Рязань, Российская Федерация _____________________________________________________________________________ В представленном литературном обзоре освещены современные взгляды отечественных и зарубежных авторов на роль генетических факторов в развитии диабетической нефропатии при сахарном диабете 2 типа, уделено внимание роли генов-кандидатов, кодирующих компоненты ренин-ангиотензиновой системы (РАС)-гены ACE и AGT и факторы эндотелия-гены eNOS3 и EDN 1. Наиболее широко исследуется вариант, обусловленый инсерцией или делецией Alu-повтора в 16-м интроне гена ACE, а также полиморфизм, связанный с однонуклеотидной заменой М235Т в гене AGT. Клинической практикой доказано, что данные полиморфизмы РАС наиболее значимы. Проанализированы результаты многолетних исследований, выполненных в России и за рубежом, по выявлению ассоциаций между полиморфизмом генов, детерминирующих факторы эндотелия, и развитием и прогрессированием диабетической нефропатии (ДН) у больных сахарным диабетом 2 типа. По данным мета-анализа выявлена ассоциация трех полиморфизмов гена eNOS3 c развитием ДН: 4b/a, Т-786C и G984T. Это позволило сделать заключение, что ген eNOS3 играет роль в развитии ДН. Результаты исследований сцепленности гена EDN 1 с развитием ДН до настоящего времени остаются крайне противоречивыми и, вероятно, зависят от этнических характеристик выборки. Дальнейшее изучение молекулярных основ развития и прогрессирования патологии почек при сахарном диабете важно для формирования алгоритмов ранней доклинической диагностики, разработки эффективных профилактических мероприятий и персонализированной предиктивной терапии.
