The article is devoted to the analysis of various pathogenetic mechanisms involved in the formation of chronic brain ischemia. The authors discuss current trends in society, leading to a wide spread of cerebrovascular diseases: an increase in life expectancy and a constantly increasing level of psychosocial stresses. The paper considers the variety of clinical manifestations, course options, the pace of development of this pathology, diagnostic and therapeutic difficulties encountered by doctors treating this category of patients. Particular attention is paid to symptoms that prevent a full course of treatment and prevention of chronic cerebral ischemia, such as asthenia and impaired statolocomotor function. The leading pathogenetic mechanisms that play a role in the formation of these syndromes are discussed. Various approaches to the treatment of this pathological condition are considered. The possibility of using drugs with a multimodal effect is discussed. An analysis of a clinical case involving a patient with a long history of chronic brain ischemia is presented. The main factors that are obstacles to a full pathogenetically directed treatment are identified and ways to solve this therapeutic problem are given. The authors conclude that it is necessary to formulate a wellthought- out therapy strategy for this pathological condition, including timely, well-considered use of drugs with a neurotrophic, neuromodulating effect, which allows further measures, including non-pharmacological approaches. Particular attention is paid to the role of social support and physical activity for the prevention of cerebrovascular diseases.
Highlights. Bioabsorbable vascular scaffolds for percutaneous coronary interventions in multivessel coronaryartery disease patients and concomitant type 2 diabetes mellitus may increase the effectiveness of endovascular treatment and improve the long-term prognosis. The review article discusses recent data and evidences on the efficacy and safety of various stent generations for treating patients with coronary artery disease. The main complications commonly occurred following the implantation of drug-eluting stents in the long-term period are summarized, suggesting the rationale for the use of the next generation bioabsorbable vascular scaffolds in routine clinical practice. Bioabsorbable vascular scaffolds for endovascular treatment of patients with coronary artery disease show comparable efficacy compared with conventional drug-eluting stents. Particular attention is paid to the technical approaches of bioabsorbable vascular scaffold implantation, the use of intavascular imaging to control the implantation, as well as the timing of dual antiplatelet therapy in the postoperative period. The final term of the first-generation scaffold absorption, dual antiplatelet therapy, the effectiveness of bioabsorbable vascular scaffold-treated distal lesions and safety in patients with type 2 diabetes are highlighted. Future perspectives of using the next generation bioabsorbable vascular scaffolds that may improve the efficiency of percutaneous coronary interventions are presented.
The article is devoted to the discussion of possible mechanisms of the development of cognitive disorders, the onset and progression of neurodegenerative diseases and the level of sex hormones in women during perimenopause, a condition associated with endocrine changes caused by a decrease in the follicular mass of the ovaries, and, as a consequence, a decrease in the level of their steroid hormones (in particular, estrogen). It is known that estrogen has a neuroprotective effect, affects the mental and cognitive spheres. Reducing it in the perimenopausal period contributes to the appearance of vegetative symptoms (hot flashes, insomnia, mood changes, etc.), increases the risk of developing depressive-anxiety symptoms, cognitive disorders. In order to correct them and prevent the development of neurodegenerative diseases, it is possible to use hormone replacement therapy to prolong the neuroprotective effect of estrogens in a woman’s body.
Цель. Оценить эффективность эндоваскулярных вмешательств у больных с сахарным диабетом, оперированных с использованием биоабсорбируемых сосудистых эндопротезов и стентов, покрытых эверолимусом. Материал и методы. В исследование изначально было отобрано 143 пациента. В дальнейшем включены 125 пациентов, которые были рандомизированы на 2 группы. Пациентам I группы (n=57) имплантировались биоабсорбируемые сосудистые эндопротезы, а второй группы (n=68)-коронарные стенты, покрытые эверолимусом. Для контроля имплантации биоабсобируемых эндопротезов использовали оптическую когерентную томографию, которая выполнялась в конце вмешательства и в отдаленном периоде наблюдения. Отдаленные результаты прослежены через 12 месяцев после вмешательства. Результаты. Всего имплантировано 63 биоабсорбируемых сосудистых эндопротезов пациентам из I группы и 102 стента, покрытых эверолимусом, пациентам II группы. средний диаметр имплантируемых эндопротезов составил в I группе-2,88±0,06 мм, а во II группе-2,68±0,12 мм (р>0,05). Технический успех вмешательства составил 100% в обеих группах. суммарная частота серьезных сердечно-сосудистых осложнений в период госпитализации в обеих группах составила 3,5% и 2,94%, соответственно, (р>0,05). Отдаленные результаты прослежены у 41 пациента из I группы и у 52 пациентов из II группы. Через 12 месяцев выживаемость пациентов составила 100% в обеих группах. Частота нефатального ИМ среди пациентов I и II группы составила 4,9% и 3,8%, соответственно, (р>0,05). Причиной явилось прогрессирование атеросклеротического процесса в других артериях. Частота рестеноза, потребовавшего повторных вмешательств на целевом поражении, составила 2,4% и 1,9%, соответственно, (р>0,05). Позднего тромбоза в отдаленном периоде не наблюдалось ни в одной группе. Поздняя потеря внутреннего просвета сосуда по данным оптической когерентной томографии составила 0,14±0,19 мм и 0,12±0,23 мм, соответственно для I и II групп (р>0,05). заключение. Биоабсорбируемые сосудистые эндопротезы у больных ИБс с сопутствующим сахарным диабетом и преимущественным поражением средних и дистальных сегментов коронарных артерий, показали одинаковую эффективность и безопасность со стентами, покрытыми эверолимусом, через 12 месяцев после вмешательства.
