ЦЕЛЬ: оценить общую коморбидность у пациентов с сахарным диабетом (СД) 2 типа в сочетании с первичным гипотиреозом (ПГ) с использованием индекса коморбидности Charlson, выявить факторы формирования коморбидности при указанной патологии.МАТЕРИАЛЫ И МЕТОДЫ: обследовано 203 пациента с СД 2 типа, из них 1-ая группа (76 чел.) -СД 2 типа в сочетании с ПГ (аутоиммунный тиреоидит -45 чел., послеоперационный гипотиреоз -31 чел.), 2-ая группа (127 чел.) -СД 2 типа без тиреоидной патологии. Группы не различались значимо по основным характеристикам. Уровень тиреотропного гормона на фоне терапии левотироксином в 1-ой группе -5,51 [2,93;11,08] мкМЕ/мл. Проведено антропометрическое обследование, исследование углеводного, липидного обмена, альбуминурии (АУ), СКФ (CKD-EPI-creat,2009 и CKD-EPI-cys,2012), толщины интраабдоминального жира (ИАЖ) методом ультразвукового исследования, эндотелийзависимой вазодилатации (Δd) по методике D.Celermajer, суточное мониторирование артериального давления (СМАД). Для оценки общей коморбидности рассчитывали индекс коморбидности (ИК) по методу M.Charlson.РЕЗУЛЬТАТЫ: выявлено значимое повышение ИК в 1-ой группе -7,0 [5,0;9,0] в сравнении со 2-ой группой -5,0 [3,0;7,0] (р=0,00009) и ассоциация ИК с уровнем свободного Т4 в 1-ой группе: r= -0,2779 (p=0,0358). ИК не зависел от причины ПГ и был выше у женщин (значимо во 2-й группе). INTRODUCTION: The increasing number of patients with diabetes mellitus (DM) and chronic kidney disease (CKD) is one of the most pressing problems of modern medicine. In comorbid pathology a combination of type 2 DM and thyroid hypofunction the negative effect of hypothyroidism on carbohydrate metabolism, lipid metabolism, endothelial function, and glomerular filtration rate (GFR) is a risk factor for the formation and progression of diabetic nephropathy and CKD and requires further study. AIM: To identify risk factors and epidemiological peculiarities of CKD in type 2 DM in combination with thyroid hypofunction and to determine the possibility of using cystatin C levels for the evaluation of the kidney function in this pathology. MATERIALS AND METHODS: The prospective study involved 203 patients with type 2 DM undergoing inpatient treatment in the endocrinology department of the Ryazan Regional Clinical Hospital: group 1 (n = 76), type 2 DM combined with the primary hypothyroidism, and group 2 (n = 127), type 2 DM without thyroid pathology. Carbohydrate, lipid metabolism, albuminuria (AU), thyroid hormone spectrum, adipokines (leptin, plasminogen activator inhibitor-1, interleukin-6, and tumor necrosis factor-) were analyzed. The GFR was calculated using the CKD-EPI formula based on the levels of creatinine and cystatin C. Arterial pressure daily monitoring (APDM) was conducted, and intra-abdominal fat thickness was evaluated by ultrasonography. RESULTS: The incidence of kidney pathology in patients with type 2 DM was 52.22%. In group 1, there was a significant increase in the prevalence of CKD (64.47%, p = 0.006) and of normoalbuminuric CKD (NAU-CKD, 32.89%; p = 0.010). The risk of CKD development in patients with concomitant PH was more than twice that in patients without thyroid pathology with an odds ratio of 2.229 (95% confidence interval (CI) 1.2414.003) and that for NAU-CKD was 2.474 (95% CI 1.2674.833). Significant impairment of several metabolic parameters and individual APDM parameters was revealed in group 1 in comparison with group 2. The dependence of AU and GFR on the body mass index and of AU on the intra-abdominal fat thickness was noted. A negative relationship between GFR and leptin was revealed; in group 1, a correlation of interleukin-6 and thyrotropic hormone was found (r = 0.809, p = 0.001). With concomitant PH, cystatin C values were lower, and the GFR (CKD-EPI-cys) was reliably higher. CONCLUSION: Hypothyroidism is a risk factor for CKD development including NAU-CKD in type 2 DM. Obesity and hormonal activities of the intra-abdominal fatty tissue facilitate AU progression and GFR reduction. The use of cystatin C as a marker of the filtration function of the kidney in patients with hypothyroidism may lead to the underestimation of kidney function; thus, further investigation is required.

