Endocrine ophthalmopathy (EOP) is a multidisciplinary problem at the intersection of endocrinology and ophthalmology. The patients presenting with this condition experience deficit of adequate medical aid due to the poor cooperation between ophthalmologists and endocrinologists. There are practically no specialized centres in this country where the patients with EOP could receive the combined treatment of this pathology including the surgical intervention. Taken together, late diagnostics and delayed seeking the efficacious medical assistance, the absence of stable compensation of the functional disorders of the thyroid gland, erroneous identification of the phase of the disease, and incorrect choice of the methods for its treatment, the lack of coordination and consistency in the actions of ophthalmologists and endocrinologists are responsible for the low effectiveness of EOP treatment. On the other hand, the absence of the unified approach to diagnostics and treatment of endocrine ophthalmopathy, the necessity of introducing the international experience gained in this field into the routine clinical practice and pooling efforts of representatives of different medical disciplines (endocrinologists, ophthalmologists, radiologists, endocrine surgeons, and neurosurgeons) created the prerequisites for the solution of the EOP problems and gave impetus to the development of the recommendations being proposed.
The proposed clinical recommendations discuss the modern approaches to diagnostics and treatment of the diseases with the clinical picture of thyrotoxicosis.
A total of 139 patients (278 eyes) presenting with Graves' disease (GD) and endocrine ophthalmopathy (EOP) were examined. The age of 35 men and 104 women ranged from 17 to 71 years. All of them were tested for the functional activity of the thyroid gland and underwent standard ophthalmologic examination; anti-TSH receptor antibodies were measured. Both the activity and severity of EOP were verified as recommended by the European Group on Graves' Orbitopathy (EUGOGO) It was shown that the frequency of detection of anti-TSH receptor antibodies and their titers in patients with GD and EOP depended on the activity of the intraorbital process and the severity of EOP manifestations. The functional state of the thyroid gland also influenced the level of anti-TSH receptor antibodies level during the active phase unlike that in the inactive phase. The in-depth analysis of the relationship between the level of anti-TSH receptor antibodies and clinical characteristics of either EOP (activity, severity, manifestation of selected clinical symptoms) or GD (thyrotoxicosis, euthyroidism, hypothyroidism) demonstrated the possibility to use these characteristics as the factors predicting the severity and outcome of EOP. Also, they may be helpful for the choice of a therapeutic strategy for the treatment of such patients.
The present study included a total of 22 patients (44 eyes/orbits) presenting with endocrine ophthalmopathy (EOP) and Graves' disease. All of them underwent routine ophthalmological examination. The activity and severity of EOP were verified in accordance with the EUGOGO recommendations. The MRI protocol included obtaining T1-, T2-weighted, and STIR sequence images of the orbits. quantitative measurement of signal intensity (SI) from extraocular muscles (EOM), and the signal intensity ratio of EOM to ipsilateral temporal muscle (SIR value). The study has revealed the relationship of the minimal, maximum, and average EOM SI with EOP activity and severity. Moreover, it demonstrated direct correlation between SIR and clinical activity of EOP. It is concluded that MRI of the orbits with STIR sequence in the patients suffering EOP provides the additional criteria for the verification of the activity and severity of the disease as well as for prognostication of the response to the immunosuppressive treatment.
