BackgroundThere is increasing interest in the microbiome of the hepatobiliary system. This study investigated the influence of infection with the fish-borne liver fluke, Opisthorchis felineus on the biliary microbiome of residents of the Tomsk region of western Siberia.Methodology/Principal FindingsSamples of bile were provided by 56 study participants, half of who were infected with O. felineus, and all of who were diagnosed with gallstone disease. The microbiota of the bile was investigated using high throughput, Illumina-based sequencing targeting the prokaryotic 16S rRNA gene. About 2,797, discrete phylotypes of prokaryotes were detected. At the level of phylum, bile from participants with opisthorchiasis showed greater numbers of Synergistetes, Spirochaetes, Planctomycetes, TM7 and Verrucomicrobia. Numbers of > 20 phylotypes differed in bile of the O. felineus-infected compared to non-infected participants, including presence of species of the genera Mycoplana, Cellulosimicrobium, Microlunatus and Phycicoccus, and the Archaeans genus, Halogeometricum, and increased numbers of Selenomonas, Bacteroides, Rothia, Leptotrichia, Lactobacillus, Treponema and Klebsiella.Conclusions/SignificanceOverall, infection with the liver fluke O. felineus modified the biliary microbiome, increasing abundance of bacterial and archaeal phylotypes.
Проведен сравнительный анализ изменений билиарной системы у 565 пациентов, прооперированных в гепатологическом центре г. Томска, с альвеококкозом, эхинококкозом, хроническим описторхозом, осложненными механической желтухой. Определены три уровня окклюзии и соответствующие виды расширения желчных протоков в зависимости от вовлечения в процесс анатомических структур желчевыводящей системы печени. Выявлены и представлены дифференциальноотличительные ультразвуковые критерии билиарной гипертензии при паразитарных поражениях печени, что позволит повысить эффективность ультразвуковой диагностики данной патологии, выбрать адекватный способ хирургической коррекции и улучшить результаты лечения.Ключевые слова: альвеококкоз, эхинококкоз, описторхоз, билиарная гипертензия, расширение желчных протоков, механическая желтуха.The comparative analysis of the changes of biliary system in 565 patients with alveolar Hydatid disease, hydatidosis, chronic opisthorchiasis, complicated by chronic obstructive jaundice operated in the Hepatology Center of Tomsk. РЕЗЮМЕВ последние десятилетия отмечается увеличение числа пациентов с осложненными формами хронического панкреатита и кистами поджелудочной железы. Заболеванию подвержены преимущественно лица трудо-способного возраста, что придает этой проблеме социальное значение. В статье представлен современный взгляд на выбор метода хирургического лечения хронического кистозного панкреатита и оптимальных сроков его проведения, в зависимости от степени «зрелости» панкреатической кисты. Дается небольшой исторический экскурс относительно хирургических методов лечения кист поджелудочной железы.В настоящем обзоре литературы проведен детальный анализ как традиционных хирургических, так и современных малоинвазивных методов лечения панкреатических кист. Сделан акцент на радикальных методах лечения, в частности на дуоденумсохраняющих операциях, проведено их патогенетическое обоснование. Проблема выбора наиболее радикального и в то же время органосохраняющего метода, способствующего улучшению ближайших и отдаленных результатов, качества жизни и социально-тру-довой реабилитации, не потеряли своей значимости, а исследования, проводимые в этом направлении, весьма актуальны.КЛЮЧЕВЫЕ СЛОВА: киста поджелудочной железы, хронический кистозный панкреатит, хирургиче-ское лечение, дуоденумсохраняющие операции.
