Цель исследования – изучить особенности кожных проявлений COVID-19 у детей. Материалы и методы Проанализированы данные историй болезней 55 детей с подтвержденной инфекцией COVID-19. Из них 36 находились на стационарном лечении с диагнозом: внебольничная пневмония коронавирусной этиологии (вирус SARS-CoV-2) в детском инфекционном отделении ГБУЗ МО «МГКБ» г. Мытищи и 19 получали лечение амбулаторно по поводу ринофарингита и/или бронхита / трахеобронхита. Большинство пациентов имели среднюю степень тяжести (64% - 35 пациентов), лёгкую – 36% (20 пациентов). Дети были в возрасте от 8 месяцев до 18 лет. Вирус SARS-CoV-2 был идентифицирован методом полимеразной цепной реакции (ПЦР). Все пациенты получали стандартную терапию в соответствии с временными методическими рекомендациями, по показаниям – другие лекарственные средства. Результаты В 13% (у 7 детей) на фоне коронавирусной инфекции были отмечены уртикарные высыпания, в 5% (у 3 детей) – макуло-папулёзные. Зуд, как правило, был умеренно выражен. По данным анамнеза и наблюдений в динамике заболевания, экзантема у детей появлялась на сроке от 1-х до 8-х суток развития симптоматики COVID-19. Элементы сыпи сохранялись в течение от 1 до 7 суток. У 2 детей с наиболее выраженной экзантемой на фоне ринофарингита в отсутствии поражения лёгких отмечен длительный период значимых астено-вегетативных науршений. В одном случае в 1-е сутки экзантема была единственным проявлением коронавирусной инфекции. Выводы У 18% детей, наблюдавшихся с лёгкой и средне-тяжелой формами COVID-19, были выявлены кожные высыпания. Экзантема при коронавирусной инфекции у детей является одним из симптомов заболевания. Несмотря на более частое обнаружение уртикарной сыпи, прямой связи с аллергологическим анамнезом не выявлено.
Цель исследования – изучить особенности нарушений функции желудочно-кишечного тракта у детей, больных коронавирусной инфекцией, вызванной вирусом SARS-CoV-2. Материалы и методы Проанализированы данные историй болезней 36 детей, находившихся на лечении с диагнозом: внебольничная пневмония коронавирусной этиологии (вирус SARS-CoV-2 идентифицирован) в детском инфекционном отделении ГБУЗ МО «МГКБ» г. Мытищи. Большинство пациентов имели средне-тяжелую форму заболевания (96%), тяжелых форм не было. Возраст детей был от 8 месяцев до 17 лет. Всем детям в 1-й день госпитализации был проведен клинический осмотр, рентгенографии или компьютерной томографии (КТ) легких, анализы крови клинический и биохимический, коагулограмма, общий анализ мочи. Пациенты получали терапию в условиях стационара в течение 8-15 дней стационарного лечения. За этот период оценка клинического статуса проводилась не реже 1 раза в день, дополнительное обследование – не реже 1 раза в 7 дней, по показаниям – чаще. Вирус SARS-CoV-2 был идентифицирован методом полимеразной цепной реакции (ПЦР). Все пациенты получали стандартную терапию в соответствии с временными методическими рекомендациями, по показаниям – другие лекарственные средства. Результаты и обсуждение Желудочно-кишечные нарушения проявлялись у пациентов на 4-5 день у 23х детей (64%) и продолжалась от 3 до 5 дней. У большинства детей отмечали диарею (64%) и боли в животе (57%). Другие симптомы, свидетельствующие о дисфункции желудочно-кишечного тракта, были зарегистрированы у 67% детей. Выводы Желудочно-кишечные нарушения имеются у большинства детей, госпитализированных с внебольничной пневмонией, вызванной вирусом SARS-CoV-2. Симптоматика купируется при проведении курса основной терапии, показанной для лечения COVID-19, и дополнительной, рекомендуемой при острых кишечных инфекциях.
Objective of the work was to investigate the peculiarities of gastrointestinal tract disorders in children with coronavirus disease caused by SARS-CoV-2 virus.Materials and methods. We have analyzed the frequency, terms of emergence, duration and character of gastrointestinal disorders on the basis of medical records of 36 children receiving the treatment for community-acquired pneumonia of coronavirus etiology (positive for SARS-CoV-2 virus) in the children’s infectious disease department of Moscow State Clinical Hospital, Mytishchi. The majority of patients had moderate form of the disease (97 %), severe cases were not registered. The age range of the children – 8 months to17 years old. All patients were subjected to supplementary examination on Day 1 in accordance with the current recommendations. Children were treated in hospital settings for 8–15 days. Assessment of clinical status was performed at least once a day over the stated period, supplementary survey – at least once in a week, upon indications – more frequently. SARSCoV-2 virus was identified using polymerase chain reaction. All patients received standard therapy in compliance with advanced methodological recommendations, upon indications – other pharmaceutical products.Results and discussion. Gastro-intestinal disorders in 23 children (64 %) emerged on Day 4–5, in 8 children (22 %) – before the start of the treatment; they lasted 3–5 days. In most of the patients diarrhea (64 %) and stomachaches (57 %) were observed. 67 % of the children demonstrated other symptoms indicative of gastrointestinal dysfunction. The disorders were characteristic for most of the children hospitalized with community-acquired pneumonia caused by SARS-CoV-2 virus. The symptoms are contained when the main course of therapy is conducted, indicated for treatment of COVID-19, and additional therapy recommended for management of gastro-intestinal infections is performed.
