O poder de acessar a saúde: uma análise do acesso à saúde na etnia indígena Xukuru do Ororubá, Pesqueira (PE)

Saúde Colet., 2014, Rio de Janeiro, 22 (3): 275-80Resumo O artigo analisou o acesso à saúde indígena em Pernambuco após a criação do Subsistema de Saúde Indígena, hoje gerido pela Secretaria Especial de Saúde Indígena do Ministério da Saúde (MS), tendo como foco o povo indígena Xukuru do Ororubá. A partir da sociologia de Pierre Bourdieu, foi realizado um estudo de caso buscando caracterizar as "relações de poder" como fator dinâmico associado ao acesso, utilizando-se entrevistas, grupos focais, análise documental e observação direta. Identificou-se que essas relações nos campos micro e macro da saúde indígena constituíram elementos importantes na garantia do acesso à saúde. A posição ocupada pelos indígenas nesse campo, resultado de longos anos de relações conflituosas com a sociedade e o Estado, alterna entre a subalternidade e a busca de autonomia, aspectos constituintes do habitus Xukuru que os orienta frente às necessidades de acessar a saúde. O exercício do poder no acesso à saúde foi associado aos diferentes tipos de capital ativos no campo da saúde indígena. O capital cultural étnico surgiu ora como elemento facilitador, ora dificultador do acesso, dependendo dos fatores dinâmicos e do espaço de relações que o requereram.Palavras-chave: acesso aos serviços de saúde; sistemas de saúde; saúde de populações indígenas; poder. AbstractThe paper analyzes the access to indigenous health in Pernambuco after the creation of the Indigenous Health Subsystem, today managed by the Indigenous Health Special Secretariat of Ministry of Health, focusing on the Xukuru indigenous people of Ororubá. From the sociology of Pierre Bourdieu, a case study trying to characterize the "power relations" as a dynamic factor associated with access, using interviews, focus groups, direct observation and document analysis was conducted. It was found that these relationships at the micro and macro fields of indigenous health were important elements in ensuring access to health. The position occupied by the Indians in this field, the result of long years of conflicting relations with society and the State, alternates between the subalternity and the search for autonomy, constituent aspects of Xukuru habitus that guides them front the needs to access health. The exercise of power in access to health was associated with different types of capital assets in the field of indigenous health. The ethnic cultural capital emerged either as a facilitator, or complicating issue to the access, depending on dynamic factors and on the space relationships that have required it.
