Purpose Systemic review to compare selective laser trabeculoplasty (SLT) to other glaucoma treatment options in terms of their intraocular pressure (IOP)-lowering effect. Methods Searches of the following databases were performed: PubMed, Cochrane Central Register of Controlled Trials, Ovid, EMBASE, metaRegister of Controlled Trials, and ClinicalTrials.gov. Only randomised controlled trials (RCTs) published in peer-reviewed journals comparing SLT to other glaucoma treatment options were considered. The main outcome measure was the change in IOP from baseline. Results An initial search of PubMed identified 23 RCTs with 17 meeting the inclusion criteria. Nine RCTs compared 1801 SLT to 1801 argon laser trabeculoplasty (ALT) and one trial compared 3601 SLT to 3601 ALT, all reporting no difference in terms of IOP reduction from baseline. One RCT reported better outcomes with SLT at 1 year but this effect regressed at 2 years. Three trials compared 3601 SLT to medical therapy and found no difference between the two treatment options. One trial found greater IOP reduction with latanoprost vs 901 and 1801 SLT, and greater IOP reduction with 1801 and 3601 SLT versus 901 SLT, however no differences were found between 3601 SLT versus latanoprost or 3601 vs 1801 SLT. Two trials compared 1801 SLT to 3601 SLT finding no difference in IOP reduction. Two trials compared 1801 SLT to 901 SLT, one finding no significant difference and one finding greater IOP reduction with 1801 SLT over 901 SLT. One trial compared excimer laser trabeculotomy (ELT) to 1801 SLT, finding no differences in IOP reduction up to 3 months follow-up but greater IOP reduction with ELT at time intervals between 9 and 24 months. There were no RCTs identified that compared SLT to surgery. Conclusion In terms of the IOP lowering effect, there is no difference between SLT and ALT. Three trials indicate no difference between 3601 SLT and medical therapy, with one of the trials indicating greater IOP reduction with latanoprost over 901 and 1801 SLT. Three trials indicate no difference between 1801 SLT and 3601 SLT. It is inconclusive whether 901 is less efficacious than 1801 SLT. One trial reports greater IOP reduction with ELT over 1801 SLT in the long term.

Purpose: Reducing intraocular pressure (IOP) seems to be the only treatment that slows progression in glaucoma. The IOP can be decreased by pharmaceutical treatment, laser [selective laser trabeculoplasty (SLT)] treatment, or surgery. Prostaglandin analogues have been postulated to share action mechanisms with SLT and to possibly diminish the effects of SLT treatment. The aim of the current study was to investigate the effects of prostaglandin analogues in inflammation and IOP reduction after SLT treatment. Methods: Prospective nonrandomized study. One hundred and eighteen patients were included in the study. Inclusion criteria: Glaucoma (open-angle or pseudoexfoliation glaucoma) patients who will be treated with SLT. Inflammation was measured with a laser flare meter (Kowa FM-500). Measurements were made before SLT and then 2 h, 1 week, and 1 month after SLT treatment. IOP was also checked at the same time intervals. The SLT treatment was performed over 90°. All patients were divided into two groups: those receiving prostaglandins analogues and those treated with nonprostaglandin analogues. Results: Inflammation before and after SLT showed no significant difference between the groups at all the time intervals studied (t-test, before: P = 0.16; 2 h: P = 0.14; 1 week: P = 0.12; and 1 month: P = 0.36). IOP reduction showed no significant difference between the groups (t-test, P = 0.31). Conclusions: SLT treatment effects do not seem to be influenced by the use of prostaglandin analogues.

Amaç: Selektif laser trabeküloplasti (SLT) sonrası gözlenen erken dönem göz içi basıncı artışının değerlendirilmesi. Gereç ve Yöntem: Kliniğimize 01.11.2010 ve 31.10.2011 tarihleri arasında başvuran ve primer açık açılı glokom (PAAG) veya oküler hipertansiyon (OH) tanısı konan hastalardan primer tedavi SLT uygulanmış 100 hastanın 200 gözü dahil edildi. Hastaların yaş ve cinsiyet gibi demografik bilgileri, iris rengi, gonyolens ile açı pigmentasyonu ve açı dereceleri, pakimetri değerleri (optik koherens tomografi ile-OKT-), cup/disk oranı (indirekt oftalmoskopi ile) ve GİB ölçümleri (applanasyon tonometri ile) kayıt edildi. İris rengi, açı pigmentasyonu, pakimetri değerleri ve kullanılan toplam enerji ile uygulama sonrasında erken dönemde (1. ve 2. saatlerde) gözlenen ön kamara reaksiyonu ve GİB değişimleri arasındaki ilişki araştırıldı. Bulgular: Hastaların, %37'si (37) erkek, %63'ü (63) kadındı. Erkeklerin yaş ortalaması 61,24±11,33 (33-85), kadınların yaş ortalaması ise 58,21±12,35 (22-80) idi. SLT sonrası 1. ve 2. saat GİB artışı ve ön kamara reaksiyonları ortalamaları sırasıyla 3,35 mmHg ve 2,21 idi ve aralarında pozitif korelasyon gözlendi (p=0,0001). Tüm gözler, iris rengine göre incelendiğinde, kahverengi gözlerde izlenen GİB artışı (3,77 mmHg), yeşil ve ela gözlerde izlenenden (sırasıyla 2,62 ve 1,85 mmHg) anlamlı derecede yüksektir (p=0,0001). Erken dönemde gözlenen GİB artışları ile pakimetri ve kullanılan enerji arasında anlamlı bir ilişki bulunmazken açı pigmentasyonu ile istatistiksel anlamlı pozitif korelasyon gösterilmiştir (p1=0,904, p2=0,823, p3=0,0001). Sonuç: SLT sonrasında erken dönem GİB artışı koyu renkli ve yoğun açı pigmentasyonu olan gözlerde daha sık gözlenmektedir. Bu grup hastaların daha yakından takip edilmesinin önemli olduğuna inanmaktayız. ( Patients' demographic data, iris color, angle pigmentation, and angle degree measurements with goniolens, pachymetry measurements by optic coherence tomography (OCT), cup/disk measurements (by indirect ophthalmoscopy), and IOP measurements (by applanation tonometry) were noted. The relations between iris color, angle pigmentation, pachymetry and total energy with IOP spike and anterior chamber reactions observed at the 1st and 2nd hour of treatment were evaluated. Results: Thirty-seven percent of the patients were male and 63% of the patients were female. The mean age of males and females was 61. 24±11.33 (33-85) and 58.21±12.35 (22-80), respectively. Mean IOP spikes and anterior chamber reactions at the 1 st and 2 nd hour of the treatment were 3.35 and 2.21 mmHg, respectively, and they were positively correlated (p=0.0001). As the eyes were examined according to iris color, statistically significant higher IOP spikes were observed in brown eyes (3.77 mmHg) than in green (2.62 mmHg) and hazel eyes (1.85 mmHg) (p=0.0001).