Рязань, Российская Федерация _____________________________________________________________________________ В представленном литературном обзоре освещены современные взгляды отечественных и зарубежных авторов на роль генетических факторов в развитии диабетической нефропатии при сахарном диабете 2 типа, уделено внимание роли генов-кандидатов, кодирующих компоненты ренин-ангиотензиновой системы (РАС)-гены ACE и AGT и факторы эндотелия-гены eNOS3 и EDN 1. Наиболее широко исследуется вариант, обусловленый инсерцией или делецией Alu-повтора в 16-м интроне гена ACE, а также полиморфизм, связанный с однонуклеотидной заменой М235Т в гене AGT. Клинической практикой доказано, что данные полиморфизмы РАС наиболее значимы. Проанализированы результаты многолетних исследований, выполненных в России и за рубежом, по выявлению ассоциаций между полиморфизмом генов, детерминирующих факторы эндотелия, и развитием и прогрессированием диабетической нефропатии (ДН) у больных сахарным диабетом 2 типа. По данным мета-анализа выявлена ассоциация трех полиморфизмов гена eNOS3 c развитием ДН: 4b/a, Т-786C и G984T. Это позволило сделать заключение, что ген eNOS3 играет роль в развитии ДН. Результаты исследований сцепленности гена EDN 1 с развитием ДН до настоящего времени остаются крайне противоречивыми и, вероятно, зависят от этнических характеристик выборки. Дальнейшее изучение молекулярных основ развития и прогрессирования патологии почек при сахарном диабете важно для формирования алгоритмов ранней доклинической диагностики, разработки эффективных профилактических мероприятий и персонализированной предиктивной терапии. The article presents the results of studying the characteristics of arterial hypertension and endothelial dysfunction in patients with diabetes mellitus type 1 and diabetes mellitus-type 2 in combination with primary hypothyroidism. We revealed the increased time index of diastolic blood pressure and disorders circadian blood pressure profile with high prevalence of non-dippersrate. Decrease of endothelium-dependent vasodilation and increased concentration of endothelin-1 were diagnosed. The analysis showed the correlation of endothelial function parameters with carbohydrate and lipid metabolism, blood pressure, insulin resistance, TSH level in patients with combined pathology.
Background. Chronic obstructive pulmonary disease and bronchial asthma, when combined in one patient, are characterized by a low level of control. Excess weight aggravates the course of obstructive diseases. The study of the features of this syntropy will improve the effectiveness of therapeutic measures. Aim. The study of the level of cytokines and carbonylated proteins in patients with a combination of bronchial asthma and chronic obstructive pulmonary disease with overweight and normal weight during an exacerbation. Material and methods. The study included 136 people: the first group a combination of bronchial asthma and chronic obstructive pulmonary disease (n=30), the second bronchial asthma (n=36), the third chronic obstructive pulmonary disease (n=29), the fourth volunteers without respiratory diseases (n=41). Each group was divided into two subgroups depending on the body mass index (less than 25 kg/m2 or 25 kg/m2 and more). The concentrations of interleukins-6 and -8, tumor necrosis factor in blood plasma were determined by enzyme immunoassay. The level of carbonylated plasma proteins was assessed spectrophotometrically. Statistical processing was performed in the Statistica 10.0 program using nonparametric criteria. The correlation of the studied parameters was assessed using the Spearman coefficient. Results. In patients with a combination of bronchial asthma and chronic obstructive pulmonary disease, statistically significant positive correlations between the levels of interleukins-6 and -8, tumor necrosis factor and carbonylated proteins were found 0.51, 0.59 and 0.55, respectively (p 0.05). Patients of the first group with overweight differed by 37.5% in higher levels of interleukin-6 compared with patients with body mass index 25 kg/m2. Conclusion. The intensity of systemic inflammation in patients with a combination of bronchial asthma and chronic obstructive pulmonary disease during exacerbation correlates with the intensity of oxidative damage.
