Modern data on the role of vitamin B12, and its biologically active form holotranscobalamin in particular, in the development of anemia of pregnancy are presented. Information on the vitamin B12 metabolism and its coupling with the folic acid and homocysteine metabolism is supplied. The new automated immunochemiluminescent method for the active vitamin B12 determination is described. Reference intervals for the holotranscobalamin and related indices that can be used for the differential diagnosis of anemia of pregnancy are specified.
Материалы II Национального конгресса «Дискуссионные вопросы современного акушерства» и обучающего преконгресс-курса XI Всемирного конгресса по перинатальной медицине ■ Проведена оценка эффективности методов остеопатии при подготовке организма беременных к родам. В исследование было включено 65 беременных женщин с одноплодной доношенной беременностью, без тяжелых соматических и акушерских осложнений. В основную группу (n = 36) были включены женщины, у которых в качестве комплексной подготовки к родам наряду с медикаментозными средствами (спазмолитики, ламинарии, антигестагены, физиотерапия) применялись остеопатические методы. В группе сравнения (n = 29) применялись только медикаментозные средства. У рожениц, получивших комплексную подготовку к родам с использованием методик остеопатии, по сравнению с группой, получавшей только медикаментозную подготовку, отмечено увеличение скорости раскрытия шейки матки на 0,2 см/ч в латентной и активной фазах родов, достоверное снижение частоты аномалий родовой деятельности и гипоксии плода, и, как следствие, снижение в 3 раза частоты экстренного родоразрешения операцией кесарева сечения. Отмечено снижение частоты перинеотомий и разрывов шейки матки.■ Ключевые слова: остеопатия; подготовка к родам; зрелость шейки матки; осложнения родов. Актуальность В современном акушерстве проблема подготовки к родам является крайне важной, поскольку оптимальная биологиче-ская готовность шейки матки является залогом успешных ро-дов как спонтанных, так и индуцированных. МеТОДы ОСТеОПАТИИ В КОМПлеКСНОйМеханизм созревания шейки матки к родам до сих пор до конца не изучен. На сегодняшний день известно, что в про-цессе ее созревания происходит гормонозависимое биохими-ческое преобразование -деградация коллагеновых волокон, составляющих основу структуры шейки. Значительную роль в осуществлении этого преобразования играют простаглан-дины, в первую очередь Е 2 , релаксин, оксид азота, «лейкоци-тарные» факторы, цитокины. На основании этих знаний раз-работан целый спектр современных медикаментозных средств для подготовки шейки матки к родам, включающий антигеста-гены, простагландины Е 2 и Е 1 , β-адреномиметики с антагони-стами кальция, механические средства (ламинарии и специ-альные баллоны), спазмолитики [3]. Тем не менее, несмотря на широкое применение этих медикаментозных средств, проблема достижения полной биологической готовности ор-ганизма к моменту спонтанного начала родовой деятельно-сти или к сроку необходимого родовозбуждения еще далека от окончательного решения.Наличие незрелой шейки матки у беременных с доношен-ным сроком составляет, по данным разных авторов, от 16,5 до 38,7 % у первородящих и 3,5 % у повторнородящих [1, 8]. По литературным данным [1, 3, 5], при наличии незрелой или недостаточно зрелой шейки матки происходит преждевремен-ное излитие околоплодных вод в 57,2 % случаев, аномалии родовой деятельности, в частности развитие гипертонической дисфункции матки, отмечаются в 44,2 % случаев; увеличи-вается частота родовых травм, развития дистресс-синдрома плода, в результате чего возр...
A urinary tract infection (UTI) during pregnancy remains one of the most important problems of obstetrics, urology, and nephrology. Pregnancy as physiological process contributes to UTI. This is caused by urinary clinical signs changing during pregnancy, approaches to diagnosis and treatment, as well as to the risk of developing urological, obstetric and neonatal complications. This article analyzes contemporary literature over the past 10 years.
According to the modern concepts, symphysiopathia usually occurs in the second half of pregnancy as a result of mineral metabolism violations. This complication of pregnancy mainly manifested as a pubic bone diastasis, pain in the bone of the pelvis and of the lower limbs, and cramps in the calf muscles. There are clinical manifestations of the symphysiopathia; indicators of calcium, magnesium and phosphorus in the blood and daily excretion of these minerals in the urine compared with those which have healthy pregnant women in the second and third trimesters. There is histological and immunohistochemical evaluation of placentas of patients on background treatment. The vast majority of pregnant women with symphysiopathia found increase in urinary calcium and magnesium in the urine remaining in the normal performance of mineral metabolism. According to the studies, the preparation of magnesium, calcium, and the local ultraviolet irradiation of symphysis pubis may reduce the clinical symptoms of symphysiopathia. The use of calcium supplements together with magnesium does not lead to excessive deposition of calcium salts in the placental tissue and improve metabolic function of placenta.
Over the past few years, the prevalence of chronic kidney disease in pregnant women has been increasing rapidly, which is one of the most important problems of obstetrics, urology, and nephrology. The severity of the disease is assessed by a decrease in glomerular filtration rate (GFR). However, modern formulas of GFR give errors, therefore, it is necessary to search for new diagnostic markers of renal function in pregnant women. It is believed that elimination of cystatin C from the circulation by more than 99% is carried out by the kidneys. In an intact form, this molecule is not thought to undergo either tubular reabsorption or secretion. In this sense, cystatin C can be considered an almost ideal marker of GFR. This article discusses the possibilities of using cystatin C as a marker of renal function in pregnant women.
