Objectives: to evaluate the effectiveness of a quality improvement cycle applied to the care of spontaneous demand in a primary care center. Methods: quasi-experimental before and after study, with a quantitative approach and no control group. An improvement cycle was carried out in a primary care center in the city of Guarabira/PB using five quality criteria. An evaluation, an intervention focused on the most problematic criterion and a reassessment were carried out. The samples were random (n = 60). The percentages and confidence intervals of compliance with each criterion were verified. Statistical significance was calculated using the Z test. Results: after the intervention, there was a significant improvement in two quality criteria of the care of spontaneous demand users (the user must be heard by a professional and go through the risk classification). Conclusions: the improvement cycle was an effective quality management method.

Objective: to understand the relationship of social determinants in health promotion for women living in a quilombola community, through Paulo Freire's Research Itinerary. Method: a qualitative study, with a participatory character, developed through Paulo Freire's Research Itinerary. Ten quilombola women aged between 24 and 54 years, living in the Morro do Fortunato Community, Garopaba, Brazil, participated in the study from April to June 2016. During the three-month-period, the thematic research, codification, decoding and critical unveiling were performed. Results: 20 generating themes were initially investigated which were coded and decoded in eight and unveiled in two significant themes, quilombola women and access, which, when unveiled, deepened the empowerment and the relationship with health promotion in this community. Conclusion: this study enabled women to be aware of their realities and consequent empowerment and fruitful reflections which provided an increase in self-esteem, appreciation and self-care, as well as the perception of their strengths, often muted by the adversities of the day to day.

Resumo: Evidências científicas apontam que um sistema fortemente baseado em Atenção Primária à Saúde (APS) promove melhores desfechos em saúde. No Brasil, a APS acontece, no sistema público, preferencialmente, no modelo de Estratégia de Saúde da Família. São considerados atributos essenciais da APS: o acesso, a longitudinalidade, a abrangência dos serviços e coordenação do cuidado. Esta pesquisa, com metodologia qualitativa, visou explorar as barreiras de acesso à APS na visão dos usuários que frequentam uma UPA (Unidade de Pronto Atendimento). Revelaram-se barreiras: distância de casa, demora na resposta da APS, compreensão sobre o funcionamento do sistema, acesso facilitado à UPA para problemas não urgentes e horário de funcionamento abreviado da Unidade Básica de Saúde (UBS). Há percepção de que a UPA tem um acesso mais facilitado do que a UBS, apesar de não ser, em teoria, a porta de entrada oficial do sistema.
