary and inter-institutional work occurring in some areas as the great fortress in the context of rehabilitation. The main weaknesses found were the lack of knowledge of high complexity professionals about the networks of health care, and the way rehabilitation is seen, not inserted in a context of care unless there is the possibility or occurrence of amputation. ResumoObjetivo: Analisar a rede de atenção à pessoa amputada na perspectiva dos profissionais de saúde inseridos nos serviços de alta complexidade. Métodos: Pesquisa qualitativa com coleta de dados mediante entrevista semi-estruturada com profissionais de diferentes áreas de atuação envolvidos no cuidado à pessoa amputada. Resultados: Observou-se que uma parte dos profissionais entrevistados conhece e atua em uma perspectiva de encaminhamento para reabilitação, enquanto outros desconhecem o fluxo de encaminhamento bem como a rede de atenção que ampara estes usuários. Conclusão: Os dados evidenciaram como grande fortaleza no contexto da reabilitação o trabalho multiprofissional e inter-instituicional ocorrido em alguns espaços. Como principal fragilidade aponta a falta de conhecimento dos profissionais sobre a alta complexidade das redes de atenção à saúde, assim como a maneira como a reabilitação é vista, não sendo inserida em um contexto de cuidado desde que exista a possibilidade ou ocorrência da amputação.

RESUMEN | La salud de las personas con discapacidad fue prioridad en la agenda del gobierno brasileño resultando en la formulación de políticas específicas para estos usuarios del Sistema Único de Salud (SUS). La creación de una red de atención a personas con discapacidad y el uso de la Clasificación Internacional del Funcionamiento, de la Discapacidad y de la Salud (CIF) se encuentran entre las normativas recién publicadas. Este estudio cualitativo examinó una propuesta de formación coordinada entre la Universidade Federal de Minas Gerais y la Secretaria Municipal de Salud, diseñada para ayudar a la reorientación de la Red de Rehabilitación de Belo Horizonte, Brasil, partiendo del modelo CIF. El objetivo fue conocer la percepción de los profesionales de salud con respecto a sus experiencias en los escenarios del SUS, en vista de los conocimientos adquiridos acerca del CIF como un posible cambio de las prácticas de trabajo diarias y la construcción de la Red de Salud. Se utilizó el análisis de documentos, la observación no participante y grupos de enfoque. El análisis de contenido nos permitió identificar dos ejes temáticos: la armonía entre el trabajo y la formación profesional, y la potencia del modelo biopsicosocial para la organización de la rehabilitación. Los resultados revelaron una brecha entre la formación académica y la realidad del trabajo y un proceso de transformación en la manera de concebir constructos importantes para la reorientación de la rehabilitación, como la funcionalidad, el contexto y la relación entre los mismos. El modelo de atención actual sigue deficiente para la plena coordinación de la atención, sin embargo, la motivación, el conocimiento de las políticas públicas y los cambios en la formación de profesionales para el SUS pueden facilitar la comunicación entre los agentes y servicios, favoreciendo el trabajo en red.