ЦЕЛЬ: существующие терапевтические подходы к лечению эндокринной офтальмопатии (ЭОП) основываются на неспецифической иммуносупрессии глюкокортикоидами (ГК) и лучевой терапии орбит. При этом часть пациентов остаются резистентными к лечению. Целью настоящей работы явилось исследование динамики TGF-β1 и рецепторов цитокинов: sTNFα-R1, sTNFα-R2, sIL-2R на фоне проведения иммуносупрессивной терапии высокими дозами ГК как возможных предикторов эффективности лечения. Материалы и методы: в исследование были включены 49 пациентов (98 орбит) с БГ в состоянии эутиреоза и субклинического тиреотоксикоза и ЭОП в активной фазе, не получавших ранее лечения по поводу ЭОП. Определены концентрации цитокина TGF-β1, sTNFα-RI и sTNFα-R2, sIL-2R, антител к рецептору тиреотропного гормона (рТТГ), свободных фракций тироксина (свТ4) и трийодтиронина (свТЗ), ТТГв сыворотке крови. Выполнено ультразвуковое исследование щитовидной железы (УЗИ ЩЖ), мультиспиральная компьютерная томография (МСКТ)/магнитно-резонансная томография (МРТ) орбит. Пациентам была назначена иммуносупрессивная терапия высокими дозами ГК (метилпреднизолоном) в режиме пульс-терапии, в стандартной дозировке 4500-8000 мг с учетом тяжести и активности клинических проявлений ЭОП. Обследование проводилось через 3, 6, 12 месяцев от начала лечения. РЕЗУЛЬТАТЫ: через 3 и 6 месяцев от начала введения ГК резистентными к лечению оставались более 30% пациентов. У пациентов с положительной динамикой ЭОП уровень TGF-β1 существенно не изменился. У пациентов, резистентных к лечению ГК, уровень TGF-β1 достоверно снизился по сравнению с пациентами с положительной динамикой. Уровень sTNFR1, sTNFα-R2 существенно не изменился. Достоверных отличий уровней антител к рТТГ, тиреоидных гормонов у пациентов резистентных к лечению ГК и с положительной динамикой не отмечено. ВЫВОДЫ: иммуносупрессивная терапия высокими дозами метилпреднизолона в режиме пульстерапии показала высокую эффективностью и хорошую переносимость, при этом часть пациентов остаются резистентными к лечению. Более низкие показатели цитокина TGF-β1 исходно и в процессе лечения позволяют использовать TGF-β1 в качестве биомаркера активности процесса, эффективности лечения и прогноза заболевания. Активация TGF-β1, как фактора роста фибробластов, может способствовать развитию фиброза, косоглазия и диплопии
Радиойодтерапия (РЙТ) -наиболее эффектив-ный, безопасный и экономически выгодный метод лечения болезни Грейвса (БГ). Этот метод позволя-ет избежать травмирующей предельно субтотальной или тотальной струмэктомии, а также послеопера-ционных осложнений, трудно поддающихся меди-каментозному лечению. РЙТ не имеет альтернати-вы при тяжелых формах тиреотоксикоза с высоким риском оперативного лечения и непереносимо-стью лекарственных препаратов [1]. В медицин-ской практике РЙТ применяется более 60 лет. Од-нако продолжает активно дискутироваться вопрос о целесообразности применения РЙТ в лечении БГ с проявлениями эндокринной офтальмопатии (ЭОП). Целью РЙТ является разрушение тиреоци-тов, достаточное для ликвидации тиреотоксикоза и достижения стойкого гипотиреоза [2]. В то же вре-мя имеются сообщения, что в 15-37% случаев РЙТ провоцирует развитие или ухудшает течение ЭОП [3,4]. К сожалению, при проведении РЙТ по поводу БГ расчет терапевтической активности не гарантирует достижение стойкого эутиреоза и не предотвращает развития гипотиреоза; возможно также сохранение тиреотоксикоза после облучения.Цель настоящей работы -исследование те-чения ЭОП после РЙТ у пациентов с БГ в зависи-мости от наличия или отсутствия эффекта от РЙТ (пост радиационного гипотиреоза), а также опреде-ление факторов риска прогрессирования ЭОП по-сле лечения 131 I. The primary objective of the present work was to study the clinical course of endocrine ophthalmopathy (EOP) following radioiodine therapy (RIT) of Graves' disease (GD) and depending on its effect (development of post-radiation hypothyroidism).The secondary objective was to determine risk factors of EOP progression after radioiodine therapy. This prospective study included 38 patients (76 eyes) allocated to two groups. The patients of group 1 (n=19/38 eyes) presented with thyrotoxicosis at each visit and continued to use thyrostatic agents; those in group 2 (n=19/38 eyes) had hypothyroidism at its early stages (3 and 6 months) and were given substitution therapy with levothyroxin. The development of post-radiation hypothyroidism was shown to strongly influence the clinical course of EOP. In the patients of group 1, EOP remained active throughout the entire observation period (12 months) in the absence of appreciable variations of its integral severity index. In group 2, the same index decreased significantly, but active forms of EOP could be detected by the time of onset of hypothyroidism (6 months) (p=0.0000). After 12 months, the level of anti-TSH receptor antibodies in the patients of group 1 was significantly higher than in those of group 2 (10.8±8.3 and 2.9±2.0 respectively, p=0.0003). The regression rate of EOP symptoms following radioiodine therapy (RIT) of Graves' disease was a function of the efficacy of thyroid 131 I radioablation.
